This directory is for the BlueJourney PPO and BlueJourney HMO 21-county service area which includes the following counties: Adams, Berks, Centre, Columbia, Cumberland, Dauphin, Franklin, Fulton, Juniata, Lancaster, Lebanon, Lehigh, Mifflin, Montour, Northampton, Northumberland, Perry, Schuylkill, Snyder, Union and York. The network providers listed in this directory have agreed to provide you with your healthcare services. You may go to any of our network providers listed in this directory. Routine dental and routine vision are not covered for all BlueJourney PPO or BlueJourney HMO members. BlueJourney PPO and BlueJourney HMO member ID cards include the following indicators on the front of the member ID card if you have routine dental and/or routine vision coverage. BlueCross DentalSM BlueCross VisionSM Out-of-network providers are under no obligation to treat BlueJourney PPO enrollees, except in emergencies. For a decision about whether we will cover an out-of-network service, we encourage you or your provider to ask us for a pre-service organization determination before you receive the service. You may also refer to your Evidence of Coverage (EOC) for more information, including the cost sharing that applies to out-of-network services. If you receive a bill from an out-of-network provider for covered BlueJourney PPO or BlueJourney HMO services provided to you, don’t pay the bill. On behalf of Capital BlueCross, National Vision Administrators, LLC (NVA®) provides the network and assists in the administration of network management services for the BlueCross Vision benefits program. NVA is an independent company. Fitness Centers This chapter lists participating fitness centers in our service area that are part of the SilverSneakers® program contracted through Tivity Health, Inc. Silver Sneakers has a national network of contracted fitness centers that members can use. To find additional participating fitness centers please visit silversneakers.com. Fitness centers within this listing may cater to a specialized population, including but not limited to gender. Members are encouraged to contact the fitness centers to find out whether a facility center fits their needs.
