This form is used for scouts bsa, venturers and adults and is good for only one year. You are required to fill out sections A, B and C. Participants must bring the appropriate, valid form, with them to camp. A check of this form is a mandatory part of check in, no one may participate in any camp activity until his or her medical form is completed and turned in. We will be doing med checks on a unit basis. We encourage units to have all medical forms sorted and stored in one binder that will be turned in to our Medical Staff by the Unit Leader.
