PCN Patient Engagement Questionnaire – slide 13 PCN Update March 2021 PCN overview IIF Acronyms PCN = Primary Care Network ARRS = Additional Roles Reimbursement Scheme Primary care networks explained | The King's Fund IIF = Impact and Investment Fund IPCC = Integrated Primary and Community Care IH = Integrated Health SC = Social Care PCN Network Contract • All Primary Care Network’s (PCN’s) are required to comply with a Network Contract, called the ‘Directed Enhanced Service’, commonly known as the DES • The ‘DES’ is the basis of their work and runs April–March, with revisions each year • The key sections of the DES are: • Additional Roles Reimbursement Scheme (ARRS) – funding for 14 extra roles in primary care such as paramedic, dietitian, health coach, podiatrist, physiotherapist, mental health • Impact and Investment Framework (IIF) – 6 indicators that PCNs receive extra payment for based on their achievement, for example learning disability health checks, social prescribing referrals, influenza vaccination etc • Structured Medication Reviews (SMR) and Medicine Optimisation* • Extended Hours Access • Enhanced Health in Care Homes* • Early Cancer Diagnosis* • Social Prescribing • Patient Engagement *Have clear specifications for what a PCN is required to do. 4 specifications are due to follow in October 2021: anticipatory care (with community services), weight management, cardiovascular disease, health inequalities Patient Engagement – requirements • Must meet requirements under PCN’s core network practice’s primary medical services contracts by: ✓Engaging, liaising and communicating with the PCN’s patients in the most appropriate way; ✓Informing and/or involving them in developing new services and changes related to service delivery; and ✓Engaging with a range of communities, including ‘seldom heard’ groups Patient Engagement – examples Engaging, Informing liaising and and/or involving communicating them in with the PCN’s developing new patients in services and the most changes related appropriate way to service delivery Engaging with a range of communities, including ‘seldom heard’ groups Patient Engagement – how to?     APPENDIX A Policy for the provision of equipped play areas Aim The overall aim of the policy and strategy for equipped play areas is to promote the provision of good quality, stimulating and easily accessible play areas, appropriately located and distributed throughout the Borough, which are freely available, well maintained and without undue hazards. This will encourage young people to take part in challenging physical activity outdoors, to explore their surroundings and, through interaction with other young, people help them develop important social skills. Policy context Standards for the provision of areas for play and recreation have been incorporated into spatial plans for the Borough over a number of years; most recently in the Core Strategy and Development Control Policies of Local Development Framework adopted in February 2008. This provides the following policy context: Strategic Objective BC2: Promote social inclusion through equality of opportunity and equality of access to social, educational, health, employment, recreational, green space and cultural facilities for all in the Borough. Strategic Objective QL1: Provide high quality social, educational, leisure and sports facilities, excellent parks and green spaces, and a full range of cultural opportunities for meeting, worship, entertainment and celebration. Core Policy CP12 – Protecting and Enhancing Recreational Provision: The Borough Council will seek to maintain and enhance the provision of formal and informal recreation facilities, at appropriate locations, within the Borough including the designation of new local parks and gardens, country parks and other public open spaces. Core Policy CP16 – Promoting Social Inclusion: In considering proposals for new development, the Borough Council will promote social inclusion through equality of opportunity and equality of access for all to social, educational, health, employment, recreational, green space, community buildings and cultural facilities.
