Pronesti, [email protected] Abstract Spigelian hernias are uncommon and predominantly affect the abdominal wall. The incidence of Spigelian hernias after colonoscopy is even rarer with only one case outlined in the surgical literature. This is the case of a 66-year-old man who underwent routine colonoscopy and presented to the hospital with systemic inflammatory response syndrome (SIRS). A computed tomography (CT) scan demonstrated a Spigelian hernia in the location of a prior left ventricular assist device (LVAD) placement. This required surgical resection and resulted in a complicated post-operative course. This case offers a unique perspective on a rare colonoscopic complication not well represented in the literature. It offers the learning point of remaining vigilant for a rare, but potentially deadly, colonoscopic outcome. This case also illustrates the decision-making heuristic of availability bias. Abdominal wall hernias The classical surgical definition of a hernia is the protrusion of an organ or the fascia of an organ through the wall of the cavity that normally contains it. Risk factors for abdominal wall hernias include: obesity ascites increasing age surgical wounds Features palpable lump cough impulse pain obstruction: more common in femoral hernias strangulation: may compromise the bowel blood supply leading to infarction Types of abdominal wall hernias: Inguinal hernia Inguinal hernias account for 75% of abdominal wall hernias. Around 95% of patients are male; men have around a 25% lifetime risk of developing an inguinal hernia. Above and medial to pubic tubercle Strangulation is rare Femoral hernia Below and lateral to the pubic tubercle More common in women, particularly multiparous ones High risk of obstruction and strangulation Surgical repair is required Umbilical hernia Symmetrical bulge under the umbilicus Paraumbilical Asymmetrical bulge - half the sac is covered by skin of the abdomen directly hernia above or below the umbilicus Epigastric Lump in the midline between umbilicus and the xiphisternum hernia Most common in men aged 20-30 years Incisional hernia May occur in up to 10% of abdominal operations Spigelian hernia Also known as lateral ventral hernia Rare and seen in older patients A hernia through the spigelian fascia (the aponeurotic layer between the rectus abdominis muscle medially and the semilunar line laterally) Obturator A hernia which passes through the obturator foramen. It protrudes through a slit like defect in the anterior abdominal wall adjacent to the semilunar line. The hernia ring is formed as a well-defined defect in the transversus abdominis. Extraperitoneal fatty tissue surrounds the hernial sac and it passes through the transversus and the internal oblique aponeuroses. Then spreads out beneath the intact aponeurosis of the external oblique. Spigelian hernia is a rare entity and constitutes 0.12% of abdominal wall hernias. Due to its interparietal position, it is difficult to diagnose it clinically. It commonly does not cause a noticeable bulge in the abdominal wall. But in the present case a swelling was visible and the hernial content was palpable but no hernial orifice could be felt. Using a relatively rare case of an incisional port hernia, we demonstrate the ultrasound findings of a strangulated hernia causing a partial small bowel obstruction. A 46-year-old female presented four days following a laparoscopic surgery complaining of abdominal pain, nausea and lack of bowel movements. There was a palpable mass in the left lower quadrant under the 12mm trocar port incision. ED point-of-care ultrasound revealed herniated akinetic loops of bowel through her laparoscopy incision. This is the first case report to describe the use of point-of-care ultrasound for the diagnosis of a strangulated incisional port hernia at the bedside. Smith, MD, DMI, FRCS (General Surgery), Nikhil Pawa, MD, LLM, MSc, FRCS Introduction Large bowel obstruction (LBO) is defined as a surgical emergency where a mechanical inter- ruption (either complete or partial) occludes the flow of intestinal contents. 1 Understanding the varying etiologic causes of LBO is important for clinicians and surgeons when tailoring manage- ment to each patient. Knowledge of large bowel anatomy, embryology, and pathophysiology is vital when investigating and treating LBO. Many clinicians will have encountered patients with LBO on a ward or in the operating room and will appreciate the challenges posed by such presentations. Although less common than small bowel obstruction (25% of all intestinal obstructions 2 ) LBO poses more immediate risks in the form of perforation and subsequent peritonitis. Establishing the cause of an obstruction is paramount, given the high associated morbidity and mortality, 3 in order to facilitate the guid- ance of treatment. Recent studies highlight high morbidity and mortality rates of 42% to 46% and 13% to 19%, respectively, following operation. 3,4 LBO accounts for nearly 2% to 4% of all surgical admissions. 5 Colonic malignancy remains the most common cause of LBO, representing approximately 60% of cases. 3,6 Other prevalent etiolo- gies relate to adhesions, diverticulosis, hernia, inflammatory bowel disease (IBD), and volvulus. -

They are located between the muscular layers of the abdominal wall and can be easily overlooked because of abdominal obesity. Generally, they are difficult to diagnose because of their location and vague symptoms. The diagnosis has been considerably aided by the introduction of ultrasonography and Computed Tomography. Once the diagnosis is made operative management is indicated due to risk of incarceration. We report a 32 years old female patient from who presented with right upper abdominal pain associated with a swelling below the right subcostal margin. A diagnosis of Spigelian hernia and gallbladder stones was made. The patient underwent laparoscopic mesh repair and cholecystectomy. Her recovery was uneventful. The hernia was first described Klinkosch in 1764 [1]. Spigelian hernia is a rare abdominal hernia, occurring through the spigelianaponeurosis, it carries a significant risk of incarceration and strangulation. Most spigelian hernias occur below the level of the umbilicus close to the level of the arcuate line (inferior margin of posterior leaflet of rectus sheath within the abdomen), though they have being reported to occur above the level of the umbilicus [2]. -

Spigelian hernias are relatively uncommon, and represent only 2% of all abdominal hernias. Due to their overall rarity and evasive presentation, Spigelian hernias and their associated complications are not well described in literature. We report here a rare presentation of a Spigelian hernia with large and small bowel strangulated portion in hernia sac, necessitating emergent laparotomy with bowel resection and repair of the hernia defect. A spigelian hernia results from protrusion of a viscus or preperitoneal fat through a defect in the transversus abdominis muscle, it occurs laterally to the lateral border of the anterior rectus sheath along the semilunar line, where a transition from muscle to aponeurosis occurs. Spigelian hernia management is a challenge to general surgeons. Elective repair of uncomplicated Spigelian hernias can be performed both laparoscopically and by an open technique, with the former reported to be associated with a lower morbidity and shorter hospital stay [1,2]. However, an open approach is more feasible in emergent presentations associated with viscus incarceration, as this prevents undue delays and rapid reduction with possible revival of ischemic tissues, as well as reduction of the rate of iatrogenic bowel injury OPEN ACCESS during trocars insertion for the laparoscopic approach. -

They are situated between the muscular layers of the abdominal wall and can be easily overlooked because of abdominal obesity. The diagnosis has been considerably aided by the introduction of ultrasonography and computed tomography (CT). These hernias require surgical treatment. We report a 56-year-old male Bahraini who presented to Bahrain Defense Force Hospital Emergency department with severe abdominal pain and vomiting associated with mass in the right iliac fossa. A provisional diagnosis of Spigelian hernia was made and confirmed by CT scan. The hernia was operated and the defect was repaired. His recovery was uneventful. Bahrain Med Bull 2014; 36(2):101-102 INTRODUCTION Spigelian hernias are uncommon variety of hernias seen more in females than males1. The line which forms the transition from muscle to aponeurosis in the transversus abdominis muscle is the spigelian line. The part of the aponeurosis that lies between the semilunar line and lateral edge of the rectus abdominis muscle is called the spigelian fascia. Anteriorly, the semilunar line is reinforced by the aponeurosis of the external oblique muscle. Posteriorly, it is reinforced by the transversus abdominis muscle which is muscular almost to the midline in the upper abdomen. Thus, anatomically will prevent herniation and accordingly, it is rare above the umbilicus. Spigelian hernia is defined as a herniation of a sac of peritoneum or an organ, through a weak defect in the spigelian fascia. -

Introduction A hernia is a protrusion of abdominal content (preperitoneal fat, omentum or abdominal organs) through an abdominal wall defect. Anatomically the most important features of a hernia are the hernial orifice and the hernia (peritoneal) sac, if present. The hernial orifice is represented by the primary defect in the aponeurotic layer of the abdomen, and the hernial sac by the bulging peritoneum. The neck of the hernial sac is located at the hernial orifice. As the French anatomist Henri Fruchaud (1894-1960) already stated, hernias of the abdominal wall occur in areas where aponeurosis and fascia are lacking the protective support of muscles (Fruchaud, 1953). Most of these weak areas are anatomically present in the abdominal wall congenitally, others may be acquired during life, for example by surgery. The uncovered weak aponeurotic areas are subject to elevated intra-abdominal pressures and give way if they deteriorate or represent anatomic varieties. The common sites of herniation of the abdominal wall are the groin, the umbilicus, the linea alba, the semilunar line of Spigel, the diaphragm and surgical incisions. In addition, more exceptionally obturator hernias and hernias of the triangle of Petit are also encountered. Hernias can broadly be classified into congenital and acquired types. Congenital hernias typically occur at the groin, although they may be observed at other locations such as the umbilicus or diaphragm. Abdominal wall hernias represent a common issue in general surgical practice. The definitive treatment of all hernias, regardless of their origin or type, is surgical repair.
