Previously treated for ulcerative colitis, he had had a colectomy and had undergone Park’s procedure (construction of an ileal-anal pouch). At presentation, the patient’s main complaint was a 4-day history of abdominal pain and vomiting. He was dehydrated, with associated metabolic alkalosis. He also had a history of acute-on- chronic renal failure, diabetes and hypertension. On admission, plain abdominal radiographs (Figs 1 and 2) demonstrated signs of small-bowel obstruction, multiple radio- opaque stones (including one large one) in the region of the right hypochondrium, and intrabiliary gas. The patient therefore represented a classic case of Rigler’s triad, indicating ‘gallstone ileus’. An ultrasound scan of the abdomen confirmed the pneumobilia and multiple small stones in the gallbladder. Also there was no Fig. 1. In non-iatrogenic cases, gallstone migration occurs after formation of a biliary-enteric fistula. In fewer than 10% of patients with gallstone ileus, the impacted gallstones are located in the pylorus or duodenum, resulting in gastric outlet obstruction, known as Bouveret’s syndrome. Case presentation: We report an 86-year-old female who was admitted to hospital with a 10-day history of persistent vomiting and prostration. She was in hypovolemic shock at the time of arrival in the emergency department. Investigations revealed a gallstone in the duodenal bulb and a cholecystoduodenal fistula. She underwent surgical gastrolithotomy. Unfortunately, she died of aspiration pneumonia on the fourth postoperative day. Conclusion: This case shows the importance of considering Bouveret’s syndrome in the differential diagnosis of gastric outlet obstruction, especially in the elderly, even in patients with no previous history of gallbladder disease. Fact Sheet News from the IBD Help Center INTESTINAL COMPLICATIONS The complications of Crohn’s disease and ulcerative colitis are generally classified as either local or systemic. The term “local” refers to complications involving the intestinal tract itself, while the term “systemic” (or extraintestinal) refers to complications that involve other organs or that affect the patient as a whole. Intestinal complications tend to occur when the intestinal inflammation: • is severe • extends beyond the inner lining (mucosa) of the intestines • is widespread • is chronic (of long duration) Some intestinal complications occur in both ulcerative colitis and Crohn’s disease, although they may occur more commonly in one than in the other. Not everyone will experience these complications. However, early recognition and prompt treatment are key. If you notice a change in your symptoms, be sure to contact your doctor immediately. Ulcerative Colitis-Local Complications Perforation (rupture) of the bowel. Intestinal perforation occurs when chronic inflammation and ulceration of the intestine weakens the wall to such an extent that a hole develops in the intestinal wall. This perforation is potentially life- threatening because the contents of the intestine, which contain a large number of bacteria, can spill into the abdomen and cause a serious infection called peritonitis. In colitis, this complication is generally linked with toxic megacolon (see below). In Crohn’s disease, it may occur as a result of an abscess or fistula. Fulminant colitis. This complication, which affects less than 10% of people with colitis, involves damage to the entire thickness of the intestinal wall. When severe inflammation causes the colon to become extremely dilated and swollen, a condition called ileus may develop. -

This infrequent cause of small bowel obstruction is usually compounded by multiple medical conditions that generally requires a complex surgery. In the population of patients over 65 years old, 25% of non-strangulated small bowel obstructions are caused by gallstone ileus. Due to the delayed diagnosis, mortality is high nearing one in five cases. In this case report, we address if prophylactic cholecystectomy would prevent gallstone ileus. An 83-year-old female with unknown past medical history presented to a rural emergency room with abdominal pain for the past three days. She had associated nausea and vomiting with localized pain to the right upper quadrant. There was no hematochezia, melena, or hematemesis. A CT of the abdomen revealed evidence of a large stone in the duodenum, ileus, air in the gallbladder fossa and no visualization of the gallbladder. Subsequently, she was transferred to another facility for higher level of care with admission to the intensive care unit (ICU). Vitals upon admission to the ICU reveal blood pressure 70/26, heart rate 103, respiratory rate 22, oxygen saturation of 93% on room air, temperature 98.1 Fahrenheit. Labs: WBC 10.0, Hgb 14.7, Hct 45.0, Plt 388, Sodium 133, Potassium 4.5, Chloride 98, Carbon Dioxide 17, BUN 43, Creatinine 2.20, Glucose 148, Calcium 10.2, Total bilirubin 0.9, AST 36, ALT 32, Alkaline Phosphatase 165, Total protein 7.1, Albumin 4.0, Lipase 381.
