WORK EXPERIENCE

June 2003-Present Day Private Practice, General Psychiatry My general practice includes evaluating and treating individuals with mental illness in a private setting. Treatment includes medication management and therapy. I provide treatment for individuals with ages ranging from young adults to older adults. I also provide treatment for individuals with developmental disabilities. Many of the patients I have treated suffer from anxiety and depression related to bereavement, as well as stress from interpersonal and family hardship.

June 2003-Present Day Private Practice, Forensic Psychiatry I am a Member of Court Panels in Santa Clara and Santa Cruz , and I have performed court- ordered evaluations for determination of competency, determination of sanity, and assessment of sexual offenses since 2003. I was a member of Court Panels in San Mateo, Alameda and Napa counties from 2004 to 2006.

In 2007, under my direction, a court clinic in the Santa Clara County Superior Court was initiated in Department 30. The purpose of the clinic is to be able to perform court-ordered psychological evaluations in a timely manner, and enables the court-ordered evaluator to perform evaluations at the time of judicial appointment, next to the courtroom. This clinic has been determined as successful, and continues to the present day.

Frequently I am called upon to perform fitness for duty evaluations and threat assessments of individuals in the workplace. Both types of evaluations require an assessment of mental illness in the employee, and an establishment of their imminent risk of harm to themselves or others based on their presentation of mental illness. I provide a report detailing my diagnosis, prognosis of the employee, overall risk of harm, and necessary intervention required for the goal of returning the employee to work.

I also perform evaluations at the request of private attorneys for issues in civil and criminal court. I am a Qualified Medical Examiner for the State of California Workers’ Compensation, and I perform psychiatric evaluations related to work-related injury. I perform Independent Medical Evaluations related to disability and psychiatric issues regarding vocational rehabilitation. I am a member of the Bar Association, and I frequently perform evaluations related to mental- health issues related to estate planning and testamentary capacity.

Greene, MD Page 3 of 7 September 2010-February 2014 Medical Director, Summit Estate Recovery Center, Los Gatos, California I was in charge of all medical and psychiatric treatment decisions for Summit Estate, directing medical and psychiatric care for individuals admitted to the program for substance issues. I provided administration of Subutex for opiate withdrawal while at the Center.

July 2004-March 2009 Consulting Psychiatrist, Agnews Developmental Center, San Jose, California Prior to the closing of the inpatient facility at Agnews, I was consulting with the treating physicians, to help with treatment of clients with developmental disabilities. I was a Board Member of both the Human Rights Committee and the Behavioral Management Committee, committees that met to review treatment plans of clients at Agnews, to ensure proper behavior management.

July 2003-February 2006 General Psychiatrist, Alliance for Community Care San Jose, California As a general psychiatrist, I treated patients in an outpatient setting. I was involved in medication management, brief therapy, and patient education.

March 2003-March 2005 General Psychiatrist, Medical Center San Jose, California As a general psychiatrist, I was treating patients in an outpatient clinic setting. I was involved in medication management and patient education.

July 2002-March 2003 Attending Psychiatrist, Napa State Hospital Napa, California As an Attending Psychiatrist, I was responsible for evaluation and treatment of patients found Not Guilty by Reason of Insanity. I was involved with patient education of medications and forensic issues.
