411 Ipsilateral Subclavian Steal in Association with Aberrant Origin of the Left Vertebral Artery from the Aortic Arch John Holder1 Five cases are reported of left subclavian steal syndrome associated with anomalous Eugene F. Binet2 origin of the left vertebral artery from the aortic arch. In all five instances blood flow at Bernard Thompson3 the origin of the left vertebral artery was in an antegrade direction contrary to that usually reported in this condition. The distal subclavian artery was supplied via an extensive collateral network of vessels connecting the vertebral artery to the thyro cervical trunk. If a significant stenosis or occlusion is present within the left subc lavi an artery proximal to the origin of the left vertebral artery, the direction of the bl ood fl ow within the vertebral artery will reverse toward the parent vessel (retrograde flow). This phenomenon occurs when a negative pressure gradient of 20-40 torr exists between the vertebral-basilar artery junction and th e vertebral-subc lavian artery junction [1-3]. We describe five cases of subclavian steal confirmed by angiography where a significant stenosis or occlusion of the left subclavian artery was demonstrated in association with anomalous origin of th e left vertebral artery directly from the aortic arch. In all five cases blood flow at the origin of the left vertebral artery was in an antegrade direction contrary to that more commonly reported in the subclavian steal syndrome. TOPOGRAPHY OF THE MIDDLE MENINGEAL ARTERY AND ITS BRANCHES Middle meningeal artery (MMA) [1] is most commonly the strongest branch of maxillary artery (from external carotid artery) [2]. It supplies blood to cranial dura mater, and through the numerous perforating branches it nourishes also periosteum of the inner aspect of cranial bones. It enters the middle cranial fossa through the foramen spinosum, and courses between the dura mater and the inner aspect of the vault of the skull. Next it divides into two terminal branches — frontal (anterior) which supplies blood to bones forming anterior cranial fossa and the anterior part of the middle cranial fossa; parietal branch (posterior), which runs more horizontally toward the back and supplies posterior part of the middle cranial fossa and supratentorial part of the posterior cranial fossa. -

The origin of the left vertebral artery was from the aortic arch. On the right side, no thyrocervical trunk was found. The two branches which normally originate from the thyrocervical trunk had a different origin. The transverse cervical artery arose directly from the subclavian artery and suprascapular artery originated from the internal thoracic artery. This variation provides a short route for posterior scapular anastomoses. An awareness of this rare variation is important because this area is used for diagnostic and surgical procedures. Downloaded from INTRODUCTION When faced with a patient who may have had a NEUROLOGICAL SIGN stroke or transient ischaemic attack (TIA), one needs to ask oneself some simple questions: was the event vascular?; where was the brain lesion, and hence its vascular territory?; what was the cause? A careful history and focused physical examination are essential steps in getting the right answers. Although one can learn a great deal about the state of a patient’s arteries from expensive vascular imaging techniques, this does not make simple auscultation of the neck for carotid bruits redundant. In this brief review, we will therefore defi ne the place of the bruit in the diagnosis and management of patients with suspected TIA or stroke. WHY ARE CAROTID BRUITS IMPORTANT? A bruit over the carotid region is important because it may indicate the presence of athero- sclerotic plaque in the carotid arteries. Throm- boembolism from atherosclerotic plaque at the carotid artery bifurcation is a major cause of TIA and ischaemic stroke. Plaques occur preferentially at the carotid bifurcation, usually fi rst on the posterior wall of the internal carotid artery origin. We report a rare variant of the left vertebral artery arising as the most distal and inferior branch off the aortic arch in a 69 year- old male cadaver. Arising postero- inferiorly from the arch, the variant coursed superiorly and medial -ward, posterior to the left subclavian artery, enteringthe transverse cervical foramina at C5 level to run more cranially cervical foramina C5-C2. The variant artery was an observed with some tortuosity just proximal to entry into C5 foramina. The normally arising left or right vertebral artery plays a vital role in the Subclavian Steal Syndrome, a retrograde flow in the ipsilateral vertebral artery in an occlusion proximal origin of its ipsilateral subclavian artery. In our reported variant, modelled with a possible occlusion in the proximal segment of the left subclavian artery, despite an hypothesized retrograde flow in the left vertebral artery will not be helpful in delivering blood into the subclavian-axillary continuum, as such retrograde flow will dump into the aortic arch directly and unhelpful to the occluded left subclavian artery. -

Detailed dissection was performed on thirty specimens of adult’s cadaveric neck specimens and each dissected specimen was carefully measured the following patterns and distances using digital and ruler. The superior thyroid, lingual, and facial arteries arise independently from the external carotid artery (ECA), but can also arise together, as the thyrolingual or linguofacial trunk. We observed that 83.3% of STAs arose independently from the major artery, while 16.7% of the cases arose from thyrolingual or linguofacial trunk. We also measured the distance of STA from its major artery. The origin of the STA from the ECA was 0.9±0.4 mm below the hyoid bone. The STA was 4.4±0.5 mm distal to the midline at the level of the laryngeal prominence and 3.1±0.6 mm distal to the midline at the level of the inferior border of thyroid cartilage. The distance between STA and the midline was similar at the level of the hyoid bone and the thyroid cartilage. Also, when the STA is near the inferior border of the thyroid cartilage, it travels at a steep angle to the midline. -

Problems in Family Practice Heart Murmurs in Infants and Children Thomas A. Riemenschneider, MD Sacramento, California A system is presented for evaluation of heart murmurs in in fants and children. The system places emphasis on identifica tion of functional murmurs, which the physician encounters so frequently in daily practice. A three-part approach is presented which includes: (1) evaluation of cardiovascular status, (2) as sessment of the heart murmur, and (3) decision regarding the need for further evaluation. This approach relieves the physi cian of the necessity to remember the multiple details of the many congenital cardiac lesions, and requires only the knowl edge of a few easily remembered details about functional murmurs. The system enables the physician to confidently distinguish organic and functional murmurs and to decide which children need further evaluation and referral to the pediatric cardiologist. The course of the internal carotid artery showed no curvature in 191 cases, but in 74 cases it had a medial, lateral or ventrocaudal curve, and 17 preparations showed kinking (12) or coiling (5) out of a total of 265 dissected carotid sheaths and 17 corrosion vascular casts. In 6 cases of kinking and 2 of coiling, the internal carotid artery was located in direct contact with the tonsillar fossa. No significant sex differences were found.Variations of the internal carotid artery leading to direct contact with the pharyngeal wall are likely to be of great clinical relevance in view of the large number of routine procedures performed. Whereas coiling is ascribed to embryological causes, curving is related to ageing and kinking is thought to be exacerbated by arteriosclerosis or fibromuscular dysplasia with advancing age and may therefore be of significance in relation to the occurrence of cerebrovascular symptoms.
