Player Code of Conduct to be signed & included with player registration form

When playing soccer, I will:  Always do my best, even if we're losing.  Play fairly - I won't cheat, complain or waste time  Never be rude to my team-mates, the other team, the referee, spectators or my coach/team manager  Do what the referee tells me  Shake hands with the other team and referee at the end of the game  Listen to my coach/team manager and respect what he/she says  Talk to someone I trust, for example my parents or a club official about anything at my club.

I understand that breaches of the code may result in action being taken.

 Remain outside the field of play and behind the Designated Spectator Area (where provided)  Never engage in, or tolerate, offensive, insulting or abusive language or behaviour  Always respect the match officials' decisions  Applaud effort and good play as well as success

In addition, when attending games I will:  Remember that children play for FUN  Let the coaches do their job and not confuse the players by telling them what to do  Encourage the players to respect the opposition and match officials  Never criticize a player for making a mistake  Recognize that mistakes are part of learning

I, the undersigned, a parent or legal guardian of the player herein registered, in consideration of good and valuable consideration, the receipt of which is acknowledged for themselves, their heirs, executors, administrators, and assigns hereby release and forever discharge the Surrey Guildford United, its coaches, players and others involved in the activities of playing soccer from any and all actions, causes of actions, claims and demands for or by reason sustained in consequence of any activity of the soccer association and for the said consideration of the undersigned agrees not to make claim or take proceedings against any other person or corporation who might claim contribution of indemnity under the provision of any statute or otherwise.

And, it is hereby declared that the terms of this release are fully understood. I, the undersigned, have read, and agree to the contents of this registration form.

TERMS & CONDITIONS 1. All formal communication from parents to the club, executive, administrative staff and technical staff must be submitted via their designated team coach or manager. 2 Players and Parents (guardians) must sign and abide by the Player/Parent Code of Conduct. 3. Practice times, locations and days are determined by the executive in accordance with our field allocations policy posted on our website. 4. Any Player wishing to play up an age group must have the prior permisssion of the Club's Technical Director. 5. The season runs from September to March with a break mid December until early January. Cup Play may extend into April & May. 6. Game Days are usually Saturdays for Boys and Sundays for Girls.
