Shock waves are generated at high- or low-energy intensity, and treatment protocols can include more than 1 treatment. ESWT has been investigated for use in a variety of musculoskeletal conditions. OBJECTIVE The objective of this evidence review is to examine whether the use of extracorporeal shock wave treatment for plantar fasciitis, lateral epicondylitis, tendinopathy (shoulder, Achilles, and patellar), medial tibial stress syndrome, osteonecrosis of the femoral head, acute fracture nonunion or delayed union, or spasticity improves the net health outcome BACKGROUND Chronic Musculoskeletal Conditions Chronic musculoskeletal conditions (eg, tendinitis) can be associated with a substantial degree of scarring and calcium deposition. Calcium deposits may restrict motion and encroach on other structures, such as nerves and blood vessels, causing pain and decreased function. One hypothesis is that disruption of calcific deposits by shock waves may loosen adjacent structures and promote resorption of calcium, thereby decreasing pain and improving function. Plantar Fasciitis Plantar fasciitis is a common ailment characterized by deep pain in the plantar aspect of the heel, particularly on arising from bed. While the pain may subside with activity, in some patients the pain persists, interrupting activities of daily living. On physical examination, firm pressure will elicit a tender spot over the medial tubercle of the calcaneus. The exact etiology of plantar fasciitis is unclear, although repetitive injury is suspected. Heel spurs are a common associated finding, although it is unproven that heel spurs cause the pain. Asymptomatic heel spurs can be found in up to 10% of the population. Current Procedural Terminology © American Medical Association. All Rights Reserved. Many tendinitis and tendinopathy syndromes are related to overuse injury. Table 1. Fracture Nonunion and Delayed Union The definition of a fracture nonunion remains controversial, particularly the duration necessary to define nonunion. One proposed definition is a failure of progression of fracture healing for at least 3 consecutive months (and at least 6 months after the fracture) accompanied by clinical symptoms of delayed/nonunion (pain, difficulty weight bearing).

It hurts because you are putting tension on a place where the tissue is weakened, which is usually due to a degenerative process that seems to take a long time for your body to recognize and heal. WHY DO I HAVE IT? This is a common problem and unfortunately we don’t know why some people get this condition and others do not. Surprisingly, it is not at all clear that it comes from overuse or something that you did “wrong”. There is no doubt that if you do have tennis elbow, it will bother you more to do certain things, but that does not necessarily mean it was caused by those activities.

    Dr. Edward Kelly www.edwardkellymd.com A Patient’s Guide To Tennis Elbow (Lateral Epicondylitis) WHAT IS TENNIS ELBOW? Tennis elbow is breakdown and degeneration of tendons which attach to the outside (or lateral side) of the elbow. The muscles which work the hand and wrist begin as tendons which attach on a bony prominence on the lateral side of the elbow. This prominence is the lateral epicondyle of the humerus, so tennis elbow is degeneration of the tendons that attach to the lateral epicondyle (and so it is also called “lateral epicondylitis”). The pain can radiate into the forearm and occasionally into the hand. WHAT CAUSES IT? Tennis elbow typically is caused by repetitive gripping and grasping activities or occasionally from direct trauma to the outside of the elbow. Examples include when someone increases the amount of squeezing or gripping they perform, such as trimming the hedge or playing more tennis than usual. Once the tendons get injured it can be difficult to eradicate because those tendons are used every time the hand grips or squeezes. IS IT A SERIOUS CONDITION? Tennis elbow can be a painful and debilitating problem but does not lead to serious problems, like arthritis. However, x-rays or an ultrasound scan may be necessary in some cases to evaluate the elbow joint. An examination by a physician in the office will confirm the diagnosis of lateral epicondylitis. Lateral epicondylitis is the type of condition that will never get so bad that treatment cannot be performed. In many cases, it will resolve over time with non-operative treatments.

Achilles tendinitis and plantar fasciitis caused a severe talalgia and they were observed mainly in males with Reiter's syndrome or ankylosing spondylitis. On the other hand, sub-Achilles bursitis more frequently affected women with rheumatoid arthritis and rarely gave rise to severe talalgias. The simple calcaneal spur was associated with generalized osteoarthrosis and its frequency increased with age. This condition was not related to talalgias.     Advice and exercises for patients with plantar fasciitis This leaflet provides management advice and exercises for people diagnosed with plantar fasciitis, a condition causing pain in the heel, sole and /or arch of the foot. Structures of the foot The plantar fascia is a sheet or broad band of fibrous tissue that runs along the bottom of the foot. This tissue connects the heel to the base of the toes. Under normal circumstances, the plantar fascia supports the arch of the foot and acts as a shock absorbing “bow string” within the arch of the foot. With weight bearing, the foot flattens and the plantar fascia stretches, then springing you forward ready for the next step. While walking, the stresses placed on the foot can be one and a quarter times your body weight (this increases to two and three quarter times your body weight when running). Unsurprisingly then, that heel pain is common. If tension on this “bowstring” becomes too great, irritation or inflammation can occur causing pain. What is plantar fasciitis? Plantar fasciitis is a relatively common foot problem affecting up to 10-15% of the population. It can occur at any age. It is sometimes known as “policeman’s heel”. When placed under too much stress due to abnormal loading, the plantar fascia stretches causing micro tearing and degeneration of the tissue. It can lead to pain in the heel, across the sole of the foot and sometimes into the arch area of the foot too. These micro tears repair with scar tissue, which is less flexible than the fascia and can cause the problem to persist for many months.

  • Mcl Injuries
    MCL INJURIES Definition Medial Collateral Ligament (MCL) injury is an injury to the ligament on the inner part of the knee. This ligament keeps the shin bone (tibia) in place. It can be a stretch, partial tear or complete tear of the ligament. Causes The MCL is usually injured by pressure or stress on the outside part of the knee. A block to the outside part of the knee during football is a common way for this ligament to be injured. It is often injured at the same time as an anterior cruciate ligament (ACL) injury occurs. Symptoms Symptoms of a tear in the medial collateral ligament are: Knee Swelling Locking or catching of the knee with movement Pain and tenderness along the inside of the joint The knee gives way or feels like it is going to give way when it is active or stressed in a certain way First Aid A health care provider should examine your knee. A MCL test will be done to detect looseness of the ligament. This test involves bending the knee to 25 degrees and putting pressure on the outside surface of the knee. Other tests may include: Knee joint x-rays Knee MRI Treatment Includes: Applying ice to the area Nonsteroidal anti-inflammatory drugs (NSAIDS) Raising the knee above heart level You should limit physical activity until the pain and swelling go away. The health care provider may put you on crutches and in a brace to protect the ligament. You may also be told not to put any weight on your knee when you walk.
