We have government by the majority who participate.” That statement by Thomas Jefferson is a commentary on citizens’ right to “petition the government for a redress of grievances.” Effective communications and interactions between citizens and elected officials are essential to the democratic process, both to sustain the credibility of government and to maintain a free flow of ideas which can be integrated into policy decisions. However, since the introduction of the Internet, maintaining that free flow has been a challenge both for Congress and citizens. Congressional offices are overwhelmed with the significant increase in volume and the diversity of delivery methods used by advocate organizations. On the other hand, citizens and the advocacy community have been frustrated by the myriad of technological tools utilized by offices to authenticate that actual constituents are sending messages, yet result in technological and “user interface” obstacles to communications. -

Invisible Forces: Gender, Race, and Congressional Staff Kelly Dittmar, Ph.D. Rutgers University – Camden Center for American Women and Politics, Rutgers University – New Brunswick **WORKING PAPER** 1 INTRODUCTION Forty years ago, Harrison Fox and Susan Hammond published a seminal book on congressional staff, calling staffers “the invisible force in American lawmaking.” They were not the first observers of congressional dynamics to note the often unseen and under-investigated role of professional staff on Capitol Hill, nor have they been the last. Just two years later, Michael Malbin (1980) published Unelected Representatives: Congressional Staff and the Future of Representative Government, seeking to make visible what he perceived as an underappreciated level of staff influence on the policy process. By the mid-1990s, however, Herbert and Karen Foerstel (1996) still characterized congressional staff as representing “the large and influential Capitol Hill infrastructure” that is “invisible to most of the public” (145). Staff remained largely invisible to scholars as well, central to only a handful of book-length publications over the past four decades (Fox and Hammond 1977; Malbin 1980; Pierce 2014; Jones 2017a). The dearth of research on congressional professionals stands in stark contrast to the increased professionalization of the nation’s top legislative institution. The “ever-increasing complexity of governing,” as the national policy agenda has become larger and more complicated, has required members of Congress to hire specialists able to assist them in navigating the new political realities of effective representation (Romzek and Utter 1997, 1251; see also Polsby 1969). The Legislative Reorganization Acts of 1946 and 1970 both increased manpower and encouraged specialization among staff and members, helping them to meet mounting legislative demands. -

White Paper Series Ranking and analysis of overuse in U.S. hospitals EXECUTIVE SUMMARY Overuse, which is also known as low-value care, is often defined as the provision of health care services for which the potential for harm exceeds the likely benefit to a patient. Overuse occurs in all healthcare settings, including hospitals. It exposes patients to preventable harm and wastes more than $100 billion each year. To date, no study has published the rate of overuse in specific U.S. hospitals. The Lown Institute Hospital Index is the first hospital ranking to grade individual hospitals by how well they avoid overuse. This white paper provides an analysis of the overuse of 13 low-value services in 3,282 U.S. hospitals using Medicare data. These 13 services include imaging tests, surgeries, and cardiovascular procedures, and have all been validated in previous studies of low-value care. Only instances of these services deemed inappropriate in the literature were counted as overuse. Hospitals that did not have the capacity to perform a specific low-value procedure did not have that service counted in their total grade. We find that overuse varies across hospitals by type, size, and location. Teaching hospitals, larger hospitals, urban hospitals, and for-profit hospitals on average ranked lower for avoiding overuse overall, compared to non-teaching, smaller, rural hospitals. However, these patterns were not always consistent across all low-value services. Safety net hospitals and small rural hospitals had higher rankings in avoiding overuse overall, but their scores differed by specific low-value service. For-profit hospitals consistently ranked lower than nonprofit hospitals for avoiding nearly every low-value service measured. -

Given the catastrophic impact that Donald Trump’s losing presidential campaign will have on down-ballot Senate and House races, we urge you to immediately suspend all discretionary RNC support for Trump and focus the entirety of the RNC’s available resources on preserving the GOP’s congressional majorities. The signatories to this letter have been involveD with Republican politics at the local, state, and national levels for more than three decades. We have served as past and current elected officials and as staffers for the RNC; appointees in every Republican administration since President Reagan; advisors on the last nine GOP presidential campaigns; aiDes on leadership, personal, and committee staff in both the House and Senate; grassroots workers; and delegates to multiple GOP conventions. We believe that Donald Trump’s divisiveness, recklessness, incompetence, and record- breaking unpopularity risk turning this election into a Democratic landsliDe, and only the immediate shift of all available RNC resources to vulnerable Senate and House races will prevent the GOP from drowning with a Trump-emblazoned anchor around its neck. This should not be a difficult decision, as Donald Trump’s chances of being elected president are evaporating by the day. Therefore the path of giving U.S. politicians a “way out” won’t in fact work because Castro will twist it to his interests. Better to just do it unilaterally on our own timetable. For the time being it appears U.S. policy will remain reactive—to Castro and to Cuban American pressure groups—irrespective of the interests of Americans and Cubans as a whole. Like parrots, all presi- dential hopefuls in the 2000 presidential elections propose varying ver- sions of the current failed policy. We have made much here of the negative role of the Cuba lobby, but we close by reiterating that their advocacy has not usually been different in kind from that of other pressure groups, simply much more effective. The buck falls on the politicians who cannot see the need for, or are afraid to support, a new policy for the post–cold war world. Notes 1.
