FAMILY VIOLENCE DEATH REVIEW COMMITTEE FIFTH REPORT DATA: JANUARY 2009 TO DECEMBER 2015 1 FOREWORD The Health Quality & Safety Commission (the Commission) welcomes the Fifth Report Data: January 2009 to December 2015 from the Family Violence Death Review Committee (the Committee). This companion report presents data on family violence deaths in Aotearoa New Zealand during 2009–15 and contextualises the thinking about family violence discussed in the Committee’s Fifth Report. This report emphasises the Committee’s call to shift how the sector thinks about and responds to family violence. Preventing deaths from family violence requires an integrated family violence system in which agencies, organisations and practitioners are oriented to work together with families and whānau to provide safe, effective and culturally responsive support. The over-representation of Māori among family violence deceased and offenders, as well as those from the most deprived neighbourhoods, illustrates that family violence is not distributed equally. For Māori, multiple intersecting disadvantages, both contemporary and historical, continue to contribute to the violence within whānau seen today. For those experiencing family violence and living in the most deprived neighbourhoods, including many Māori whānau, the health sector’s continued focus on improving equity in the quality and safety of services for all populations is paramount. The Commission commends the Committee’s contribution to the family violence sector work. This report, together with the Fifth Report, provide critical analysis and direction for guiding system reform and preventing deaths from family violence. These reports are important for informing the integrated system focus of the Ministerial Group on Family Violence and Sexual Violence. Family violence causes harm that can traverse generations. This report reminds us that there are many child survivors of family violence who have lost a sibling and/or a parent, and some of these children were present when their family member was killed. To prevent intergenerational patterns of harm, we hope the conceptual shifts and approaches summarised in this report continue to inform the work of practitioners and organisations working to address and ultimately prevent family violence. It is through collective responsibility and action across the workforce and our communities that we can address the complexities of family violence, and enable safety and wellbeing for families and whānau. Professor Alan Merry ONZM FRSNZ Chair, Health Quality & Safety Commission June 2017 2 CHAIR’S INTRODUCTION This companion report documents the devastation of family violence – the lives lost and the enduring burden of harm afflicted upon the living. These lives and deaths are a testament to why we must transform our ways of addressing family violence and violence within whānau. History shows we cannot be effective in preventing violence through a series of system tweaks; we must be prepared to make some transformational leaps. We know the people in this report lived in communities, they all accessed health care services and their children went to schools. To be preventative we need to consider new configurations of services and ways of responding to family violence and violence within whānau. Every day, many people are working with people experiencing or perpetrating violence. There are multiple opportunities to wrap support around child and adult victims, their families and whānau, as well as to work with fathers, men and their communities in ways that respectfully challenge them to take responsibility for their behaviour and to be the parent their family and whānau needs. We need a workforce capability lift, so we can maximise these opportunities for change. Kaupapa Māori approaches are an essential part of this reorientation. The size of the social problem means this work cannot remain the role of the few – New Zealand Police, Oranga Tamariki and family violence non-government organisations – it must become the responsibility of the many. What is required is whole-of-family and -whānau, whole-of-organisation and whole-of- system responsiveness. The challenge is large, but so is our collective workforce. The Committee commends Minister Adams and Minister Tolley for their leadership of the Ministerial Group on Family Violence and Sexual Violence, demonstrated on 7 June 2017 with the launch of the Family Violence, Sexual Violence and Violence within Whānau: Workforce Capability Framework June 2017 (the Framework). The Framework sets out a vision and expectation of excellence – a workforce capable of responding safely and respectfully to family violence, sexual violence and violence in whānau. As Interim Chair of the Committee, I wish to recognise the exceptional leadership and dedication of the previous Chair, Professor Julia Tolmie. She has worked tirelessly to ensure the Committee’s learning from reviewing deaths positively influences reform in the sector. It is a privilege to have the role of kaitiakitanga of the Committee and to work with such inspiring people. I would also like to acknowledge the many practitioners who continue to respond with compassion and empathy to all those affected by family violence and violence within whānau. To all the family and whānau members whose loved ones are detailed in this report: we know family violence is preventable. Now is a critical time in Aotearoa New Zealand to forge our collective commitment to the changes needed in our society – changes that will enable prevention to become a reality for this generation of mokopuna and for generations to come.     1 Multilingual Discourses Vol. 1.2 Spring 2014 Tanya Ball The Power of Death: Hierarchy in the Representation of Death in Pre- and Post-Conquest Aztec Codices hrough an examination of Aztec death iconography in pre- and post-Conquest codices of the central valley of Mexico T (Borgia, Mendoza, Florentine, and Telleriano-Remensis), this paper will explore how attitudes towards the Aztec afterlife were linked to questions of hierarchical structure, ritual performance and the preservation of Aztec cosmovision. Particular attention will be paid to the representation of mummy bundles, sacrificial debt- payment and god-impersonator (ixiptla) sacrificial rituals. The scholarship of Alfredo López-Austin on Aztec world preservation through sacrifice will serve as a framework in this analysis of Aztec iconography on death. The transformation of pre-Hispanic traditions of representing death will be traced from these pre- to post-Conquest Mexican codices, in light of processes of guided syncretism as defined by Hugo G. Nutini and Diana Taylor’s work on the performative role that codices play in re-activating the past. These practices will help to reflect on the creation of the modern-day Mexican holiday of Día de los Muertos. Introduction An exploration of the representation of death in Mexica (popularly known as Aztec) pre- and post-Conquest Central Mexican codices is fascinating because it may reveal to us the persistence and transformation of Aztec attitudes towards death and the after-life, which in some cases still persist today in the Mexican holiday Día de Tanya Ball 2 los Muertos, or Day of the Dead. This tradition, which hails back to pre-Columbian times, occurs every November 1st and 2nd to coincide with All Saints’ Day and All Souls’ day in the Christian calendar, and honours the spirits of the deceased.

Purpose of Submission This submission draws on previously published and forthcoming research to identify emerging issues in the distribution, disposal, and reuse of the digital assets of deceased internet users for the Commission’s consideration. The submission will not make comments upon the current operation of, or recommendations for specific amendments to, specific laws. Rather, it will provide suggestions for questions to be considered by NSWLRC in undertaking its review. It will also outline principles emerging in the literature that could usefully inform regulation of the use and disposal of digital assets. 2. Context The management of ‘digital remains’ (Gibson 2014), the electronic artefacts left by internet users after they die, has been an emerging topic in sociology and cultural studies in the last decade. It has also presented an ongoing practical and legal problem for individuals, corporations, and governments around the world. However there has been relatively little attention paid to this topic by ethicists, while policy making has tended to be reactive and ad hoc, attempting to deal with challenges generated by new technologies after the fact. To date, policy responses have often simply applied existing concepts of ownership or rights of disposal to digital assets, without recognizing and responding to the ways in which these differ from traditional (physical, financial, intellectual) assets.

In English the same process resulted in the word “DEADEN”, where the suffix “EN” means “to cause to be”. We all know that the things which has life is going to be dead in future anytime any moment. So, the sentence we know popularly that “Man is mortal”. The sources of life comes into human body when he/she is in the womb of mother. The active meeting of sperm and eggs, it create a new life in the woman‟s overy, and the woman carried the foetus with 10 months and ten days to given birth of a new born baby . When the baby comes out from the pathway of the vagina of his/her mother, then his/her first cry is depicted that the new born baby is starting to adjustment of of the newly changing environment . For that very first day, the baby‟s survivation is rairtained by his/her primary environment. But the tendency of death is started also. In any time of any space the human baby have to accept death. Not only in the case of human being, but the animals, trees, species, reptailes has also the probability of death. The above mentioned live behind are also survival for the fittest.

    Singapore Med J 2017; 58(3): 129-133 Original Article doi: 10.11622/smedj.2016179 Death among elderly patients in the emergency department: a needs assessment for end-of-life care Rakhee Yash Pal1, MBBS, MMed, Win Sen Kuan1, MBBS, MRCS, Yiwen Koh2, BSc(Hons), Kuhan Venugopal1, MBBS, Irwani Ibrahim1, MBBS, FRCS INTRODUCTION Elderly patients with serious chronic diseases often present to the emergency department (ED) in the last moments of their life, many with identifiable trajectories of dying: organ failure, advanced cancer and chronic frailty. These patients and their families may benefit more from good end-of-life (EOL) care provision than the standard resuscitative approach. This study aimed to determine the incidence and nature of death among patients aged ≥ 65 years in an ED, and characterise their trajectories of dying. METHODS This was a retrospective study carried out over a one-year period in a tertiary ED. All ED deaths in patients aged ≥ 65 years over this period were included. Information on the patients’ demographics, comorbidities and details of death were extracted from the hospital’s electronic medical records database. Based on the available information, their Karnofsky Performance Status (KPS) scores and trajectories of dying were ascertained. RESULTS In one year, 197 patients aged ≥ 65 years died in the ED, 51.3% of whom suffered from serious chronic illnesses, with identifiable trajectories of dying. Of these patients, 46.5% had premorbid functional limitation with KPS scores of 0–40. However, only 14.9% of patients had a pre-existing resuscitation status and 74.3% received aggressive resuscitative measures.
