It is important that our company shapes global public policy debates on issues specific to the people we serve and to our other key stakeholders including shareholders and employees. Our engagement in the political arena helps address the most pressing issues related to ensuring that patients have access to needed medications—leading to improved patient outcomes. Through public policy engagement, we provide a way for all of our locations globally to shape the public policy environment in a manner that supports access to innovative medicines. We engage on issues specific to local business environments (corporate tax, for example). Based on our company’s strategy and the most recent trends in the policy environment, our company has decided to focus on three key areas: innovation, health care delivery, and pricing and reimbursement. More detailed information on key issues can be found in our 2014 Corporate Responsibility Update. Through our policy research, development, and stakeholder dialogue activities, Lilly develops positions and advocates on these issues. Government actions such as price controls, pharmaceutical manufacturer rebates, and access to Lilly medicines affect our ability to invest in innovation. Lilly has a comprehen- sive government relations operation to have a voice in the public policymaking process at the federal, state, and local levels. Lilly is committed to participating in the political process as a responsible corporate citizen to help inform the U.S. debate over health care and pharmaceutical innovation.

PLATT FAMILY RECORDS CENTER NOTES Volume 3.1 compiled by Lyman D. Platt, Ph.D. Platt Family Records Center The Redwoods, New Harmony, Utah 2008 2 The Platt Family Records Center Copyright © 2008 The Lyman D. Platt Family Protection Trust All Rights Reserved Manufactured in the U.S.A. 3 INTRODUCTION Over many years the collections that comprise the Platt Family Records Center (PFRC) have been gathered from a diversity of sources and locations. These have been cataloged as they have been received, or in the order that they were initially organized. It was not felt in preparing this final version that a re-cataloging was necessary due to the versatility of the indexing systems used. There are twelve divisions to the PFRC: 1) Documents; 2) Letters; 3) Notes; 4) Family Histories; 5) Journals & Diaries; 6) Manuscripts; 7) Photographs; 8) Maps; 9) Books; 10) Genealogies; 11) Bibliography; and 12) Indexes. The collection that follows - Notes - is divided into several volumes, 3.1, 3.2, 3.3, etc. These notes do not contain all of the material that I have collected, but they are a compilation of much of it. Some information went directly into histories, family group records, pedigree charts and short biographies. Copies of these volumes have been given to: 1) Special Collections, Marriott Library, Brigham Young University, Provo, Utah; 2) The Church Historical Library, Salt Lake City, Utah; 3) Special Collections, Southern Utah University, Cedar City, Utah; and 4) The Daughters of the Utah Pioneers Museum, Salt Lake City, Utah. Additional copies have been given to each of my siblings and to some of our children.

Digitized by the Internet Archive in 2013 http://archive.org/details/historicalcollecOOphil : ' HISTORICAL COLLECTIONS RELATING TO CHISWICK EDITED BY W. P. W. PHILLIMORE AND W. H. WHITEAR. LONDON Phillimore & Co Essex Street, Strand. , 36, 1897. NOTE. THIS volume is an attempt to collect together in a handy form some historical information relating to the parish of Chiswick. It originated in some weekly contributions sent by the Editors to a local newspaper, the " Chiswick Times," during the years 1895 an^ 1896. This serves to explain the fact that the book is more a collection of essays than a systematic parochial history, though all the same it may be hoped that it will hereafter prove a useful groundwork to some one able and willing to compile a history worthy of the parish. Incomplete as the present work is, it will serve to direct attention to the many points of interest in the past history of Chiswick. Much still remains to be done, for as yet the public records have been but little drawn upon, and the reader must not think that we have at all exhausted the field of research which lies open to us. Some of the chapters are merely reprints from other works ; some are by one or other of the Editors ; for the chapter on Sutton Court the reader is indebted to Mr. W. M. Chute, and for the account of the prebendal manor to Mr.

1 Verizon Political Activity January – December 2016 A Message from Craig Silliman Verizon is affected by a wide variety of government policies -- from telecommunications regulation to taxation to health care and more -- that have an enormous impact on the business climate in which we operate. We owe it to our shareowners, employees and customers to advocate public policies that will enable us to compete fairly and freely in the marketplace. Political contributions are one way we support the democratic electoral process and participate in the policy dialogue. Our employees have established political action committees at the federal level and in 18 states. These political action committees (PACs) allow employees to pool their resources to support candidates for office who generally support the public policies our employees advocate. This report lists all PAC contributions, corporate political contributions, support for ballot initiatives and independent expenditures made by Verizon and its affiliates during 2016. The contribution process is overseen by the Corporate Governance and Policy Committee of our Board of Directors, which receives a comprehensive report and briefing on these activities at least annually. We intend to update this voluntary disclosure twice a year and publish it on our corporate website. We believe this transparency with respect to our political spending is in keeping with our commitment to good corporate governance and a further sign of our responsiveness to the interests of our shareowners. Craig L. Silliman Executive Vice President, Public Policy and General Counsel 2 Verizon Political Activity January – December 2016 Political Contributions Policy: Our Voice in the Democratic Process What are the Verizon Good Government Clubs? setting of monetary contribution limitations and the The Verizon Good Government Clubs (GGCs) exist to help the establishment of periodic reporting requirements.
