Chin Med (2019) 14:57 https://doi.org/10.1186/s13020-019-0277-2 Chinese Medicine REVIEW Open Access Huang-Lian Jie-Du decoction: a review on phytochemical, pharmacological and pharmacokinetic investigations Yiyu Qi1,2,3, Qichun Zhang1,2,3,4* and Huaxu Zhu1,2,3* Abstract Huang-Lian Jie-Du decoction (HLJDD), a famous traditional Chinese prescription constituted by Rhizoma Coptidis, Radix Scutellariae, Cortex Phellodendri and Fructus Gradeniae, has notable characteristics of dissipating heat and detoxi- fcation, interfering with tumors, hepatic diseases, metabolic disorders, infammatory or allergic processes, cerebral diseases and microbial infections. Based on the wide clinical applications, accumulating investigations about HLJDD focused on several aspects: (1) chemical analysis to explore the underlying substrates responsible for the therapeutic efects; (2) further determination of pharmacological actions and the possible mechanisms of the whole prescrip- tion and of those representative ingredients to provide scientifc evidence for traditional clinical applications and to demonstrate the intriguing molecular targets for specifc pathological processes; (3) pharmacokinetic feature studies of single or all components of HLJDD to reveal the chemical basis and synergistic actions contributing to the pharma- cological and clinically therapeutic efects. In this review, we summarized the main achievements of phytochemical, pharmacological and pharmacokinetic profles of HLJDD and its herbal or pharmacologically active chemicals, as well as our understanding which further reveals the signifcance of HLJDD clinically.     CHAPTER SEVEN The Age of Aquarius Although the idea of a New Age was popularized by Alice Bailey in the early twentieth century, the term had been around at least since the American Rev- olution before it was used self-consciously by Theosophists like her who be- lieved a “master” would come to enlighten humanity and usher us into a new age. The concept picked up relevance as the s counterculture looked to- ward the Age of Aquarius as a utopian future of peace and equality. Move- ments aimed at social and personal transformation that emerged or were given new meaning in the s continue to shape New Age and Neopagan religions. Ideas about the expected new era vary among Neopagans and New Agers, just as they ranged in the s from social revolution to com- munal escape from society. But most agree that it will include a changed dy- namic between men and women, healthy diet, holistic healing practices, and peacefulness. Along with and related to holistic healing and feminist restructuring, en- vironmentalist concerns are seen as key in bringing about the transformation of society. Many New Agers and Neopagans believe that an ecologically vi- able relationship to the natural world will characterize the future age, when humans will live more harmoniously on earth. Some also believe the earth it- self, a living being that has been ill used by humanity, will bring about cata- clysmic changes, while others expect a gradual dawning of enlightened con- sciousness among large numbers of people to usher in the New Age. Precise teaching of Kampo is one of the important issues in medical education, and therefore acupuncture and moxibustion education has been applied to students in Tokai University School of Medicine. To investigate and compare the awareness the medical students have of acupuncture and moxibustion before and after the education, we conducted questionnaire surveys. Methods: The questionnaires were distributed to 117 fourth-year students before and after the education. Results: Of the 117 students issued surveys, 111 responded before, 115 after, and 109 both before and after the education program. Before the education, 79% of the respondents were interested in acupuncture and moxibustion, and 67%, 73%, and 80% thought they were effective, necessary, and worth learning. These results were increased to 92%, 95%, 94% and 97% after the education, respectively (p<0.001 in all four com- parisons). Although 9% said they would perform acupuncture and moxibustion in their routines proactively before the education, after the education this increased to 24% (p<0.001), and 43% supposed that they would definitely introduce patients to acupuncturists. Conclusion: Medical students’ awareness improved after the education, which demonstrated importance of the acupuncture and moxibustion education in medical school.

Methods: A retrospective cross-sectional study was conducted using electronic medical records. All patients aged 65 years or older who continued visiting internal medicine physicians and were prescribed any Kampo medications from January 2015 to March 2015 were included. The primary outcome was the proportion of patients taking any potentially inappropriate Kampo medications that should be used with caution (hereafter referred to as UWC Kampo medications). The medication appropriateness was evaluated based on the 2015 Japan Geriatrics Society guidelines. Results: Eighty eligible patients were identified. The mean age was 76.7 years, 45 patients (56.2%) were female, the mean Charlson Comorbidity Index was 1.7, the median number of non-Kampo medications used was 5.0, and the median number of Kampo medications used was 1.0. The proportion of patients taking any UWC Kampo medications was 28.8% (95% confidence interval, 18.6 to 38.9%). Medications containing Glycyrrhizae radix for chronic kidney disease or loop diuretics were the most common UWC Kampo medications. Compared with patients who did not take any UWC Kampo medications, patients who did take such medications used aconite compositions more frequently (p = 0.04) and were more likely to have uncontrolled hypertension (p = 0.02) and chronic kidney disease (p < 0.001).

Background: Kampo medicine is the traditional form of medicine practiced in Japan, based on ancient Chinese medicine. Kampo medicine includes acupuncture and moxibustion, as well as herbal medicine. Physicians are allowed to prescribe ethical Kampo extract granules used in various formulae and practice acupuncture and moxibustion as acupuncturists and moxibustionists. However, medical schools do not offer many classes in traditional medicine. This study aims to analyze the use of Kampo formulae and practice of acupuncture and moxibustion in contemporary community health care. Methods: The subjects（1538 clinicians）were graduates of Jichi Medical University on or after 1978, affiliated with a clinic or hospital with 300 beds or less on July 2010, and surveyed by postal questionnaire in October 2010. Results: The effective response rate across Japan was 44%（n＝679). Of these responders, 30%, 45%, and 22% were found to prescribe Kampo formulae regularly, occasionally, and rarely, respectively, in daily clinical care. Frequently prescribed formulae included shakuyakukanzoto, daikenchuto, kakkonto, rikkunshito, and hochuekkito. The reasons for using Kampo formulae given by 61% and 58% of the responders were, respectively, applicability for common diseases and ease of use for unidentified complaints and psychosomatic disease.
