1. Approval of the Diploma course and examination in Pharmacy conducted at Siddhi Vinayaka Institute of Technology & Sciences, Near Dindayal Awas Yojna, Ulaspur, P.O. Mangala, Bilaspur (Chhattisgarh), in the light of 4th Inspection Report (May, 2012). ------(17-762/2009-PCI) The latest information on record was placed and considered.

It was decided to recommend to the Council to extend the approval of the D.Pharm course and examination upto 2014-2015 academic session for 60 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

2. Approval of the Diploma course and examination in Pharmacy conducted at H.M.S. College of Pharmacy, H.M.S. It was decided to recommend to the Council to extend the approval of the D.Pharm course and examination upto 2014-2015 academic session for 60 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

It was decided to recommend to the Council to extend the approval of the D.Pharm course and examination upto 2014-2015 academic session for 60 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

It was decided to recommend to the Council to extend the approval of the D.Pharm course and examination upto 2014-2015 academic session for 40 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

Regarding diploma course, it was decided to recommend to the Council to extend the approval of the D.Pharm course and examination upto 2014-2015 academic session for 60 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

It was decided to seek compliance with documentary evidence.

7. Approval of Diploma course in Pharmacy conducted at Dr. S.K. Toshniwal Educational & Research Trust’s Vidarbha Institute of Pharmacy, Gut No. 114, Anjankhed, Borala Phata, Hingoli Road, Washim – 444 505 (Maharashtra), in the light of 1st inspection report (April, 2012). ------(17-982/2012-PCI)

The latest information on record was placed and considered.

It was decided to recommend to the Council to -

i) grant approval for 2011-2012 & 2012-2013 academic session for 60 admissions for the conduct of Ist & IInd year D.Pharm course.

ii) allow 60 admissions for 2012-2013 academic session subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to inspect the institution for considering final approval u/s 12 of the Pharmacy Act, 1948 on receipt of an undertaking on duly attested affidavit from an institution to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

8. Approval of the Diploma course and examination in Pharmacy conducted at Satyam Polytechnic & Pharmacy College, Village Kotla Doom Ram Tirath Amritsar (Punjab), in the light of 2nd Surprise Inspection Report (April, 2012). ------(17-945/2010-PCI)

It was decided to recommend to the Council to grant approval to the D.Pharm course and examination from 2008-2009 to 2014-2015 academic session for 60 admissions u/s 12 of the Pharmacy Act, 1948 subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

It was decided to recommend to the Council to permit raise in admissions from 60 to 100 from 2012-2013 academic session.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

10. Approval of the Degree course and examination in Pharmacy conducted at Royal College of Pharmacy Science, Behind Pt. R.S. University Campus, Dumar Tabla (Mohba Bazar) Raipur (Chhattisgarh), in the light of 2nd Surprise Inspection Report (May, 2012). ------(32-760/2010-PCI)

It was decided to recommend to the Council to grant approval to the B.Pharm course and examination from 2008-2009 to 2014-2015 academic session for 60 admissions u/s 12 of the Pharmacy Act, 1948 subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

It was decided to recommend to the Council to extend the approval of the B.Pharm course and examination upto 2014-2015 academic session for 60 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

It was decided to recommend to the Council to -

i) grant approval for 2012-2013 academic session for 60 admissions for the conduct of IVth year B.Pharm course.

ii) allow 60 admissions for 2012-2013 academic session subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to inspect the institution for considering final approval u/s 12 of the Pharmacy Act, 1948 on receipt of an undertaking on duly attested affidavit from an institution to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

13. Approval of Degree course in Pharmacy conducted at School of Pharmacy SRTM University, Nanded Vishnupuri Nanded – 431 606 (Maharashtra), in the light of 1st inspection report (April, 2012). ------(32-1073/2012-PCI)

The latest information on record was placed and considered.

It was decided to recommend to the Council to -

i) grant approval for 2011-2012 & 2012-2013 academic session for 60 admissions for the conduct of Ist & IInd year B.Pharm course.

ii) allow 60 admissions for 2012-2013 academic session subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution – ] i) that every year it has to apply to PCI in the SIF for the conduct of early inspection for year-wise approval or rejection of approval on the basis of which the institution can make further admission/no admission. When the students are in the final year B.Pharm an inspection will be conducted by PCI for consideration approval of B.Pharm course u/s 12 of the Pharmacy Act for the purpose of registration as pharmacist. It is the responsibility of the institution to apply in the prescribed SIF of to PCI before 15th September every year alongwith complete documents.

ii) to comply with the requirements of Education Regulations, 1991 particularly regarding appointment of teaching staff and equipments.

It was also decided to seek an undertaking on duly attested affidavit from an institution to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

The decisions taken in respect of following institutions are as under –

a) Prasad Institute of Pharmaceutical Sciences, Vikas Nagar, Shameerpet, Jangaon – 506 167 Warangal (A.P.). ------(F.No.32-932/2010-PCI)

It was decided to convey the findings of the Personal Hearing Committee to the institution.

It was decided to intimate the institution that in the light of the findings of the Personal Hearing Committee, the PCI intends to issue notice u/s 13(1) of the Pharmacy Act, 1948 for withdrawal of approval. In view of it, no admission shall be made from 2012-2013 academic session and institution shall submit compliance with documentary evidence.

b) Saroj Insititute of Techology & Management, Ahimamau, P.O. Arjunganj, Sultanpur Road, Lucknow (U.P). ------(F.No.32-315/2011-PCI)

It was decided to convey the findings of the Personal Hearing Committee to the institution.

It was decided to intimate the institution that in the light of the findings of the Personal Hearing Committee, the PCI intends to issue notice u/s 13(1) of the Pharmacy Act, 1948 for withdrawal of approval. In view of it, no admission shall be made from 2012-2013 academic session and institution shall submit compliance with documentary evidence.

c) Azad Institute of Pharmacy & Research, Azadpuram, Post: Chandrawal, Bangla Bazar Road, Lucknow – 226 021 (Uttar Pradesh). ------(F.No.32-497/2008-PCI)

It was decided to convey the findings of the Personal Hearing Committee to the institution.

d) Goel Institute of Pharmacy & Sciences Vill.-Anora, Post-Pargana Near Indira Canal Faizabad Road Lucknow (Uttar Pradesh) ------(F.No.32-925/2010-PCI)

It was decided to recommend to the Council to extend the approval of the D.Pharm course and examination upto 2012-2013 academic session for 30 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

It was decided to recommend to the Council to extend the approval of the D.Pharm course and examination upto 2014-2015 academic session for 60 admissions subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

Regarding diploma course, it was decided to recommend to the Council to grant approval to the D.Pharm course and examination from 2010-2011 to 2012-2013 academic session for 60 admissions u/s 12 of the Pharmacy Act, 1948 subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

Regarding degree course, it was decided to recommend to the Council to -

i) grant approval for 2012-2013 academic session for 60 admissions for the conduct of IIIrd year B.Pharm course.

ii) allow 60 admissions for 2012-2013 academic session subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution –

i) that every year it has to apply to PCI in the SIF for the conduct of early inspection for year-wise approval or rejection of approval on the basis of which the institution can make further admission/no admission. When the students are in the final year B.Pharm an inspection will be conducted by PCI for consideration approval of B.Pharm course u/s 12 of the Pharmacy Act for the purpose of registration as pharmacist. It was decided to recommend to the Council to grant approval to the B.Pharm course and examination from 2007-2008 to 2011-2012 academic session for 60 admissions u/s 12 of the Pharmacy Act, 1948 subject to submission of affiliation fee per annum to the PCI within the stipulated time period.

It was further decided to instruct the institution to submit duly attested affidavit to the effect that -

· Institute will restrict admission to sanctioned intake by PCI & will not admit students beyond sanctioned intake without prior approval of the PCI. · Institute will not start additional Pharmacy Programme/start 2nd / evening / additional shift in the existing pharmacy institution. · Institution shall comply with statutory norms and standards prescribed by the PCI from time to time. · The entire consequences of failure to comply with the undertaking shall rest on the institution and PCI in no way shall be responsible for the same.

It was also decided to intimate the institution that in the light of the findings of the Personal Hearing Committee, the PCI intends to issue notice u/s 13(1) of the Pharmacy Act, 1948 for withdrawal of approval. 143. Consideration of approval of the Pharm.D & Pharm.D (P.B.) course and examination in Pharmacy conducted at P. Rami Reddy Memorial College of Pharmacy, 1-35, Prakurthi Nagar, Utukur, Kadapa – 516 003 (Andhra Pradesh), in the light of 6th inspection report (June, 2012). ------(50-207/2012-PCI) The latest information on record was placed and considered.

It was decided to recommend to the Council to grant approval to Pharm.D. (Post Baccalaureate) (Doctor of Pharmacy) course from 2009-2010 to 2012-2013 academic sessions for 10 admissions u/s 12 of the Pharmacy Act, 1948 for the purpose of registration as a pharmacist.

It was further decided to recommend to the Council to grant approval for 2012-2013 academic session for 30 admissions for the conduct of Vth year Pharm.D course.

It was further decided to instruct the institution –

i) that every year it has to apply to PCI in the SIF for the conduct of early inspection for year-wise approval for conduct of course or otherwise on the basis of which the institution can make further admission/no admission. When the students are in the final year Pharm.D/Pharm.D (Post Baccalaureate), an inspection will be conducted by PCI for consideration of approval u/s 12 of the Pharmacy Act for the purpose of registration as pharmacist. It is the responsibility of the institution to apply in the prescribed SIF to PCI before due date.

ii) to comply with the requirements of Pharm.D. Regulations, 2008 particularly regarding appointment of teaching staff, equipments and Hospital facility.
