Interested OHTs are encouraged to reach out to program staff early on to discuss potential ideas. OHTs may submit more than one EOI but may only host one Impact Fellow at a time.

4. OHT Priority Areas Based on the population health data that we received from the Ministry which we have used as our foundational work pillars, RRDOHT is focusing on Mental Health and Addictions and Diabetes. We have also added focus on Digital Health to explore how we can streamline technology to better enhance patient experience.

5. OHT Goals The RRDOHT is committed to improving how we collect and report on the attributed population health measures in our dened geographic region. The data needs to follow the path of the patient through and beyond provincial and regional borders. Further, we aim to rene our mental health and addictions and diabetes pathways. We are also working on our engagement plan as we begin our work on year 1 priorities and deliverables which includes establishing the patient, family, and caregivers advisory committee and primary care providers advisory council.

6. Proposed Projects RRDOHT envisions the role of the Fellow as follows:

Data Mining – help us understand the referral pathways within our District and also pathways for those being referred outside the District - help us understand the data that we can collect to help advance our OHT focus: Mental Health and Addictions, and Diabetes (utilization/demand, baseline information – current state) - data that we can use to develop our performance metrics - To help design and implement needed surveys as part of data collection, needs assessment and performance / project evaluation

2. Develop Performance Metrics Help RRDOHT in creating performance metrics and ensuring that such metrics can be collected and supported by individual partner organizations

3. Content Support for Website, Social media, and OHT information materials Help us to develop content for stakeholder sharing / public information

4. Collaboration and Immersion Attend all meetings, support the development of our Patient and Family Advisory Council and our Primary Care Provider, collaborate with other Fellows in NW Ontario.

Project Summary The Fellow will be expected to be fully immersed in the operations of the RRDOHT from getting to know the individual and group dynamics of members, supporting all meetings, and helping establish the councils. System mapping and development of Data Framework are key deliverables to help establish our baseline data. The Fellow is coming at a good time in our OHT development as there will be signicant work in helping develop our branding, social media presence and RRDOHT processes. Development of robust performance metrics is another key work pillar for this project.

7. Desired Competencies The Fellow will have demonstrated experience with: 1. Meaningful and respectful engagement with our indigenous people and various partners by displaying cultural sensitivity and curiosity 2. Data collection, extraction, interpretation, and presentation - ability to create clear picture of cause and effects with data 3. Quantitative and qualitative analysis 4. System-level capacity planning 5. Evaluation and framework design 6. Leadership, exibility, initiative, and collaboration 7. Excellent verbal and written communication skills (presenting and writing reports) 8. Group Facilitation including Strategic Planning (preferred) 9. Knowledge of various electronic platforms (preferred)

8. OHT Environment RRDOHT is a wave 2-approved OHT. However, the partners have a long standing history of successful collaboration in various projects throughout the Rainy River District. We are still in the process of forming our desired identity as an OHT specifically around collaborative decision-making and having meaningful conversations before arriving at a consensus. The role of the Fellow will further enhance our relationship-building through the identified projects. There is a strong desire for our OHT to be successful and pockets of initiatives are already happening. We are willing to engage, welcome, and mentor our matched Fellow because we understand the value of having a full-time support even for a year. We are also committed to ensuring that our Fellow will have opportunities to collaborate with other Fellows in the Northwest region. The Impact Fellow would be well supported and resourced with

2 mentorship from our RRDOHT members. We would like to offer the Fellow with an opportunity to co- create our one-year plan, embedding in-kind supports that the Fellow identifies specifically around projects that will promote the Fellow's growth and development

Main Host in Atikokan: Jorge VanSlyke, Atikokan General Hospital CEO Co-host: Karen Lusignan, ED, Atikokan Family Health Team

We have offices identified for our Fellow both at the hospital and the Family Health Team. Jorge will take on the main support for the Fellow. Jorge has her Masters in Social Work and Public Administration, which will assist in the Fellow's projects. We will provide regular monthly meetings to ensure we have focused time to review, guide, support our Fellow.

Supporting host in Fort Frances: Dan McCormick, CAO Rainy River DSSAB .

Dan has an office space for our Fellow as well when trips to Fort Frances are needed because our other partners are in that area.

Our Steering Committee would be providing support as well in terms of directions .

Preferred Work Arrangement for Hybrid Fellow

9. Opportunities for Professional Growth and Development Through co-creation of the one-year plan with the successful matched Fellow for RRDOHT, we are offering the opportunity to foster a learner-centered environment. RRDOHT is a smaller OHT that is already actively nurturing collegiality among members which should help our Fellow get immersed in a hospitable environment. The projects that we identied have key impacts to our OHT which would create a mutual meaningful relationships and outcomes. The northern, rural environment, with the right candidate, can offer a unique experience to our Fellow because to be successful, one needs to be creative and an outside the box thinker. There will also be a signicant experience working with our Indigenous population which adds to the plethora of experience that our Fellow will have.

10. Additional Information This is the rst time that our RRDOHT will receive a Fellow to help advance our work. The lifestyle and cost of living in the Northwest is safe, affordable, and with a strong sense of community that is not usually available in bigger, urban centers. We prefer that our Fellow will live within our District for the year with hybrid options based on need and preference. We will support district travel, equipment required, and other needs to perform all job duties. We are also open to having sessions with prospective candidates so they can ask questions before they make their decision to commit. Another option is to do a site visit that will allow the Fellow to understand our District environment / geography.
