2003wasthethirdyearofMinisterMeyer’stenureasHealthMinisterand thecontinuationoftheEmergencyMedicalServicesasaMinisterial Project.EMSisgratefulfortheMinistersinsight,encouragement,criticism andsupport,whichareundoubtedlyshapingtheWesternCapeEMSfor thefuture.

ThefollowingreviewreflectstheactivitiesofEMSoverthelastcalendar yearanddocumentstheachievementsattainedthroughthecommitment andhardworkoftheservice.

EMSManagement

ThepeoplecomplimentofEMSnownumberssome1200staffwith289 currentlyremainingasemployeesoftheCityofCapeTown.

Negotiationsonthetransferoftheremaining289EMSpeopleare continuingandindicationsarepositivethattheiranguishwithregardsto insecurityanduncertaintywithrespecttothefuturewillbesolvedinearly 2004.

ThedelayinthetransferoftheCityofCapeTownpersonnelhasseverely hamperedtheabilityoftheservicetodeliverintheProvinceonits mandateofEmergencyCare.

Thetransferoftheremainingpeopleearlyinthenewyearwillinject renewedenthusiasmandcapacityandenabletheservicetoperform. 2004isthereforewelcomedwithexpectantanticipation.

EMSwelcomedDrKrishVallabhjeeastheimmediatetopmanagement supportduringtheyear.Krishcomesfromaruralhealthandpolicyand planningbackgroundandwillbringessentialskillstoensuringthe successinthedevelopmentofEMSintheProvince.DrBethEngelbrecht remainsachampionofEMSinHealthexecutivemanagementand deservesourheartfeltthanksforheractiveinvolvementinEMSduringthe managementtransition.Hergentle,professional,businesslike,calm approachtoemotiveandoftencontroversialissuesspeakstoher maturity.Wethankherforherabilitytomotivatepersistenceand commitmentinthefaceoffrustratingandoftendesperatetimes.

ProfessorHousehamhasbeenashamelessyetcriticalsupporterofEMS alongwithourMinsterandfortheirappropriatechampioningofEMSwe aregrateful.Emergenciesshouldbedealtwithfirst,weagree.

DrRobertson,MrPapuandDrSmithremainedastheexecutive managementofEMS.Theirrelentlessdoggedcommitmenttoachieving thevisionandmissionofEMSremainsapillarofthesuccessindelivering ontheexpectationsoftheMinisterandthepeopleoftheWesternCape whosehealthinterestsherepresents.EMSmanagementremained collectivelyfocussedanddeterminedin2003,intheprocessmakinghuge personalsacrifice.

EMScontinuedtobuildmanagementcapacity(twoyearsagotherewere almostnomanagersorsupervisorsinruralareas).MrMaralackwas appointedinanactingpositiontomanagetheBoland/Overbergregion whileMessrsVisser,BritsandAhmedcontinuedtomanagetheWest Coast,SouthernCapeandMetropolitanRegionsrespectively.

Supervisorsappointedduringlastyearcontinuedtodevelopandimprove localservicemanagement.Nonewsupervisorswereappointedduring 2003asbudgetpressuresqueezedtheorganizationagain.

TheLifeSupporttraininglevelsofpersonnelachievedduringtheyearare 46%BasicLifeSupport,42%IntermediateLifeSupportand2% AdvancedLifeSupport.Theprojectedtargetsforskillslevelsare20% AdvancedLifeSupport,50%IntermediateLifeSupportand30%Basic LifeSupport.

Thedistributionofstaffisasfollows,9%managementandsupervisors, 8%administrativeandsupportpersonneland83%clinicalpersonnel.

ThepostsofAssistantDirectorFinanceandHumanResource Managementwereadvertised.

Contractorswhothenbecamepermanentemployeesfilledthirty-nine posts.

ManagementCapacityisaspecificfocuswithintheEMSHuman ResourceDevelopment.MrSuttondesignedandpresenteda ManagementDevelopmentprogramforthefirsttimethisyear.The programaddressescapacitybuildingwithspecificreferencetoEMS. Forty-threemanagersparticipatedintheprogramandwillcontinueto progressnextyear.

AworkshopofEMSmanagerswasheldduringtheyearattheSports ScienceCentreinNewlandstofocusoncollectivestrategicissuesand thefundamentalsofEMSmanagement.Issuesdiscussedatthe workshopwillbetakenforwardduring2004.

TheCapeTechnikonAcademyofEmergencyCarewasofficiallyopened bytheMinisterofHealthattheoldOttoDuPlessisNursingCollege premises.MrLloydChristopherbeganhistenureasPrincipleofthe InstitutionwiththesupportofGavinSuttonasoperationsmanager.

TheAcademyis,likemostSouthAfricaneducationalinstitutions, undergoingcontinuoustransformation,whichisoftenunsettlingand disturbsthefocusontraininganddevelopment.Theacademypersonnel haveadaptedandcollectivelywork-shoppedthevision,missionand strategicdirectionoftheinstitutionintothenextfiveyears.

ThefirstyearoftheNationalDiplomainEmergencyMedicalCarewas completedforthefirsttimewith12studentsgoingthroughtothesecond year.

ElevenparamedicssuccessfullycompletedtheAdvancedLifeSupport Paramedicprogram.Congratulationstoall.

RobinHenekejoinedthestaffasaparamedictrainer,whilePeterLesch joinedtoassistwithIntermediateLifeSupporttraining.

MosesHuman,TrevorLeibrandtandJacobVanZyllefttheAcademyand optedtocontinuetheircareersintheMetropolitanRegionDivisionsas supervisors. LabourRelations

TheStaffPerformanceManagementSystemwasintroducedforthefirst timein2003.

GenderForum

AstaffGenderForumwasestablishedthisyeartoassistwithadviceon genderissuesintheworkplace.Theforumhasestablishedaconstitution andworkingframeworkandwillprovidearesourcewheregenderissues canbedebated,consultedandaddressedtoassistwithmanagement decision.

ThepolicyonpregnancyintheworkplaceforEMSpersonnelwillbethe firstproductoftheforumwhenitisfinalisedin2004.

AbsenteeismandSickLeave

Absenteeismandsickleavelevelshaveimprovedbuttheman-dayslost totals12882,theequivalentof6442ambulanceshifts(includesnormal leave).

GrievancesandDisciplinary

Grievanceanddisciplinaryregisterswerekeptforthefirsttimeand15 Individualgrievancesand38disciplinaryactionswererecorded.

TransformationandEquity

TheEMSTransformationSteeringCommitteemetforthefirsttimeto constructaframeworkfromwhichtomonitortransformationwithinEMS.

SmokingPolicy

IntheinterestofgoodhealthEMShasembracedanon-smokingpolicy, whichmakesallEMSfacilitiesandvehiclesnon-smokingareas.

EMSRESOURCES

Finance

TheEMSbudgetincreasedby 8.3% to R165642000 forthecurrent financialyear.

Mostoftheincreaseinfundingisprovidedforpersonnelcosts(one-man ambulanceeliminationandsalaryprogression).

Thisfundinglevelignoredprovisionforthefull17000000kmtravelled everyyear(deficitofR4million,eachkmcostsR1)andthefunctionsof theRedCrossAirMercyService(deficitofR4million).AsaresultEMSis projectedtooverspendbyR12million.

RegionalManagershavedoneasuperbjobofcontrollingexpenditureand whereasEMSeveryyearincurreddeficitsofR10-20millionwithLocal Authoritiesthishasnowbeenbroughtundercontrol.Overtimehasbeen severelyrestrictedandthetotalearningsofpersonnelintheshortterm havesufferedseverely.

TherevenuegeneratingcapacityofEMShasbeensignificantlyimproved bytheinstallationoftheDelta9System.MrsSadieandthreeclerksare nowworkingfeverishlyatdevelopingstructuretotherecoveryofprivate andpublicaccountsforthefirsttime.TherevenuerecoveryofEMSis projectedtobeatnewhighsfortheyear.

EMSpreparedasolidbudgetargumentfor2004/5andasaresultan additionalR30millionhasbeenallocatedtoEMSforthenextfinancial year.

Therescuevehiclesabovereplacetrailersintheruralareasandrestore capacityandpridetoourrescueservicesinsmallertowns.

TheDepartmentofTransportthroughGovernmentMotorTransportand MrJohanKoegelenberghascontinuedtoprovideexcellentsupportto EMSandEMSisgratefulfortheenthusiasticcommitmentthatis continuouslyprovided.WethanktheMinister,MrsEssopandtheHeadof DepartmentMrManyathifordemonstratingthevalueofinterdepartmental co-operation.

VehicleWorkshops

Thevehicleworkshopsintheregionshaveallbeenregisteredas merchantswithFirstAutoandareprovidingservicestoGMTforEMSand otherEmergencyServices.

Revenuerecoveryfromtheworkshopshasimprovedremarkably, particularlyintheWorcesterworkshop.

E-fuelwasintroducedthisyearinco-operationwithGMTandwill significantlyimpactontheriskofvehiclefuelfraud.

DrivingSkills

88%ofEMSpersonnelnowhaveCode10DriversLicencesand98% havePublicDrivingPermits.

Drivereducationandtrainingwasinitiatedagainthisyearnowhasa drivinginstructortrainedbytherelevantmanufacturer.Allnewappointees willbetrainedinEmergencyDrivingtechniques.

Severalaccidentsthisyearandhaveraisedattentiontotheneedfor continueddriversupport.Drivingsafelyrequiresbehaviouralandattitude changewhichisuptoeveryoneofus.

ApaneloffourofficersnowinvestigateseachandeveryEMSvehicle accident,withtechnicalsupportfrommanufacturersandvehicle monitoringsuppliers.

InformationTechnology

Basicinformationtechnologyi.e.networkconnectionandpersonal computers,wereestablishedataregionallevel(eachregionalmanager, AssistantDirector)forthefirsttime.

Thecapacitywillberolledoutnextyeartoincludeeachstationorsector toassistwithbasicadministrationandcommunication. TheAcademywasnetworkedforthefirsttimeandinstructorsand studentsnowhaveaccesstoe-mail,theInternetandbasiccomputer resources.

Facilities

AnEMSfacilitiesauditwascompletedthisyear,whichidentifiessevere deficienciesinfixedinstallationsforEMS.

ThereporthasbeenforwardedtoPropertyManagementandthe Departmentforaction.

NewfacilitiesforEMSwereopenedattheGrabouwCommunityHealth Centre,ClanwilliamandPriceAlbertthisyear.

TheDivisionalModelfacilitatedthenecessityfortheDelftEMSBaseto befullyutilisedandforastationatLentegeurHospital.TheLentegeurand Delftmanagementarecomplimentedfortheirassistanceinfacilitatingthe operationalstatusofthesebaseswithinshorttimeframes.

EMSheadofficehasundergoneafacechangeandvisitingpersonnelwill noticethatthedécorreflectstheworkandbusinessofEMS.

MedicalEquipment

ThemedicalequipmentstatusofEMSremainscritical.

EMShasaR15millionequipmentbacklog,whichincludesbasic equipmentlikebag/valve/masks.

EMScommitteditsentireMedicalEquipmentbudgettoCommunications Equipmentin2003becauseoftheabsenceoffundingprovidedinthe budgetforthiscriticalelement.

TheDepartmentprovidedR1.5millioninadditionbutnoequipmenthas beenprocuredtodate.R900000worthofHydraulicTools,Ventilators andOxygenregulatorsareonorder.

TheoverlybureaucraticcommitteeapproachbytheDepartmenttoavery simplebasicprocessofEMSequipmentprocurementmustnotgo unchallenged.EMSisfightingtobuybasiclifesavingequipmentthatit doesnothavewhilethedepartmentreplacesexistingequipment!

SupplyChainManagement

Inventoriesbyambulancewerecompletedresultingintheauditedand quantifiedshortageofEMSequipmentthevalueofwhichamountstoR15 million.

Disposablesupplychainmanagementhasimprovedandshortagesof disposablestockshavediminished.

EMSOPERATIONS

ResponseTimes

TheaverageresponsetimesfortheProvincehaveimprovedtoan averageof30minutesoutofbuiltupareasand22minutesinbuiltup areas.Thisignoresthosecallsthatwedonotrespondtowhichofcourse, ifthereisonecallwitharesponsetimeofinfinity,makesourresponse timesinfinite.Calculatingaverageresponsetimescloudstheactual deficiencyandcouldprovidemanagementwithafalsesenseofsecurity. Thecollectivedelaysforanaverageresponsetimeof22minutesacross 15000callsintheMetropolitanareaaresignificant.

TheNationalnormsforresponsetimeare15minutesinabuiltuparea and40minutesinanoutofbuiltuparea.Ourresponsetimesfall significantlyoutsidethosetargetsinmanyareasandthereistherefore muchworktobedone.Themetropolitanareaisofgreatconcern.

ParticularproblemareasaretheMetropolitanAreaofCapeTown, BeaufortWest,Knysna(No serviceinPlettenbergBay ),PrinceAlbert andMurraysburg.

MetropolitanRegion

Themostsignificantchangeinoperationsintheregionhasbeenthe institutionoftheDivisionalModel,whichdefinesfourdivisionsinthe MetroRegioneachwithadistinctmanagementandresourcestructure.

InadditiontheProvincialEMShasassumedoperationalcontroloverthe personneloftheCityofCapeTown,whichasaninterimmeasureshould improvetheservicelevelswithinthemetropolitanarea.

Uniforms

Uniformallowanceswereprovidedtopersonnelforthefirsttimeto facilitateprocurement.Thestandardbluejeanandshirtuniformwillbe retainedfornextyear.

Auniformcodewasissuedthisyear,whichdetailstheinsigniaandformat oftheuniform.

Onlysupervisorswillwearshoulderrank.

Personnelwillbeidentifiedbynameandqualificationontheuniform.

Personalprotectiveclothing(Helmets,reflectivevests,gloves,eye protection)willreceiveattentionin2004.

Volunteers

EMShasavolunteercomplimentof626personnelmadeupof269 TrainedPersonneland400others. TheVolunteerAssociationwasformallyconstitutedthisyearandcurrently residesunderthechairmanshipofMichaelSerelina.

Thevolunteerguidelinesandconstitutionofindependentvolunteer servicesarebeingrevisited.

OperationalMeetings

Thequalityandoutcomesofmeetingsheldintheregionshasimproved alongwiththecontactandinteractionwithlocalgovernmentandother localstructureswithrespecttoEMS.

Communicationhasimprovedasaresult.

EMSSPECIALOPERATIONS

WildernessSearchandRescue

Theco-operativestructureofWSARcontinuestodemonstratethevalue ofservicesandorganizationsworkingtogether.

TheWSARAdvisoryCommitteewasestablishedintermsofthe MemorandumofUnderstandingandnowperformsanoversightrolein designinganddevelopingtheWSARsystemintheWesternCape.

NewWSARcellscontinuetobedeveloped.Thisyearcellswereinitiated inGeorge,PaarlandtheWildernessareas.

TheAirSupportWorkGroupwasconstitutedtooverseetheoperationsof helicopterrescueandsupportinpartnershipwiththeRedCrossAirMercy ServiceandSouthAfricanAirforce.

SwiftwaterrescueequipmentwaspurchasedfortheEasternDivisionto provideresponsecapacitywithrespecttotheBergRiver.

UrbanSearchandRescue

TherecentwarningswithrespecttoanearthquakeintheCapeTown areahaveacceleratedattentiondevotedtoUrbanSearchandRescue.

MrVanRensburgandDrSmithhavefacilitatedavisitbyanUSAR consultantasapreludetodevelopingUSARteamsintheWesternCape nextyear.

RedCrossAirMercyService

EMShasdependedheavilyontheRedCrossAirMercyServicefor AdvancedLifeSupportinterfacilitytransfersandprimaryresponses.EMS wouldnothavebeenabletodeliveronitsmandatein2003withoutthis support.

TheAirMercyServiceprovidestheprovincewithaflexibleAeromedical ModelwhereEMScanchoosethemostappropriateresponsebyfixed wing(normalaeroplane)orrotorwingaircraft(helicopter)toaparticular incident.

Theservicehasthreeaircraftavailable,athreebedPilatusPC12,a doublecotMesserschmidtBO105HelicopterandanAlouetteIII Helicopter.

Theserviceflewtheequivalentof700000kmthisyearwith700patient contacts.

SkymedIItherescuehelicopterflew65missionsandrescued45 patients.

TheEMScalltakingthroughtheNationalEmergencyNumber10177was consolidatedthroughonecentreineachoftheDistrictGovernmentareas ofSouthernCape(George),Boland(Worcester,Overberg(Bredasdorp), CentralKaroo(BeaufortWest),WestCoast(Mooreesburg)and.

All10177emergencycallsnowgotothosecentres,whosecapacityhas beenseverelychallengedandurgentlyrequiresattention.

ApilotComputerAidedDispatchSystemhasbeeninstalledinPinelands andcurrentlyincorporatesadigitalelectronictelephoneswitchand ComputerAidDispatchSoftwareapplicationandcomputernetwork. ThecommunicationscentreatPinelandswasrenovatedbytheProvince andnowhascomfortablemodernoperatorconsoles.

ThemissinglinkintheCADSystemistheAutomaticVehicleTracking System,whichwehopewillbeprocuredbytheTransportDepartment earlyinJanuary2004.Thesystemiscriticallydeficientwithoutvehicle tracking.

TheEMSdispatchcentreisindesperatecircumstances.

EMSCommunicationshasbeenlistedasakeypriorityinnextyear’s budgetandstrategicplantoensurethatcommunicationsanddispatch technologyreceivestheappropriateattention.

CurrentprojectionsindicatethatwewillbeabletorolloutCADSystems toGeorgeandWorcesternextyear(PossiblyBredasdorp).

EMShavebeencontractedtoprovidemedicalsupporttoAntarcticTeams shouldexceptionalmedicaleventsorincidentsrequirecasualty evacuation.

CapeTownRadioSupport

EMScurrentlysupportsCapeTownradiowithradiomedicaladvicefor shipsatsea.ThisservicewashandedovertoEMSin2003froma practiceoflocalGeneralPractitioners.

EMSClinicalAuditandSupportincludingCriticalIncident InterventionandStaffSupport

SisterAdamsandSisterCrossleyattendedto138incidentsand interventionsduring2003andcontinuedtoprovidethisessentialservice dayandnightthroughoutthewholeprovince.

DrsDeVriesandLouwprovidedamorecontinuousmedicalsupportto EMSduring2003throughtheirappointmentoncontract.Itishopedto employthreeMedicalOfficersin2004.

Theyinvestigatedandintervenedinpatientcomplaintswithrespectto carelevelsandthroughassistancewitheducationandtrainingmade someimpactonthequalityofcaredeliveredbyEMS.

EMSandtheDepartmentofCommunitySafetyhaveco-operatedin launchingtwoRoadSafetyPatrolVehiclesinsupportofArriveAlive.The twovehicleswillbejointlycrewedbytrafficandEMSpersonneland performfunctionswithspecificreferencetoroadsafety. EMSRadioCommunications

TheMetropolitanAmbulanceswereallfittedwithTrunkedRadiosand beganoperatingontheTrunkedRadioSystem.

TherealityistheEMSisstrugglingtomakeupthegroundlostsincethe earlynineties.Eliminationofone-manambulancesandsalaryprogression improvesthequalityofcarebutdoeslittletoresponsetimes.Thereal capacityoftheservicetorespondwithinexpectationshasnotsignificantly improved.Ourambulanceslookbetter,wehavenewuniformsand badges,ourtrainingissuperb,wearebetterpaidbutwehavetoask whetherourpatientsarereceivingthequalityserviceleveltheydeserve.

TheabovesaidtheEMScollectivecontinuestorisetothechallengeand wewillnotrunawayfromafighttoensurethatEmergencyCarebecomes acontinuouspriorityinthisProvince.Wearepassionateadvocatesfor non-violence,decreasingthelevelsoftraumainsocietyandforseeing thatpatientsreceiveappropriatecareinemergencies.

Thecreditforanysuccessissharedbythe1800permanentand volunteerpersonnelwhopitchupforworkeverydayandnightand committhemselvesenthusiasticallyandwithoutcomplainttothedifficult tasksweface.

MorethananysectoroftheHealthFraternitywefacehorroronadaily basis,therealityofwhichseemstogoapparentlyunnoticedbypublic, politiciansandmanagersalike.Wewillcontinuetoraiseawarenessand bringappreciationforthelifesavingworkoffrontlineEmergencyCare providers.

Inconclusion,afterthesirenshavefaded,handscleansedandtheblood washedfromourclothes,thereismuchastheWesternCapeMETRO EMStobeproudofandbeenthusiasticallypositiveabout,toregenerate ourbatteriesforthenexttenyears.TheWesternCapeEMS,asasystem, isestablishingitselfasaleaderinAfricaandtheWorld,enoughsaid.
