We work with primary care physicians (PCPs), hospitals, specialists, behavioral health providers, and other providers for your health care needs. We are a health plan partner with the Health Care Authority (HCA). Your health care will be provided by doctors who are in our provider network. We provide Washington Apple Health and Behavioral Health Services Only (BHSO). Apple Health coverage includes both physical and behavioral health care services. UHCCommunityPlan.com CSWA.15.999.21.9.007 How to use this directory Choosing a Primary Care Physician (PCP) Apple Health members need a primary care provider (PCP). There are several types of PCPs in this directory. Each covered family member may choose a different PCP in UnitedHealthcare Community Plan. Except in an emergency, your PCP and staff will give or manage all your health care. This includes referrals to specialists. PCPs may be family or general practitioners, internists, pediatricians, or other providers approved by UnitedHealthcare Community Plan. Changing your PCP If you want to change your PCP, call Member Services. They will help you pick a new PCP. You may change your PCP during the year. You may also contact a provider to ®nd out if the provider contracts with UnitedHealthcare Community Plan. When you call, be sure to mention UnitedHealthcare Community Plan. Ask if the provider is taking new patients. Seeing another doctor or specialist Your PCP might want you to go to a specialist. A specialist is a doctor who treats a special problem, like a dermatologist or a heart doctor. Your doctor will help you ®nd the right specialist. If you have a complex illness, you may need to see the specialist many times. You may see your specialist as often as needed. If you have complex care needs, a specialist can be your PCP. If you need this, call Member Services. We will help you ®nd a specialist who can meet your needs. Women©s health care services If you are female, you may want to see a provider in our network who offers women©s health care. You can get these services from an in-network women©s health care provider without a PCP referral. These women©s health services are covered without a PCP referral or health plan preauthorization: · Maternity care, including prenatal, delivery, and postnatal care · Routine gynecological exams · Examination and treatment of disorders of the female reproductive system · Other health problems treated during a woman©s health care visit, if the treatment is within the provider©s scope of practice and the service is not excluded If you need to see a women©s health provider who is not in our network, you must get a referral from your PCP. UHCCommunityPlan.com CSWA.15.999.21.9.007 Behavioral health services As a UnitedHealthcare Community Plan member, you©re eligible for behavioral health services, including mental health and substance use disorder treatment services. Mental health providers are listed in this provider directory. They can help you with personal problems that may affect you and/or your family. These problems may be stress, depression, anxiety or using drugs or alcohol. Apple Health members get behavioral health or substance use disorder treatment services through any of the below: · Your primary care provider (PCP) · A Community Mental Health Center or substance use disorder agency · Another behavioral health provider Talk to your PCP today about any health or behavioral health problems you have. Your PCP will refer you to a behavioral health provider with UnitedHealthcare Community Plan. They can refer you to specialists, and other facilities, such as hospitals and substance use and treatment services. Your PCP will stay involved with you through the stages of recovery: Health, Home, Purpose and Community. There are some treatments and services that need preapproval before you can get them. To learn more, talk to your doctor. Crisis services If you think you have an emergency, no matter where you are, call 911 or go to the nearest hospital location where emergency providers can help you. As soon as possible, call your doctor or behavioral health provider to arrange for follow-up care after the emergency is over. Mental health crisis If you or someone you know is experiencing a mental health crisis, call the number listed below for the applicable county. A mental health crisis is a moment when someone©s behavior can put themselves or others in danger, especially if they do not get help.
