Cerebrospinal Fluid: History, Collection Techniques, Indications
REVIEW ARTICLE J Bras Patol Med Lab. 2020; 56: 1-11. Cerebrospinal fluid: history, collection techniques, indications, contraindications and complications Líquido cefalorraquidiano: história, técnicas de coleta, indicações, contraindicações e complicações 10.5935/1676-2444.20200054 João Paulo S. Oliveira; Natalia T. Mendes; Álvaro R. Martins; Wilson Luiz Sanvito Hospital Santa Casa de São Paulo, São Paulo, São Paulo, Brazil. ABSTRACT This article is based on a historical review of the medical literature with the purpose of acknowledging the historical stages and anatomical findings over the years, which led gradually to performance of the first lumbar puncture by Quincke, as well as collection techniques and analyses, allowing it to be an irreplaceable diagnostic tool in daily clinical practice. Cerebrospinal fluid analyses have continued to develop and nowadays play a major role in diagnosing and understanding the physiopathology of a great variety of neurological conditions. Biomarkers and genetic sequencing have recently been the target of multiple studies and are implicated as promising diagnostic tools of a large range of diseases. Key words: cerebrospinal fluid; history; spinal puncture. -

Cambridge University Press 978-0-521-86919-5 - The Pseudotumor Cerebri Syndrome: Pseudotumor Cerebri, Idiopathic Intracranial Hypertension, Benign Intracranial Hypertension and Related Conditions Ian Johnston, Brian Owler and John Pickard Excerpt More information 1 Introduction The condition or syndrome to be considered in this monograph has been a clearly recognized clinical entity since the descriptions given by Quincke (1893, 1897) and Nonne (1904, 1914) over 100 years ago. However, reports of cases which were almost certainly examples of the same condition undoubtedly antedated their pioneering accounts by almost four decades. The essential elements of the syndrome are the symptoms and signs of intracranial hypertension without ventricular dilatation and without an intracranial mass lesion. For reasons which will be made clear in the following chapters, we shall call it the pseudotumor cerebri syndrome (PTCS) although quite a variety of terms have been applied to it. It is a particularly intriguing condition for a number of reasons, as follows: 1. Clinically the condition presents an essentially pure picture of raised intracranial pressure (ICP) without focal neurological disturbance and without investigative evidence of structural disturbance, either focal or general. As such, it is a condition which manifests, in isolation, what is a critical component of many neurological and neurosurgical conditions, i.e. intracranial hypertension, thereby creating a situation in which the pathological effects of this component exist in a pure form. 2. Despite much speculation and numerous clinical and laboratory studies (although clinical investigations are constrained by the exigent circumstances of the condition and laboratory studies by lack of a suitable model) there is still no clear consensus on its mechanism, although the predominant view is that the intracranial hypertension is due to a disturbance of cerebrospinal fluid (CSF) dynamics. -

Fixed and Dilated: the History of a Classic Pupil Abnormality
HISTORICAL VIGNETTE J Neurosurg 122:453–463, 2015 Fixed and dilated: the history of a classic pupil abnormality Peter J. Koehler, MD, PhD,1 and Eelco F. M. Wijdicks, MD, PhD2 1Department of Neurology, Atrium Medical Centre, Heerlen, The Netherlands; and 2Division of Critical Care Neurology, Mayo Clinic, Rochester, Minnesota The aim of this study was to investigate the development of ideas about the nature and mechanism of the fixed dilated pupil, paying particular attention to experimental conditions and clinical observations in the 19th century. Starting from Kocher’s standard review in 1901, the authors studied German, English, and French texts for historical information. Medical and neurological textbooks from the 19th and 20th centuries were reviewed to investigate when and how this in- formation percolated through neurological and neurosurgical practices. Cooper experimented with intracranial pressure (ICP) in a dog in the 1830s, but did not mention the pupils. He described dilated pupils in clinical cases without referring to the effect of light. Bright demonstrated to have some knowledge of the pupil sign (clinical observations). Realizing the unreliability of the pupil sign, Hutchinson in 1867–1868 tried to reason in which cases trepanation would be advisable. Von Leyden’s 1866 animal experiments, in which he increased CSF volume by injecting protein solutions intracranially, was the first observation in which the association between fixed dilated pupils and increased ICP was established. Along with bradycardia and motor and respiratory effects, he noticed wide pupils were usually present in a comatose state. Asymmetrical dilation could not always be attributed to increased ICP, but to an oculomotor nerve lesion. -
