The Walton Centre has a strong opening position: It has a high clinical and public reputation It has a successful financial and operational track record, with continuity of service risk rating (CSRR) of 4 and governance risk rating of Green for both 2013-14 outturn and 2014-15 plan It is in the lowest Care Quality Commission risk band and in its most recent inspection report published in 2014 met all standards.     REGULATION 28: REPORT TO PREVENT FUTURE DEATHS REGULATION 28 REPORT TO PREVENT FUTURE DEATHS THIS REPORT IS BEING SENT TO: East Lancashire Hospitals NHS Trust 1 CORONER I am Rachel Galloway, assistant coroner for the area of Blackburn, Hyndburn and Ribble Valley. 2 CORONER’S LEGAL POWERS I make this report under paragraph 7, Schedule 5, of the Coroners and Justice Act 2009 and regulations 28 and 29 of the Coroners (Investigations) Regulations 2013 3 INVESTIGATION and INQUEST On the 28th September 2016 an inquest was opened into the death of Patrick Clifford. Evidence was heard at inquest on the 22nd March 2017. The inquest was adjourned part heard on the 22nd March 2017 and completed on the 11th October 2017. A narrative conclusion was left: Mr Clifford fell backwards following a faint in the toilet on Ward B4 at the Royal Blackburn Hospital on the 19th March 2016. He suffered a fractured acetabulum as a result. This was treated conservatively. His condition deteriorated over time. He developed pneumonia due to immobility and heart failure and passed away on the 18th September 2016 at Springhill care home in Accrington. 4 CIRCUMSTANCES OF THE DEATH Mr Clifford suffered a fall in the toilet at Royal Blackburn Hospital where he was a patient on the 19th March 2016. No risk assessment had been carried out but I found that the fall would not have been prevented had a risk assessment taken place, as there was no requirement for Mr Clifford to be supervised in the toilet due to his level of his mobility.

Additionally, a Roll of Honour book would be compiled in order that all those who fell would become “more than just a name”. The aim was to complete the work by September 2014 in time for the anniversary of the dedication of the chapel in September 1922. PARISH COUNCIL Chairman and Vice-Chairman At the Meeting of the Council held on 20 May 2013 Councillor Mrs J Burton was elected as Chairman of the Parish Council for the year 2013/2014, and Councillor Mr F Hodgkinson was elected Vice-Chairman. 2. PARISH COUNCILLORS At May 2013 Wrightington Parish Councillors were:- APPLEY BRIDGE Mr J Clinch 16 Clifton Crescent, Wigan. Mr I McRae 11 Rookery Avenue, Appley Bridge, Wigan. Mr P Gartside 2 Chisnall Avenue, Wrightington. MOSSY LEA Mrs Judith Burton 2 Courage Low Lane, Wrightington. Mr Frank Hodgkinson Acrefield House, 17A Broadhurst Lane, Wrightington. Mr F Johnson 351 Mossy Lea Road, Wrightington. The Parish Council began the year with 6 members. The vacancies were advertised following which, in November 2013 the Parish Council appointed Mrs Amanda Kay to the vacant Appley Bridge Ward Councillors post. The remaining Parish Council Vacancy has been advertised. The Clerk has attended a number of Liaison Meetings with West Lancashire Borough Council Officers and other Clerk’s to assist with various issues arising during the course of the year and also attended the LALC Annual Conference at Leyland and the LCC Parish & Town Council conference at County Hall. Councillors have attended briefing sessions, Councillors Code of Conduct Training, open days, public consultations and site meetings to address issues which arise in the Parish as required. Councillors continue to receive information, documents, agenda and minutes electronically to save on rising postage costs. The Chairman was once again nominated to attend the Buckingham Palace Garden Party.
