Railway Accident Investigation Report Railway Operator : Kyushu Railway Railway accident investigation report Railway operator : Kyushu Railway Company. Accident type : Level crossing accident. Date and time : About 16:33, October 3, 2014. Location : Takanosu level crossing, class three level crossing equipped with road warning device, without automatic barrier machine, located at around 35,613 m from the origin at Jono station, between Buzen-Kawasaki station and Nishi-Soeda station, single track, Hita-Hikosan Line, Kawasaki Town, Tagawa District, Fukuoka Prefecture. SUMMARY The running train collided with a pedestrian walking in the class three level crossing road. The pedestrian was dead by the accident. PROBABLE CAUSES It is highly probable that the accident occurred by the collision of the train, which approached to the class three Takanosu level crossing where the road warning device was operating, with a pedestrian entered from right side and was walking in the level crossing road without noticing the approaching train and the whistle, before she went out of the level crossing. It is highly probable that the pedestrian entered into the level crossing where the alarm of the road warning device was sounding and did not notice the whistle by the train because she was hearing impaired and could not hear the alarm sound and the whistle. It could not be determined why the pedestrian entered into the level crossing where the red warning lights were blinking, but it is probable that the following situations effected the occurrence of the accident. 1. It was difficult for the pedestrian to see the blinking red warning lights by the narrow field of vision due to the parasol and the broad brimmed hat. 2. The blinking red warning lights were hard to see by the reflection of the sunlight. .

    Railway accident investigation report Railway operator Kyushu Railway Company Accident type Heavy property loss without casualty Date and time About 05:18, September 18, 2017 Location In the premises of Nogata station, Chikuho Line, Nogata City, Fukuoka Prefecture SUMMARY On September 18, 2017, the driver of the shunting vehicles for the inbound Electric 6620M train, which was the shunting vehicles scheduled to be operated as the inbound Electric 6620M train, composed of two vehicles started from Nogata station bound for Wakamatsu station of Chikuho Line, Kyushu Railway Company, started the shunting operation in the route from track 25 to the east lead track No.1 via track 15 in the premises of Nogata station, at about 05:15. After that, the vehicles collided with the car stop installed in the end edge of the east lead track No.1 and destroyed it, furthermore, all two axles in the front bogie of the front vehicle derailed to right by the shock and the vehicle body of the front vehicle disturbed the main line in the up track. As the measures such as train protection etc., accompanied with disturbing main line in the up track had not been implemented, the inbound Electric 6520H train, composed of 3 vehicles, and the shunting vehicles for outbound Deadhead Diesel 1533D train, shunting vehicles composed of four vehicles scheduled to be operated as the outbound Deadhead Diesel 1533D train, were passing through the disturbed track, and the car side pilot lamp of the shunting vehicle of the outbound Deadhead Diesel 1533D train contacted with the right edge of the front head of the shunting vehicle of the inbound Electric 6620M train and both vehicles were damaged.

The driver of the outbound limited express 2019M train, after checked the spot where the abnormal sound was noticed and inspected the train, restart train operation obeying the instruction of the train dispatcher.
