1. Job Summary

This is a replacement post. The post holder will play a key role in realising our ambition to build a high quality clinical service by bringing new skills to our Microbiology Service. The successful applicant will be a committed clinician with specialist training in Infectious Diseases and Medical Microbiology or Medical Microbiology and Virology with a track record of excellent clinical service delivery, service improvement, teaching, training and other qualities as identified in the person specification. The successful applicant will be expected to develop their leadership skills to support succession planning for the department. It is a requirement of the post that every doctor takes part in annual job planning and annual appraisal as part of the revalidation cycle. Job planning is outlined in point 4, as is a draft proposed job plan.

2. Duties / Responsibilities of the Post General Clinical The content of this Job Description represents an outline of the post only and is therefore not a precise indication of duties and responsibilities. The Job Description is therefore intended to be flexible and will be subject to review and amendment in the light of changing circumstances, following consultation with the post holder.

The post-holder will be responsible to the Microbiology Clinical Lead and then to the Divisional Director of Cancer and Clinical Support Services at The Hillingdon Hospital, with a job plan negotiated annually. It is a 10 PA post – 8 direct clinical care (DCC), 2 supporting professional activities (SPA), the latter includes clinical and educational supervision, audit and personal CPD. Individuals who are unable for personal reasons to work full-time will be eligible to be considered for the post; if such an individual is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues. CPD must be included as part of annual appraisal which is mandatory and required for the revalidation process. A proportion of this DCC and SPA relating to laboratory services is delivered through North West London Pathology (NWLP) and for these sessions the post holder is responsible to the Lead Microbiologist for NWLP, then Head of Specialty for Infection and Immunity and then to the Clinical Director for North West London Pathology. The post holder is an employee of The Hillingdon Hospitals NHS Foundation Trust.

The post holder will, together with consultant colleagues and other health care professionals in the Directorate and North West London Pathology:

 Provide an efficient, cost-effective and comprehensive diagnostic microbiology service for all patients and support continuing service developments.  Liaise with clinical colleagues regarding investigation and management of patients with suspected or confirmed infections.  Liaise with and advise the Health Protection Teams and Public Health England.  Work in close liaison with the Occupational Health department.  Provide a clinical service to all clients of Hillingdon Hospitals Foundation NHS Trust, and General Practitioners who send samples to the Hillingdon Hospitals NHS Foundation Trust Laboratories (see Clinical Responsibilities).  Support Hillingdon Hospitals in aspects of Infection prevention and control, as determined by discussions with the Lead Microbiologist in consultation with the Director of Infection Prevention and control  Support Hillingdon Hospitals in aspects of antimicrobial stewardship, as determined by discussions with the Lead Microbiologist  Provide with consultant colleagues ICU, NNU and other ward rounds, Diabetic Foot Clinic liaison, and TB and other MDTs as agreed locally  Support and advise laboratory staff regarding day-to-day performance of diagnostic tests, and reporting of results.  Contribute to the NWLP consortium as agreed through the Joint Governance structure.

The post holder will participate in the on-call microbiology rota on a 1:3 basis. This is under review subject to negotiation with North West London Pathology.

The successful candidate will in discussion with their colleagues mutually agree to undertake rotational departmental roles including Infection Prevention and Control Doctor (IPCD), Antimicrobial Stewardship, Lead for Clinical and Liaison and Lead for Laboratory Liaison for the department. A key focus will be increasing the clinical profile of the department.

Further representative details of these roles and activities are outlined below.

Laboratory responsibilities:

 Involvement in internal and external Quality Assurance Schemes through NWLP.  Contribute as required to maintenance of appropriate laboratory accreditation.  Contribute to education of the laboratory staff.  Work and liaise with external organisations such as PHE, DH and various Reference Laboratories.  Contribute to performance and review of vertical and horizontal audits in the laboratory and across the clinical service.  Support and advise the laboratory staff in reviewing procedures for diagnostic tests.  Be proactive in evaluating and introducing new diagnostic tests.  Contribute to and where appropriate provide leadership in the further integration of laboratory services with the NWL Pathology Network.  Attend relevant National and International meetings to keep up-to-date with recent developments in the field.  Represent the department at relevant governance and Trust level meetings.  To liaise, support and work with the Laboratory Management Team on a regular basis regarding the smooth running of the department.  Contribute to and where indicated chair relevant laboratory meetings.  To contribute to ongoing service development and improvement in microbiology in collaboration with consultant colleagues in microbiology.  Contribute to and support appropriate notification of organisms of Public Health significance.  Contribute to bench rounds at Hillingdon Hospital and subsequently at Hospital as agreed. 

Clinical Responsibilities  Contribute to appropriate and agreed liaison with clinical colleagues to provide the best possible management of infection in THH.  Provide microbiology, clinical infection and antibiotic advice and liaison to GPs using the THH service and diagnostics.  Contribute to communication of significant results to relevant clinicians.  Contribute to antimicrobial stewardship by advising on appropriate investigation and treatment.  Attend and contribute to MDTs, Morbidity and Mortality Reviews, Post Infection Reviews and Root Cause Analyses or other appropriate investigations.  Provide regular ward rounds or liaison for high dependency units such as ICU.  Contribute to bacteraemia and serious infection reviews.

Infection Prevention and Control The Consultant Microbiologist is a member of the Infection Prevention & Control Team (IPCT). The Executive Director responsible for Infection Prevention & Control is the Director of Nursing. The ICD is managerially responsible to the DIPC and professionally to the Medical Director. The IPC Team members also includes a Deputy DIPC, IPC Lead nurse, 2wte IPC nurses (Band 7), a further wte IPC nurse (Band 6/7) is to be appointed, and 1.0wte (Band 4) administrative support. The annual budget is £262,176 of which the majority is attributed to pay.

The Consultant Microbiologist will:  Contribute to the work of the IPCT, in liaison with the DIPC and IPC Nurses, to ensure a safe environment is maintained for patients pertaining to infection prevention and control risks, and appropriate treatment is advised.  Act as duty IPCD on a rotational basis with the other Consultant Microbiologists, as agreed with the Trust Infection Prevention and Control management team when the IPCD is not available.

Duties of the Infection Prevention and Control Doctor The IPCD will, subject to agreement on priorities, and within the time and resources allocated in the contract:

 Advise and support the Trust Board executive team (through the DIPC) on all aspects of infection prevention and control in the hospital and on implementation of agreed policies, including compliance with the Code of Practice on the Prevention and Control of Infections (in England).  Such matters include surveillance and control aspects of hospital infection (including building and renovation works), sterilisation and disinfection methods, types of ventilation, operating theatres, isolation facilities, kitchens, laundries, housekeeping, waste disposal, pest control, antibiotic usage and infection prevention and control aspects of nursing procedures.  Chair and/or be an active member of the Hospital Infection Control Committee.  Provide expert advice to the Water Safety Group.  Advise and contribute to the planning and implementation of relevant activities required to achieve the Trust’s aims and objectives and NHS standards and targets as set by regulatory authorities at the time.  Contribute to the planning, delivery and implementation of standards required to achieve the indemnity level appropriate to Trust.  Lead, or, where relevant, assist the DIPC in the investigation and management of outbreaks of hospital infection. Close liaison with the Consultant for Communicable Disease Control (CCDC) and Health Protection Team (HPT) will be necessary.  Advise and support infection prevention and control staff (including IPCNs, Link Nurses, etc.).  To contribute to the preparation and updating of infection prevention and control policies and Patient Information Leaflets, as agreed in the annual work plan.  Participate in the preparation of tender documents for support services and advise on infection aspects of these services following award of a contract.  Be involved in the planning and upgrading of hospital facilities.  Be involved in the setting of quality standards with regard to hospital infections and in audits of infection.  Support and contribute to the training of medical students, medical and nursing staff and other healthcare workers of all grades.  Liaise with other IPCDs and CCDC/Consultants in Health Protection in developing regional infection prevention and control programs, as appropriate.

Organisational  To work with the Head Biomedical Scientist to support the supervision of the work of the laboratory and development of new microbiological methods.  To represent the Microbiology Department whenever required at relevant committees and meetings.  To work with consultant colleagues in all pathology disciplines to provide a cost effective service that is responsive to the needs of all its users.  To work with the Infection Prevention & Control nursing staff to ensure the efficient management of the Infection Prevention & Control service.  Mandatory involvement in CPD, Job Planning and Appraisal on an annual basis.  To undertake managerial and budgetary responsibilities as required.

Clinical Governance, Audit and Quality  To be involved in the constant review of methodology of tests in the laboratory.  To support the department in achieving and maintaining UKAS Accreditation.  To maintain and develop safe systems of work in compliance with Health and Safety at Work regulations including COSHH.  To advise and decide on the tests appropriate to a given situation and how they are best performed and to assist clinical colleagues assessing the clinical significance of results.  To devise and take part in audit work relating to microbiology, Infection Prevention & Control, antibiotic usage and infectious diseases. To be proactive in assisting clinical colleagues in audit where appropriate. To chair and support the Antibiotic Stewardship Group as necessary, including reporting to the Hospital Medicines Management Committee.

Management To collaborate with the Lead Clinician, Senior Laboratory Management Team and the IPC Team in the development and implementation of all relevant policies for the running of the service. Participation in the medical advisory and liaison work of the hospital Consultant Staff Committee and to share the workload of the involvement in any other committees relating to the Microbiology Department, for example

 Pathology Management Board  Medicines Management Committee  Infection Control Committee  Infection Control Meetings  Water Safety Group  Pathology Governance Board  Antibiotic Stewardship Group, etc.

Ensure the use of the computer systems within the department and delivering education and direction of junior doctors’ activity.

The post holder will also be expected when required to involve themselves with the recruitment and appointment of junior doctors and, when necessary, laboratory staff in the department and nurses in the IPC Team in consultation with the Department of People and Development. The post holder will help to organise and give guidance to the deployment of junior staff duties including allocation and supervision of clinical work where necessary.

Antimicrobial Stewardship responsibilities:

 To liaise with clinical teams regarding antibiotic use in the Trust and to advise on prudent use of antibiotics.  To monitor the compliance with Antibiotic Guidelines in the Trust.  To prepare and publish Antibiotic Guidelines and review and update them at regular intervals.  To supervise, support and advise the Antibiotic Pharmacist.  To review antibiotic prescribing with Antibiotic Pharmacist on a daily basis and to deal with clinicians regarding non-compliant prescriptions and suggest alternative antibiotics.  To teach and support doctors (junior / senior) regarding antibiotic usage and stewardship.  To conduct antibiotic ward rounds as and when necessary, including multidisciplinary ward rounds for C. difficile cases.  To perform antibiotic audit with the clinical teams on a regular basis and take responsibility for delivery of action plans.  To attend relevant National and International meetings to keep abreast with recent developments in the field.  To lead the Antibiotic Stewardship Programme in the Trust and chair the Antibiotic Stewardship Group meetings.  To advise the Ambulatory Care Team regarding antibiotic usage.  To evaluate the relevance and need for Outpatient Parenteral Antibiotic Therapy (OPAT) Service in the Trust, and lead the development of a required OPAT service.  To attend the Hospital Medicines Management Committee (HMMC) Meeting and to advise on addition of new antibiotics onto the Formulary.  To conduct and collate surveillance of multiply resistant organisms such a MRSA, CPE, VRE, etc.

Education and training

 Contribute to induction.  Contribute to ongoing medical education and support junior doctor training through the Education Centre.  Contribute to multidisciplinary training throughout THH.  Contribute to GP CPD and training.

3. Details of the Microbiology Department/Service

 The Consultant Team consists of 3 Consultants (including this post) and provides a comprehensive clinical microbiology and infection control and prevention service for the Trust across both the Hillingdon and Mount Vernon sites. The laboratory, run by North West London Pathology, provides diagnostic services under a Service Level Agreement (SLA) to East and North Cancer Centre on the Mount Vernon site. Infection Prevention and Control services for the Cancer Centre are provided by East & North Hertfordshire Trust. Diagnostic and associated Infection Prevention and Control advice are provided to Hillingdon and Ealing CCGs.  All 3 Consultant posts are based at Hillingdon Hospital providing 24/7 clinical microbiology and infection services (1:3 on call rota) to our users. This may be subject to change with further integration with North West London Pathology.  There are strong professional links within the Trust between Microbiology and the Clinical Divisions on a day-to-day ward liaison and consultation basis. There are also close contacts with general practice.  Consultant ward rounds on the ITU/HDU are undertaken on a daily basis. Consultants also attend NNU ward rounds, the TB MDT and liaise with the Diabetic Foot service  There are established relationships with Environmental Health Officers (EHOs), Consultants in Communicable Disease Control (CCDCs) and other agencies (e.g., PHE).

The person appointed will have a shared office and a personal computer linked to the laboratory network and the internet. There is a part-time Secretary for the Microbiology Consultants.

The Microbiology Laboratory The Microbiology Laboratory is currently divided between the Pathology Department in the Hillingdon Hospital and the North West London Pathology hub site at . This laboratory will fully relocate to Charing Cross Hospital when IT infrastructure development has been completed (scheduled Nov 2019). The Hillingdon Hospitals NHS Foundation Trust, Chelsea and Westminster NHS Foundation Trust and Imperial College Healthcare NHS Trust have come together in partnership to develop a new model for delivering pathology services across North West London. The provides a great opportunity to drive translational research and innovation in all aspects of pathology, supporting training for medical and scientific staff as well as deliver outstanding quality to users and patients alike.

We believe the consolidation of pathology provision across these Trusts into a hub and spoke model, whereby the majority of routine, specialist, non-urgent activity is delivered at the hub and pathology required urgently for the immediate treatment of the patient is performed in 24/7 service laboratories based at the spokes, best delivers this vision.

North West London Pathology is hosted by Imperial College Healthcare NHS Trust and is integrated within the Imperial Governance structure.

It consists of a large main laboratory, serology department and a Containment Level 3 room. It is fully computerized using the Winpath system. For ordering tests, and for reporting results back to clinicians, the laboratory uses Sunquest ICE requesting software which is implemented in all GP practices served by the laboratory. The Microbiology service provides a wide range of microbiological investigations, which is mainly centralized to Charing Cross hub, only urines and H Pylori are performed on site. The total number of investigations last year performed was approximately 490,000 of which 236,000 were general bacteriology 255,000 were serological and immunological investigations. The onsite serology is limited to Hepatitis A, B and C, HIV, Rubella, EBV, VZV, Mumps and Measles screenings. All other virology specimens are sent to the Charing Cross hub or external reference laboratories. Some In house Immunology testing is carried out on the microbiology analyser system. The Microbiology services are managed by NWLP who cover pathology services across 6 sites

BMS senior staff members from the NWLP are responsible for safety in the laboratory and for technical quality assurance. The staff at Hillingdon are supported by the Pathology Service manager and Microbiology senior team based at Charing Cross, which visit the site on a weekly basis.

NWLP Microbiology services have a workforce of 100 across sites providing cover and undertake approximately 1 million tests per year activity.

1.5 PAs DCC for on call commitment. Prospective cover is included for annual and study leave. On call is paid at Category A 8% (Consultant Contract 2003) which reflects the need for immediate response and the potential for attendance on site.

There is currently no predictable on site attendance however this will be expected when THH laboratory moves fully to the hub North West London Pathology Microbiology Laboratory at Charing Cross Hospital, both in-hours and at weekends.

All SPA (CPD, Teaching) and DCC activity must be clearly defined in the Job Plan as per the Consultant Job Planning Policy (Policy number 879, ratified October 2017).

If it is not possible to agree a job plan, either following appointment or at annual review, local procedures will be followed with mediation and formal appeal as detailed in the Consultant Job Planning Policy.

The laboratory services are provided by North West London Pathology and the proportion of microbiology and haematology job plans that support the laboratory services report through Heads of Specialty to the Clinical Director of North West London Pathology. The Consultant Histopathologists are employed by North West London Pathology. North West London Pathology is accountable through the North West Pathology Board to The Hillingdon Hospitals Foundation Trust as an owner of North West London Pathology and to the Trust as a customer through the Trust customer representative, the Divisional Director of Cancer and Clinical Support Services.

North West London Pathology through the lead BMS at Hillingdon is responsible for the technical staff and the technical provision of pathology services including budgetary controls with the support of the Consultant Microbiologists.

The Consultant Microbiologists rotate the responsibilities for Lead Microbiology Clinician, Lead for Antibiotic Stewardship and Infection Prevention and Control Doctor and report to the Divisional Director for Cancer and Clinical Support Services. These responsibilities are subject to annual review.

6. Education, Teaching and Training The Hillingdon Hospital is an Associated with Imperial School of Medicine. It also has an active Postgraduate Education programme. Dr Stella Barnass, Care of the Elderly Physician, is the Director of Medical Education. There is a modern Education Centre with a good library and research links with Brunel University. There are weekly meetings for both primary and secondary care staff.

It is expected that the post holder would be a clinical supervisor and educational supervisor and undertake the appropriate courses and development to fulfil the requirements as stipulated by the London Deanery / lead provider networks depending on the needs of the service.

Teaching and Training Responsibilities The post holder is expected to provide teaching for both undergraduate and post-graduate medical staff and to may be required to become an educational supervisor.

Continuing Professional Development The Trusts encourages all their consultants to take part in continuing professional development in order to ensure that their skills are continually updated in line with relevant medical developments. Compliance with the Trust Statutory and Mandatory Training (STaM) matrix is expected. Consultants may apply to the study leave budget for help with expenses associated with education and development.

7. Research There is a vigorous Research and Development Unit led by Dr Simon Dubrey. The unit provides support and guidance for colleagues from all disciplines. There are good opportunities for liaison with academic centres in NW Thames. The successful applicant will be expected to contribute to system-wide collaborative working for academic as well as clinical aspects of medical microbiology and will be actively encouraged to undertake research.

8. Clinical Governance, Audit and Risk Management Clinical Governance is led by Jackie Walker and Dr Cathy Cale (Nursing and Medical Directors of The Hillingdon Hospital NHS Trust), and is co-ordinated via a Trust Clinical Governance Department. All staff have a responsibility to report all clinical and non-clinical accidents or incidents promptly and to co-operate with any investigations undertaken. The post holder will be required to participate in regular clinical governance meetings, both departmental and Trust wide, and in Grand Rounds. Time and facilities will be made available for clinical governance and audit.

9. Induction The post holder must attend corporate induction and have a local induction undertaken in accordance with relevant Trust policies. In addition, there is specific induction for new consultants with subsequent mentoring. Tailored induction packages are produced for each new consultant according to specialty. There is a Trust programme for structured support for all new consultants within the Trust and access to mentoring. This is a 10 half day mandatory programme commencing every year in April and the cohort consists of all new consultants appointed in the previous 12 months.

10. Revalidation and Appraisal Revalidation is a process by which all licensed doctors have to demonstrate to the General Medical Council (GMC) that they are up to date and fit to practice. The Trust has the required arrangements in place to ensure that all clinicians have a mandatory appraisal with a trained appraiser and supports clinicians / doctor going through the revalidation process. The Responsible Officer for Revalidation is the Medical Director. The Designated Body is The Hillingdon Hospitals NHS Foundation Trust. Time and support will be provided by the Trust to enable revalidation.

11. Raising concerns Staff may on occasion have genuine concerns about healthcare matters and consequently the Trust endorses the principle that these must be raised in a responsible and appropriate manner, and if necessary using the Trusts Raising Concerns at Work (Whistleblowing) Policy. 12

12. Secretarial/Administrative Support The Consultant appointed will have secretarial support, IT access and shared office facilities.

13. Clinical Excellence Awards There is an agreed local procedure for awarding clinical excellence awards.

14. Private Practice There are private outpatient facilities on The Hillingdon Hospital site. Bishopswood Hospital on the Mount Vernon site has inpatient facilities. Private practice on Trust premises falls within the Trust's Clinical Governance arrangements. The successful applicant may undertake private practice in accordance with the Trust’s Private Practice Policy and Schedules 9 & 10 of Terms and Conditions of Service. Information about the Trust

The Hillingdon Hospitals NHS Foundation Trust and Surrounding Area

Hillingdon Hospital is the only acute hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. Hillingdon Hospital is the nearest Hospital to Heathrow Airport and regularly receives emergencies from the airport, including patients with a variety of infectious diseases.

Our services are provided from both Hillingdon Hospital and Mount Vernon Hospital. The Trust has a turnover of around £250 million. We manage 614 inpatient beds and we employ over 3,300 staff. We deliver high quality healthcare to the residents of the London Borough of Hillingdon, and increasingly to those living in the surrounding areas of Ealing, Harrow, Buckinghamshire and Hertfordshire, giving us a total catchment population of over 350,000 people.

Single rooms are available in almost all wards, including one negative pressure room in ICU, two negative pressure rooms in the Acute Medical Unit and one negative pressure cubicle in Peter Pan Ward (Children’s Ward). If further isolation facilities are required patients can be transferred to the Infectious Diseases Unit at Hospital (NW London Hospitals Trust).

The Trust’s services at Mount Vernon Hospital include routine day surgery at a modern treatment centre, a minor injuries unit, and outpatient clinics. The Trust also acts as a landlord to a number of other organisations that provide health services at Mount Vernon, including East & North Hertfordshire NHS Trust’s Cancer Centre.

The North West London Sustainability and Transformation Plan

The STP will transform the way that care is experienced in 2020/21. It is a plan to implement system-wide changes which seek to improve population health & wellbeing, together with care and quality for patients. ‘We will turn a reactive, increasingly acute-based model on its head, to one where patients take more control, supported by an integrated system which proactively manages care with the default position being to provide this care as close to, or in people’s homes, wherever possible’. Hillingdon Health and Care Partners (HHCP)

HHCP represents a partnership of organisations from primary, community, and acute care; as well as the voluntary sector. We collaborate under an Alliance Agreement, to deliver integrated care services. The constituent organisations are:  The Hillingdon Hospitals NHS Foundation Trust (THH)  Central North and West London NHS Foundation Trust (CNWL)  Hillingdon Primary Care Confederation of 44 GP practices  Hillingdon for All (H4ALL) – an umbrella group of five voluntary-sector providers of social care.

HHCP’s data-sharing capabilities represent the best in class among providers in North West London. To facilitate case management, the Hillingdon Care Record connects, via the Medical Interoperability Gateway (MIG) to EMIS and pulls GP summary, medication history and diagnostic history into the application. As a result, relevant information flows with the patient’s movements along pathways. Clinicians and support staff can monitor their progress by having full access to necessary case-notes and outcomes reports.

HHCP are harnessing the capabilities afforded by the ‘Whole Systems Integrated Care Data Warehouse’ by employing a data-driven approach to improving the quality of care for specific population cohorts. This involves identifying people’s needs by applying algorithms to their electronic health records; before supporting them pro-actively with appropriate services so they don’t need to visit hospital.

Terms and Conditions of Employment

ALL APPLICANTS SHOULD ENSURE THEY HAVE READ AND UNDERSTOOD THE FOLLOWING CONDITIONS

The post holder is covered by The Hillingdon Hospitals NHS Trust Terms and Conditions of Service (see below), which have been jointly agreed for the purpose of this appointment. The appointment is superannuable, unless you choose to opt out of the Superannuation Scheme. The post holder is covered by the National Terms and Conditions of Service – Consultants (England) 2003. Any applicant who is unable for personal reasons, to work full-time will be eligible to be considered for the post. If such a person is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues.

Salary - The Hillingdon Hospitals NHS Foundation Trust holds the contract for this post. The job plan will be agreed beforehand in accordance with the requirements of the new consultant contract. Salary: ranges from £77,913 - £105,042 depending on service. In addition, a London Weighting Allowance of £2,162 per annum is payable.

GMC Registration - The successful candidate will be required to have and maintain full and specialist registration (and hold a current licence to practice) with the General Medical Council or be eligible for registration within six months of interview.

Clinical and Professional Responsibility - The Consultant will have a continuing clinical and professional responsibility for the care of patients in their charge and proper functioning of their department. The Consultant will also undertake the administrative duties associated with the care of their patients and the running of the service. It is also the duty of a consultant to:  Keep patients (and/or their carers if appropriate) informed about their condition  Involve patients (and/or carers if appropriate) in decision making about their treatment  Maintain professional standards and obligations as set out from time to time by the General Medical Council (GMC) and comply in particular with the GMC’s guidance on ‘Good Medical Practice’ as amended or substituted from time to time.  Maintain professional standards and obligations as set out from time to time by the General Dental Council (GDC) (Dental consultants only)

Confidentiality - Your attention is drawn to the confidential nature of information collected within the NHS. The unauthorised use of disclosure of patient or other personal information is a dismissible offence and, in the case of computerised information, could result in a prosecution for an offence or action for civil damages under the Data Protection Act 1998. It should be noted that contracts of employment for Consultant staff are the subject of discussion at national level and any changes will be incorporated into your contract.

Job Plan - It has been agreed between senior management within The Hillingdon Hospital and Consultant staff that the annual review of individual consultant's weekly timetable/job description will, in most cases, be carried out by the relevant Divisional Director. Management will meet with each Divisional Director to review the overall service provided by a specialty/department. Programmed activities may be re-allocated from time to time to align with alterations in circumstances such as changes in Consultant staffing, introduction of new services and expansion or reduction of existing services.

A consultant is responsible for carrying out any work related to and reasonably incidental to the duties set out in their Job Plan such as:  The keeping of records and the provision of reports  The proper delegation of tasks  Maintaining skills and knowledge

Consultants shall be expected in the normal run of their duties to deputise for absent consultant or associate specialist colleagues so far as is practicable, even if on occasions this would involve interchange of staff within the same employing organisation. This does not include deputising where an associate specialist colleague is on a rota with doctors in training. When deputising is not practicable, the employing organisation (and not the consultant) shall be responsible for the engagement of a locum tenens, however the consultant shall have the responsibility of bringing the need to the employer’s notice. The employing organisation shall assess the number of Programmes Activities required.

Removal expenses - Reimbursement of removal expenses and associated expenses will be available at the discretion of the Hillingdon Hospitals NHS Foundation Trust as stated in the Trusts Relocation Expenses Policy currently with an upper ceiling equivalent to 10% of an employee's basic starting salary paid up to a maximum of £8,000.

Medical Clearance - Prior to employment we require a satisfactory medical examination report is a condition of employment for medical and dental staff in the NHS. The successful candidate may therefore be required to undergo such an examination unless a satisfactory one has been carried out within the last twelve months.

Arrangements for Annual and Study Leave - The appointee will be expected to make their own arrangements regarding annual leave and study leave cover, liaising with the other Consultants. Annual leave entitlement is thirty-two days per year, increasing with seniority, and study leave entitlement is ten days per year. Study leave may be declined if Core Skills Training (STaM) and annual appraisal are not up to date. Study leave expenses are currently allocated up to a maximum of £1000 per year over a 3 year cycle.

Rehabilitation of Offenders Act 1974 - As the nature of work you will be undertaking during your appointment involves direct contact with people who are receiving a health service, we have been obliged to ask you to complete a disclosure form (at enhanced level), which will be processed by the. Disclosure and Barring Service. We require you to disclose any convictions, cautions, reprimands and warnings you may have under the conditions of the above order. You are not entitled to withhold such information about convictions which otherwise might be ‘spent’. Failure to disclose such convictions could result in the termination of your appointment.

Disclosure and Barring Service - The post is subject to a DBS at enhanced level. The appointed candidate will be required to complete the NHS Pre-, and Post-Appointment Declaration form as outlined in HSC2002/008.

Safeguarding - The Hillingdon Hospital NHS Trust is committed to and has a duty to make arrangements to safeguard and promote the welfare of children, young people and vulnerable adults (Children Act, 1989 and 2004; Safeguarding Vulnerable Groups Act, 2006). Staff groups will have different training needs to fulfil their duties, depending on their degree of contact with children, young people and vulnerable adults and on their level of responsibility. No Smoking Policy - It is the policy of the Trust to promote health. Smoking is therefore prohibited throughout all Trust premises and grounds.

Security - Staff must wear their identity badge at all times to assist in maintaining the security of the hospital; be observant, and not afraid to enquire of people as to their business in the hospital. Any suspicious behaviour must be reported to the Security Officer, manager, or the Support Services Dept.

Health and Safety - The post holder must co-operate with management in discharging responsibilities under the Health and Safety at Work Act 1974 and take reasonable care for his or her own health and safety, and that of others. The post holder must also ensure the agreed safety procedures are carried out to maintain a safe environment for patients, employees and visitors.

Infection Prevention and Control - It is the responsibility of all staff to recognise their role in maintaining a safe environment for patients, visitors and staff to minimise the risk of avoidable Healthcare Associated Infection. Employees are responsible for ensuring that they are fully aware of the Trust’s Infection Prevention and Control policies and the post holder will undertake infection control training on an annual basis.

Hepatitis B - The successful candidate will be required to provide documentary evidence of natural or acquired immunity to Hepatitis B, before taking up employment. Where this is not possible, the post holder will be required to demonstrate by recent (within the last year) evidence of serology showing the absence of Hepatitis B surface antigen. These provisions are to meet the requirement of the Department of Health’s instructions to Trusts (HSG (93)40).

Privacy and Dignity, Respect and Equal Opportunities - The post holder is required at all times to carry out responsibilities with due regard to the Trust’s Equality and Human Rights Policy and to ensure that staff receive equal treatment throughout their employment with the Trust.

Risk Management – All staff have a responsibility to report all clinical and non-clinical accidents or incidents promptly and when requested to, co-operate with any investigation undertaken.

Conflict of Interests - You may not without the consent of the Trust engage in any outside employment and in accordance with the Trust’s Conflict of Interest Policy you must declare to your manager all private interests, which could potentially result in personal gain as a consequence of your employment position in the Trust. Interests that might appear to be in conflict should also be declared.

In addition the NHS Code of Conduct and Standards of Business conduct for NHS Staff (HSG 93/5) required you to declare all situations where you or a close relative or associate has a controlling interest in a business (such as a private company, public organisation or other NHS voluntary organisation), or in any activity which may compete for any NHS contracts to supply goods or services to the Trust. You must therefore register such interests with the Trust, either on appointment or subsequently whenever such interests are gained. You should not engage in such interests without the written consent of the Trust, which will not be unreasonably withheld. It is your responsibility to ensure that you are not placed in a position, which may give rise to a conflict between your private patient’s interest and your NHS duties.

Working Time Regulations - The Trust is committed to the principle that no member of staff should work, on average, more than 48 hours per week.
