UNITED STATES AIR FORCE AIRCRAFT ACCIDENT INVESTIGATION BOARD REPORT F-15C, T/N 80-0041 NELLIS AFB, NV LOCATION: NELLIS AFB, NV DATE OF ACCIDENT: 24 OCTOBER 2011 BOARD PRESIDENT: LT COL DYLAN WELLS Conducted IAW AFI 51-503, Aerospace Accident Investigations EXECUTIVE SUMMARY AIRCRAFT ACCIDENT INVESTIGATION BOARD F-15C, T/N 80-0041 ACCIDENT NELLIS AFB, NEVADA 24 OCTOBER 2011 On 24 October 2011 at 1602 local time, an F-15C aircraft, tail number (T/N) 80-0041, departed controlled flight during a single-ship Advanced Handling Characteristics (AHC) sortie on the Nevada Test and Training Range (NTTR). The mishap aircraft (MA) initiated a left-hand spin at 19,000 feet mean sea level (MSL) after the mishap pilot (MP) attempted a break turn followed by a level heading reversal. The MP attempted to regain control of the MA by following the Spin Recovery Display commands. After multiple revolutions and losing several thousand feet of altitude without any noticeable change in spin characteristics, the MP lowered the landing gear in an attempt to aid MA recovery. At an MP-estimated 8-9,000 feet MSL (terrain elevation is 4,200 feet), the MA recovered from its spin. The MA settled into a 50-70 degrees nose low attitude indicative of an attempt to regain flying airspeed. The MP selected afterburner on both engines attempting to initiate a dive recovery from the MA’s low energy state. As aft control stick was applied and the MA neared the horizon, the MA nose sliced to the left. In this slice, the MP went from a controlled situation to an uncontrolled ejection situation that necessitated immediate ejection. -

Two B-52 crews fly nonstop around the world in 43.5 hours, covering 21,256 statute miles, averaging 488 mph, and carrying out sea surveillance/reconnaissance missions. April 24, 1980. In the middle of an attempt to rescue US citizens held hostage in Iran, mechanical difficulties force several Navy RH-53 helicopter crews to turn back. Later, one of the RH-53s collides with an Air Force HC-130 in a sandstorm at the Desert One refueling site. Eight US servicemen are killed. Desert One May 18-June 5, 1980. Following the eruption of Mount Saint Helens in northwest Washington State, the Aerospace Rescue and Recovery Service, Military Airlift Command, and the 9th Strategic Reconnaissance Wing conduct humanitarian-relief efforts: Helicopter crews lift 61 people to safety, while SR–71 airplanes conduct aerial photographic reconnaissance. May 28, 1980. The Air Force Academy graduates its first female cadets. Ninety-seven women are commissioned as second lieutenants. Lt. Kathleen Conly graduates eighth in her class. Aug. 22, 1980. 547th INTELLIGENCE SQUADRON MISSION 547th Intelligence Squadron is the US Air Force’s “Center of Excellence” for Adversary Tactics Analysis. The 547th: -- Compiles, writes and edits the Air Force Tactics, Techniques and Procedures 3-1, Volume 2, Threat Reference Guide and Countertactics -- Analyzes, refines and disseminates intelligence on adversary tactics and weapons employment for customers throughout the Department of Defense -- Chairs the Special Tactics Analysis Team -- Sponsors and hosts national and adversary tactics conferences -- Provides intelligence support for Air Combat Command’s test and evaluation programs -- Provides intelligence support for 57th Wing operational units and the 99th Air Base Wing -- Provides all-source intelligence support to Air Combat Command’s premier live-fly exercises -- - Red Flag, Spirit Hawk and Air Warrior -- Integrates national intelligence assets into Air Warfare Center missions -- Operates and maintains the USAF’s premier Threat Training Facility -- Operates automated intelligence systems in the Aerospace Integration Center to provide space derived intelligence to the Nellis community -- Operates the USAFWC Special Security Office The squadron accomplishes this mission through five flights: The Plans and Programs Flight is responsible for originating, developing, reviewing/revising squadron requirements, programs, budget and long-range plans. The flight also provides maintenance support, procures threat systems, schedules tours for the ACC Threat Training Facility and is responsible for the formal professional development of intelligence personnel assigned to the 547 IS. The flight manages the On-the-Job training (OJT), formal training course, and the Intelligence staff training program. The flight also ensures squadron members are ready to deploy when tasked. The Intelligence Operations Flight is responsible for the planning, development and execution of RED FLAG-Nellis exercises and provides assistance to RED FLAG- Alaska and MAPLE FLAG exercises. -
Brig. Gen. Robert G. Novotny Serves As the 57Th Wing Commander, Nellis Air Force Base, Nevada
Brig. Gen. Robert G. Novotny serves as the 57th Wing Commander, Nellis Air Force Base, Nevada. He is responsible for 38 squadrons at 13 installations constituting the Air Force's most diverse flying wing. The wing flies and maintains more than 130 aircraft of the following types: A-10, F-15C/D, F-15E, F- 16C/CG/CJ, F-22A, F-35A and HH-60G. The wing also utilizes E-3, RC-135, E-8, B-1, B-2, B-52, C-130, KC- 135, C-17, AC-130U and MC-130P aircraft and MQ-9 remotely piloted aircraft at 13 stateside bases to support the U.S. Air Force Weapons School syllabus. General Novotny is responsible for four groups: 57th Adversary Tactics Group, 57th Operations Group, 57th Maintenance Group and the U.S. Air Force Weapons School. In addition, he oversees the 561st Joint Tactics Squadron; U.S. Air Force Advanced Maintenance and Munitions Operations School; U.S. Air Force Air Demonstration Squadron, the Thunderbirds; and the Red Flag and Green Flag exercises. Brigadier General Novotny was commissioned in 1992 upon graduating from the U.S. Air Force Academy and earned his wings at Laughlin AFB, Texas. He completed five operational F-15 assignments with extensive test and combat experience, in addition to serving as an action officer at a major command, a fighter squadron commander, and a test and evaluation group commander. He is also a graduate of the U.S. Air Force Weapons School, the Naval Command & Staff College, the School of Advanced Air & Space Studies, and the National War College. -

MUNGER Conducted IAW Air Force Instruction 51-503 [Volume One of One] EXECUTIVE SUMMARY UNITED STATES AIR FORCE AIRCRAFT ACCIDENT INVESTIGATION A-10C, T/N 79-0204, AND A-10C, T/N 78-0657 NEVADA TEST AND TRAINING RANGE 65C 6 SEPTEMBER 2017 On 6 September 2017, at 19:44:09 local time (L), two A-10C aircraft, tail number 79-0204 (Mishap Aircraft 1 [MA1]) and tail number 78-0657 (Mishap Aircraft 2 [MA2]), assigned to the 66th Weapons Squadron, 57th Wing, Nellis Air Force Base (AFB), Nevada, collided over Range 65C on the Nevada Test and Training Range (NTTR), 55 miles northwest of Nellis AFB. The midair collision rendered both MA1 and MA2 uncontrollable and both pilots (Mishap Pilot 1 [MP1] and Mishap Pilot 2 [MP2]) ejected. Military search and rescue forces rapidly located MP1 and MP2. MP1 and MP2 suffered only minor injuries during the ejection or parachute landing. Both MA1 and MA2 were destroyed when they crashed on the NTTR. This resulted in the loss of $30,661,412.84 in aircraft and an environmental clean-up cost of $108,000. The accident occurred during a night mission conducting close air support as part of the Weapons Instructor Course for MP1, with MP2 as the instructor of record. -

I know what it is but instead I went to Google for another answer and this is what I found: A coefficient is a number in front of a variable. For example, in the expression x2-10x+25, the coefficient of the x2 is 1 and the coefficient of the x is -10. The third term, 25, is called a constant. If the expression were -x2+10x+25, the coefficient of the x2 would be -1, and the coefficient of the x would be 10. That’s a lot to think about! No wonder we get so many emails and calls from contractors on this topic. The coefficient is very important for any successful contractor or contract. We are excited to team up with the Center for Job Order Excellence (CJE) group to provide helpful resources for our community. Our first subject matter will be responding to a Request for Proposal (RFP) and coefficient development. The coefficient, or in some cases the coefficient(s), that’s right, some contracts have just one, some have two, a dozen and some have hundreds of coefficients. Our focus will be workshops, downloadable white papers, partners, and working examples. We want to do what we can to enable new contractors to have a good understanding of the process. In the last few weeks, there have been at least a dozen contractors who have reached out for help. They are new to the business, and it’s always exciting to hear their passion for this thing we are doing called JOC. -
549Th COMBAT TRAINING SQUADRON
549th COMBAT TRAINING SQUADRON MISSION 549th Combat Training Squadron provides the ultimate peacetime close air support training opportunity for Air Force close air support units and the aerospace power component of the U.S. Army’s brigade combat training at the National Training Center, Fort Irwin, Calif. Fighter units and theater air control system elements from the Air Force, Navy, Marine Corps and occasionally allied nations perform close-air-support missions at the NTC, a training area of almost 1,000 square miles. Similar to Red Flag, the realistic simulated combat scenarios provide valuable CAS training on a scale not available to units at or near their home stations. The friendly and enemy ground forces pit approximately 400 armored and support vehicles and more than 3,000 soldiers against each other in a dynamic and unscripted battle exercise. These fast-paced battles, the intense real-time command, control and communications requirements and visual simulations of armor, anti-armor, artillery and air defense weapons systems combine to present a highly realistic training environment for fighter pilots, forward air controllers and air liaison officers.
