The envelope will show the addresser: 地方公共団体情報システム機構 Chihou Koukyou Dantai Jyouhou Shisutemu Kikou (J-LIS for short). The Notification Card shows your number, address, name, date of birth, and gender. Until the “My Number Individual Number Card” (kojin bangou card) starts being issued in January of 2016 ,(if desired by the owner of the number), this Notification Card can be showed along with your .official identification when submission of your “My Number” is required. The Notification Card alone is not sufficient ID because it does not have a photograph of the bearer. Documents which will be enclosed are: ① Notification Card (tsuuchi card) ② Application form for official “My Number Individual Number Card” (kojin bangou card) to each person ③ Letter of Explanation (one per packet) ④ Envelope for returning the application form to the administrative office Do not lose your notification card. 国勢調査2015 通信 最終号 < Information Update: Census 2015 (Last Update) > The census was completed with your valuable contribution, and we would like to thank those who participated. The information collected is used for creating statistics regarding population which affects matters such drawing electoral zones, determining the number of assigned representatives in the city assembly, or the determining criteria for local allocation tax amount received. Other uses are providing a basis on which to create measures for improving the living conditions for citizens, offering disaster prevention, and for analyzing population for educational uses. A preliminary report will be issued in February regarding the population, and overall results will be shown in June 2016. ジェネリック医薬品利用差額通知書を送付しました Do you know? “GENERIC DRUGS” Notification of Medication Cost Difference with Generic Drugs (ジェネリック医薬品利用差額通知書) were mailed out to applicable residents who are National Health Insurance member and were prescribed brand-name drugs for their medication in the month of July. This notification lists the names and patient’s co-pay amount of purchased medicines, and also informs the cost difference between the generic products and brand-name products for those medicines (cost difference is stated only when it is over ¥100). The cost difference is how much a patient would save by changing the medications to generic drugs. This notification is issued for the purpose of raising public awareness on the generic drugs.
