However, if you can’t do that, hyperbarics is still helpful. With the pressure, you can defuse the plasma with 100 percent oxygen. Normally, the plasma is the acellular portion of the blood. The benefit is the plasma can defuse through the blood vessel into the tissue and bath the cells with oxygen. In addition, the plasma helps attract cells that are good for the healing process and killing bacteria. The greatest attribute is angiogenesis of the blood vessels, which is creating new growth. Now you have new blood vessels and oxygen in areas that were hypoxic. JZ: How important is patient compliance? Dr. M: I tell my patients that I essentially get paid for advice. Imbalances anywhere in the lower kinematic chain can lead to pathology elsewhere such as anterior cruciate ligament tears and sacroiliac joint subluxations. An inability to recognize these direct and indirect consequences is also a failure to provide patients the best care possible. Confronting the cause of distal lower extremity imbalances must also result in the treating physical therapist recognizing their own capabilities. Honest introspection by the physical therapist results in a realization of limitations in available treatments and a need to collaborate in providing the best patient care. Patients are typically grateful to attain a desired outcome regardless of the strategy, and PTs should be open to utilizing a quality pedorthist when indicated to achieve that goal. Identifying abnormalities in the structure of the foot’s longitudinal arch allows for classification of the patient’s foot type. Reliable and valid methods have been identified to determine arch quality in published literature within the scope of physical therapy practice. Biomechanical factors also are considered by the evaluating physical therapist while classifying the patient’s foot type. Mechanical issues are best improved by the physical therapist to the exclusion of other provider choices. Structural abnormalities determined contributory to the patient’s complaint should not be ignored in favor of temporary and often modality-based, symptom-reducing tactics. PFA Update on DME MAC Jurisdiction B Pre-payment Medical Review of TSD Claims PFA recently reported that the National Government Services Jurisdiction B Durable Medical Equipment Medicare Administrative Contractor (DME MAC) Medical Review Department is initiating a pilot prepayment medical review of claims for therapeutic shoes. Per the TSD medical policy, suppliers always have had the responsibility to maintain appropriate documentation as part of their patient’s records to support their claims to Medicare. Suppliers of selected claims will receive a documentation request letter by mail. They then must return the requested documentation no later than 30 days from the date of the letter, or the claim will be denied as not medically necessary. The required documentation is: • A valid physician’s order prior to the date of service on the claim; • A detailed description of the items to be dispensed; • The treating physician’s signature; • If the signature is illegible, a signature key that includes the typed name of the physician with credentials, the physician’s signature and the physician’s initials is required; • The date of the signature; • The start date of the order (only required if the start date is different from the signature date); • Copy of the Statement of Certifying Physician prior to the date of service on the claim; • Documentation from the certifying physician’s medical records prior to the date of service on the claim showing that the patient has one or more of the following conditions: 1. P revious amputation of the other foot or part of either foot; 2. History of previous foot ulceration of either foot; 3. History of pre-ulcerative calluses of either foot; 4. Peripheral neuropathy with evidence of callus formation of either foot; 5. Foot deformity of either foot; or 6. Poor circulation in either foot. • Documentation from the certifying physician’s current medical records prior to the date of service on the claim that validates the patient is under a comprehensive plan of care for his/her diabetes; • Manufacturer and product name/number of the shoes and the inserts that were provided; • For inserts billed with Healthcare Common Procedure Coding System (HCPCS) code A5513 that are not listed on the Durable Medical Equipment Coding System (DMECS) product Classification List, a description of the process and materials that are used to custom fabricate the inserts; and • Valid proof of delivery, including the patient’s name, item delivered and date of delivery.

DMEPOS Requires Detailed Written Orders Medicare requires an order for every item (except repairs) of durable medical equipment, prosthetics, orthotics and supplies (DMEPOS). Detailed written orders are used to confirm what was ordered by the treating physician following the supplier’s receipt of a verbal or written dispensing order. Detailed written orders must include separately billable options, accessories or supplies related to the base item that is ordered. Detailed written orders must not be used to add unrelated items, whether requested by the beneficiary or not, in the absence of a dispensing order from the physician for that item. Example: The treating physician calls the supplier and prescribes a glucose monitor with a verbal order. The supplier can then create a detailed written order that includes an itemized listing of all directly related, separately billable items – the glucose monitor, test strips, lancets, calibration solution, batteries and lancing device. This detailed written order is then returned to the physician for their signature. Although the initial dispensing order from the physician did not specifically include the test strips, lancets and other supplies, they are clearly related to the glucose monitor. Therefore, it is an acceptable detailed written order. For detailed written orders of this type, no further action is required from the physician beyond their signature and date. However, for other types of detailed written orders other actions by the physician may be required. In the example above, it is not acceptable for the supplier to include additional, unprescribed and unrelated items, such as a water circulating heating pad or wrist orthosis to the detailed written order. While the test strips, lancets and other supplies are related to the glucose monitor in the original order, the water circulating heating pad and wrist orthosis are not related and. Therefore. must not be included on the detailed written order. Some suppliers use preprinted forms for their detailed written orders that include a listing of many different items, not all of which may be needed by an individual beneficiary. These listings often create incompatible combinations. For example, an order form for CPAP accessories might list all possible interfaces. On these forms, the final document that is signed and dated by the physician must clearly identify the specific items that are being ordered for that patient. This may be accomplished in one of two ways: • The supplier may indicate the items that are being provided before sending the form to the physician. The physician can then review the form and accept either the items marked by the supplier or make any necessary changes. The physician must then initial and date the revised entries; or • The supplier may send the form to the physician without any items selected and ask the physician to indicate
