Ofh&FW RESOLUTION File No. HFW-Z7038/77 /Z018-NHM SEC-Dept. ofH&FW RESOLUTION OF STATE NUHM REVIEW MEETING State level meetings on NUHM were held with the District level Officials and representative of ULBs on 19.03.2019, 26.03.2019, 04.04.2019 and 05.04.2019 to review the implementation status of NUHMin presence of the following State level Dignitaries and Officials. 1) Mission Director, NHM& Secretary, West Bengal 2) Director of Health Services & Ex-Officio Secretary, West Bengal 3) Addl. Mission Director, NHM,West Bengal 4) Programme Officer-Il, NHM& Deputy Secretary, West Bengal 5) Deputy Director of Health Services, PH&CD, West Bengal 6) Asst. Director of Health Services, Maternal Health, West Bengal 7) State Nodal Officer, NUHM, West Bengal 8) Sr. Accounts Officer, NHM,West Bengal 9) Chief Public Health Officer, SUDA,West Bengal 10)SPMU, NUHM,West Bengal In the first half of the meeting the core activities of NUHM (e.g. infrastructure strengthening, HR recruitment, OPD performance of U-PHCs and outreach activities etc.) was reviewed. Different programme was reviewed in the 2ndhalf by the respective programme officer. Following decisions were taken in the meeting- 1. It was agreed that OPD foot fall of 500 per U-PHC per month is too poor. All effort will be given to increase the OPD load at least to 1000 per month per U-PHC. Average number of lab test is expected to be 50 per day per U-PHC. Districts and ULBs were requested to make necessary arrangement (e.g. filling-up the vacant position of U-PHC, availability of essentials drugs, lab equipments and IEC & BCC etc.) to improve the performance of U-PHCs in terms of OPD, lab. Test etc. 11. Number of laboratory test for the each and every laboratory must be uploaded and reported in HMIS portal. 111. Number of special outreach camps were below the expected number in some of the ULBs. The list of ULBswhere the target of special outreach camp were not achieved is attached as annexure -l. All were requested to conduct the special outreach camp as per plan. v. Districts & ULBs were requested to set a timeline to complete the construction of U• PHCs (new construction & renovation of U-PHCs) and operationalise the same within financial year 2019-20. Otherwise fund release would be a problem for the U-PHCs for late completion. Time line completion and operationalisation needs to be communicated to SPMU(NUHM)within 10th May, 2019. VI. Proposal for relocation site for construction of U-PHC at Uttarpara Kotrung Municipality was not approved by the State. Vll. Rs. 150.73 Lakh was released from the State in favour of concerned DH&FWSs for procurement of lab-equipments for U-PHCs out of total requirement (as per requisition from the district) of Rs. 33l.60 Lakh. Districts were requested to procure the lab• equipments for the U-PHCs. Rest of the fund will be released soon. Vlll. Basirhat Municipality was requested to start the laboratory services at U-PHC. Standalone laboratory is not encouraged under NUHM. IX. It is a matter of concern that few ULBs have not completed the activity under Community Process like Formation, Induction Training and opening of Bank account of MAS. All are requested to complete the activities as mentioned above within June 2019. District wise list of ULBs where activity under community process have not been completed are given in the Annexure-L. x. It was noticed that there is a acute shortage of fund in Siliguri MC. SUDA,West Bengal was requested to release fund to Siliguri MC as early as possible. Xl. In regards to service delivery it was proposed by some of the districts and ULBs that at least one Medical Officer and one Staff Nurse may be provided to each U-PHC from regular health services as the attrition rate of contractual Medical Officers and Staff Nurses are very high. Districts and ULBs were requested to ensure the availability of all the twenty-four (24) essential items for quality ANC services at U-PHCs in pursuance of the order issued under the joint signature of DHS & DME vide Memo No. H/SFWB/IIM-01-2016/Part 1/ 261, dated: 06.03.2019. In this connection, Districts were requested to share the U• PHC wise stocktaking report within 30th April, 2019. Districts were requested to take necessary steps to ensure the availability of all the 24 items as per order within June, 2019. XIV. Districts were requested to furnish the U-PHC wise population as per census 2011. This information is needed to calculate the expected level of achievement (ELA)to monitor the RCH related activities at U-PHCs. Henceforth, the performance of RCH services are being rendered from U-PHCs will be monitored against ELA. xv. Districts and ULBs were also requested to prepare the U-PHC wise ELA as per standard formula shared by the RCH Division. In this regard, handholding support of Sr.PHN & PHN from nearby BPHC/ RH may be arranged by the District for the preparation of ELA. XVI. Facility based reporting in the HMIS portal is 100%. Districts were requested to review the performance of U-PHCs duly uploaded in the HMIS portal on regular basis. XVll. HIV screening of all pregnant mothers will be done during ANCregistration and will be reviewed in regular basis. XVlll. Customised IDSP reporting from the district for urban health facilities must be sent to state in time. Districts were requested to compile the U-PHC wise weekly report and send it to the State IDSP Cell with the copy to State NUHM Cell and SUDA, West Bengal. XIX. ULB were requested further to maintain the line-list of all the positive cases in respect of dengue and enteric fever in Form-L. xx. As a policy decision, it has been decided that every functional U-PHCs will act as Designated Microscopy Center (DMC)under RNTCP. All DTOs may be requested for taking necessary initiatives so that U-PHCs may function as DMC. XXI. Districts were requested to ensure requisite equipments and logistical support of RNTCPservices at U-PHCs through DTOs of the respective Districts. Meetings concluded with thanks from and to the chair and all concerned were requested to take necessary steps as per issues resolved in the meeting. In the first stage the expenditure need and revenue capacities are estimated. In the second stage an econometric analysis is attempted to identify the determinants of fiscal health. The nature of relationship between the determinants and the fiscal health indicator is defined by the relative strength of the ‘revenue 12 13 effect’ and the ‘expenditure effect’. We find that the role of the higher tiers of the government is important in bigger and smaller cities in their financial management. For bigger cities, own revenues can also play an important role in improving fiscal health. In smaller cities the role of the demand indicators is not that prominent but the cost indicators are found to be more effective while in case of bigger agglomerations, the demand indicators can play a role.

Email: [email protected], Mobile-9593566783 ABSTRACT The present study is an attempt to analyze the spatial variation of Tuberculosis (TB) in Malda district, West Bengal, India. TB has become an emerging challenge for the health planners of the district as well as the state administrators. It may be mentioned in this context that in 2016, 5094 TB patients were notified in Malda district which is one of the high TB burden areas in respect of population compared to another district of West Bengal. TB notification data from 2011 to 2016 have been collected from District Tuberculosis Center (DTC), Malda and Ministry of Health and Family Welfare, Government of India. Spatial Autocorrelation method has been used to analyze the spatial pattern detection of Tuberculosis. ArcGIS and SPSS were used for the analysis of the distribution of tuberculosis and clustering. The findings of this study show that Englishbazar Urban, Habibpur, Kaliachak-I, and Old Malda Rural are the most affected areas of Malda district. On the other hand, Manikchak, Harishchandrapur, Baishnabnagar, and Ratua-II have a lower concentration of TB patients.

The study has been conducted based on Secondary data collection and used some appropriate statistical techniques. The result has been showed that there is an educational gap in response to the spatial and temporal context in Maldah district, West Bengal.
