ABSTRACT Title of Document: NARRATIVE and SELFHOOD in the ANTIDEPRESSANT ERA Jeffrey Nicholas Stepnisky, Doctor of Philosophy ABSTRACT Title of Document: NARRATIVE AND SELFHOOD IN THE ANTIDEPRESSANT ERA Jeffrey Nicholas Stepnisky, Doctor of Philosophy, 2006 Directed By: Distinguished University Professor George Ritzer, Sociology This dissertation is a study of the relationship between antidepressant medications, self-understanding, and the narrative construction of self. The analysis relied upon two kinds of empirical data. First, advertisements for antidepressants in popular magazines, television, and online promotional websites were collected. Second, interviews were conducted with 23 people who were taking or had taken antidepressant medications. It is argued that antidepressants are components of the larger social processes of risk, biomedicalization, and individualization. In contrast to a narrative view, which conceives selfhood as a dialogical and embodied achievement, the antidepressants participate in a set of discourses that sustain atomistic conceptions of the self. The analysis emphasizes the personal agency that antidepressant users bring to bear upon their use of antidepressants. Chapter one is an introduction to theories of risk, individualization and narrative as well as the ways in which narrative and selfhood are potentially transformed through the use of antidepressants. Chapter two offers an analysis of three theoretical conceptualizations of the relationship between biomedicine and selfhood: naturalism, poststructuralism and the narrative-hermeneutic perspective adopted in the dissertation. Chapter three analyzes the advertising materials emphasizing the manner in which relationships are constructed between selfhood, biology and antidepressant medications. Chapters four, five and six introduce interview materials in order to examine: a) how people learn to use antidepressants and in doing so come to split-off and manage unwanted elements of their selves, b) the ways in which the popular discourse of authenticity (being a “real” self) is transformed in the encounter with antidepressants, and c) the manner in which the antidepressants are taken up in social institutions such as the family. The dissertation concludes with a reflection upon the implications of a shift from a form of selfhood composed in narrative and relationship, to a form of post-social selfhood composed through the use of technologies such as antidepressants. However, with the demise of the old-fashioned mental hospital, and the rise of Big Pharma (with all its attendant evils), the term “anti-psychiatry” has outlived its usefulness. It survives merely as a term of abuse or a badge of honor, depending on the user and what rhetorical work this label is expected to perform. Those who use the term nowadays generally have a polemical axe to grind, and seldom understand the term’s origins or implications. It is time that serious scholars retire this term, or to restrict its use to R.D.Laing’s followers in the Philadelphia Associates and kindred groups that sprang up in the late 1960s and 1970s. There are substantive critiques of the psychiatric basis for the conceptualization, diagnosis, and treatment that dominate the ADHD context. ADHD discourse and practice are largely influenced by the biomedical framework of mental health and illness. Reed’s concept of politerature as a means to read fiction with a mind towards its utilization in social justice movements for the mentally ill. Through the lens of the Freudian uncanny, Johan Galtung’s three-tiered systems of violence, and Gaston Bachelard’s conception of spatiality, this dissertation examines four novels as case studies for a new way of reading the literature of madness. Shirley Jackson’s The Haunting of Hill House unveils the accusation of female madness that lay at the heart of a woman’s dissatisfaction with domestic space in the 1950s, while Dennis Lehane’s Shutter Island offers a more complicated illustration of both post-traumatic stress syndrome and post-partum depression. Thomas Mann’s The Magic Mountain and Curtis White’s America Magic Mountain challenge our socially- accepted dichotomy of reason and madness whereby their antagonists give up success in favor of isolation and illness. While these texts span chronology and geography, each can be read in a way that allows us to become more empathetic to the mentally ill and reduce stigma in order to effect change.

The Psychiatric Dichotomy and the Proliferation of Models This chapter begins by identifying the fundamental aetiological controversy within the medical model for schizophrenia — the dichotomy between theories that speculate about a biological cause for schizophrenia and theories which focus on an environmental/experiential cause. A description is given of the major biological theories and the drug treatments that support these theories. The range of enviro-experiential theories are then discussed together with the talking therapies they support. Introduction The wide ranging variety of scientific/psychiatric models that are currently used to explain the cause of schizophrenic symptoms1 is indicative of the high level of confusion that persists within the medical model. These proliferating hypotheses can be readily divided into two different types: those which assume a biologically-based aetiology2; and psychodynamic theories rooted in assumptions that the cause is to be found in stresses resulting from the past experience, and/or the past/present environment of the sufferer.3 This dichotomy is an echo of the old nature/nurture debate about human psychological attributes.4 As with the nature/nurture debate, there is also a seemingly balanced and common sense view of schizophrenia aetiology which assumes both sides of the dichotomy will eventually be found to contribute to the answer.5 Associated with this dichotomy over aetiology is a fundamental difference of opinion within the psychiatric profession over the best forms of treatment for schizophrenia.6 Supporters of a biological aetiology normally hold a view that proper treatment requires some form of physical intervention, usually with neuroleptic medication.7 Subscribers to the environmental/experiential theories, on the other hand, usually prefer one of the many forms of talking therapy.8 1 Donald W.

This new wave of resistance has emerged against the backdrop of an increasingly expansive diagnostic/treatment para- digm, and within the context of activist ideologies that can be traced through the veins of broader trends in social movements. In contrast to earlier generations of consumer/survivor/ex-patient activists, many of whom dogmatically challenged the existence of mental illness, the emerging wave of mad activists are demanding a voice in the production of psychiatric knowledge and greater control over the narration of their own identities. After years as a participant-observer at a leading radical mental health advocacy organization, The Icarus Project, I present an ethnography of conflicts at sites including Occupy Wall Street and the DSM-5 protests at the 2012 American Psychiatric Association conference. These studies bring this shift into focus, demonstrate how non-credentialed stakehold- ers continue to be silenced and marginalized, and help us understand the complex ideas these activists are expressing. Documenting and challenging forced psychiatric treatment Report on forced psychiatry and psychiatric abuse against African Americans as intersectional discrimination based on race and disability Summary This report addresses the role of the mental health system as a system for social control that exists in parallel to the criminal justice system, is governed by minimal guarantees of due process, legitimizes discrimination based on disability and gives free rein to racial prejudice. African Americans and other people of color – including children – bear a disproportionate impact of this system in the United States. In particular they experience its most cruel and callous treatment, the disproportionate application of restrictive measures such as inpatient hospitalization, restraint and seclusion (solitary confinement), the combined impact of coercive mental health and criminal justice systems, the application of dehumanizing diagnostic classifications, and the invoking of control systems of any kind for behavior of lower intensity than is the case for most whites. We ask the Committee to condemn the violence inflicted on African Americans by public mental health systems, and by private mental health providers operating with legal authorization, as intersectional discrimination based on race and disability that violates Article 5(b) of the International Convention on the Elimination of All Forms of Racial Discrimination. Forced medication in particular is an act of disability-based violence that can amount to ill-treatment or torture, as is also the suffering inflicted by indefinite detention in the mental health system. Instead of a discriminatory system of social control, we need supports and services that are accountable to communities of color and particularly to people of color with lived experience of trauma, mental health problems and psychiatric oppression.

Selfe Dr. Maurice Stevens Copyright by Elizabeth Marie Brewer 2014 Abstract “Psychiatric Disability and Rhetoricity: Refiguring Rhetoric and Composition Studies in the 21st Century” examines the ways in which mental health activists in the consumer/survivor/ex-patient (c/s/x) movement reframe medical models of mental illness by asserting their lived experience as valuable ontology. I use a mixed qualitative research methodology to analyze discussion board posts and vernacular videos as well as data from interviews I conducted with c/s/x activists. Seeking to correct the absence in rhetoric and composition of first-person perspectives from psychiatric disabled people, I demonstrate how disability studies provides a position from which the rhetorical agency of psychiatrically disabled people can be established. In Chapter 1, “Naming Psychiatric Disability and Moving Beyond the Ethos Problem,” I contextualize the absence of psychiatric disabled perspectives in the history of rhetoric. By demonstrating that the logic of psychiatric-disability-as-an-ethos-problem functions as an enthymeme that warrants re-examination, I denaturalize discourses that assume psychiatrically disabled rhetors have ethos problems.
