Introduction

Research and innovation have been crucial in informing and shaping Ireland’s public health and policy response to COVID-19. The Government’s ‘Resilience and Recovery 2020-2021 Plan for Living with COVID-19’, published on September 15th 2020, recognises the contribution that research has made across a numbers of domains, and seeks to deliver a nationally coordinated research approach to manage and respond to the health, social and economic consequences of the pandemic as well as build resilience for future emergencies. The plan contains a commitment to:

“building further capacity for public health research, research to gauge the social and economic impacts of COVID-19 at a citizen and sectoral level, strengthening epidemiological modelling and behavioural science capabilities, capacity and demand modelling, supporting the clinical trials network, infrastructure to support biorepository studies and data linkages, advances in reliable testing and tracing capabilities”.

At the National Public Health Emergency Team (NPHET) meeting on Thursday, September 3rd, the Chair of the Expert Advisory Group (EAG) presented findings of a report prepared by the research subgroup of the EAG. The report noted the significant contribution of research and innovation to date in responding to the current public health emergency. It also noted that as the pandemic evolves, there is a requirement to consolidate those gains and ensure a national, structured, co-ordinated research response to better inform health, education and economic recovery from COVID-19.

The group recommended that in order to support research into the current, and future pandemics, new research infrastructure is required, with particular focus on (i) expanding the clinical trials network; (ii) creating a national COVID-19 biorepository; and (iii) public health research infrastructure.

The paper was endorsed by the NPHET and was submitted to the Department of An Taoiseach and the Department of Further and Higher Education, Research, Innovation and Science, as a contribution to the cross Government considerations of how Ireland can, through its present infrastructural support systems and funding partners, build on this initial work. Research will be required across a spectrum of domains from basic to translational and clinical research, as well as to address the downstream effects of the disease and its impact on Irish society.

The purpose of the current paper is to provide an update to the NPHET with respect to the various COVID-related research activities and infrastructural initiatives ongoing in the healthcare and broader innovation sphere in Ireland. The paper provides a comprehensive (but not complete) snapshot of COVID-related research activities undertaken in the last year and updates in respect of the planned activities and developments. The paper is based on inputs received with thanks from the Health Research Board, Science Foundation Ireland, Health Products Regulatory Authority, and the National Office for Research Ethics Committees.

Funding for COVID-19-Related Research

In April 2020, the (IRC), the Health Research Board (HRB), Science Foundation Ireland (SFI), Enterprise Ireland (EI), and IDA Ireland developed a coordinated Rapid Response Research and Innovation programme. All five agencies worked together to support COVID-19-related research to inform the public health response to the national public health crisis. Taking this unique approach allowed funding agencies to focus on their areas of expertise and to avoid duplication or overlap. The research priorities of the call were aligned with the World Health Organisation (WHO) R&D Roadmap and informed by the Global Infectious Disease Collaboration for International Disease Preparedness (GloPID-R).

Two funding streams were made available; the first, through the HRB, in cooperation with the IRC, focused on research that addressed: medical countermeasures, health service readiness, and social and policy countermeasures to COVID-19. Details of the awards can be found in Appendix 1. A total of 36 research projects have been supported at a cost of approximately €5.5m over the period 2020-2022.

The second funding stream made available through SFI, EI and the IDA focused on supporting the development of innovative solutions based on new or alternative approaches and methodologies to address the pandemic response. SFI (in two phases) provided funding of €18m for a total of 83 projects (Appendix 1).

In July, the HRB, jointly with the Health Service Executive (HSE), provided €0.5m funding for four projects (Appendix 1) through the Research Collaborative in Quality and Patient Safety (RCQPS).

The European Commission issued two EU COVID-19 funding calls in 2020. The initial call in January allocated €38.5 million to 18 projects involving 140 research teams from across the EU and beyond to develop research on understanding the behaviour of the epidemic through epidemiology and modelling, developing rapid diagnostics, as well as treatments and vaccines. An Irish SME, working with partners from Queens University Belfast, China and Italy, is leading one of these projects, HG nCoV19 test, to develop a rapid molecular diagnostic test. The Health Protection Surveillance Centre (HPSC) is a partner in the I-MOVE-COVID-19 multidisciplinary European network for research, prevention and control of the COVID-19 pandemic led by France and including 20 EU countries.

A second call in May 2020 allocated €133m to 24 successful research projects involving over 350 research teams from 40 countries. The funding is supporting researchers to address the pandemic and its consequences by strengthening the industrial capacity to manufacture and deploy readily available solutions, develop medical technologies and digital tools, improve understanding of behavioural and socio-economic impacts of the pandemic, and to learn from large groups of patients (cohorts) across Europe. Ireland performed well and is involved in seven successful projects and leading two projects. Irish success includes a mix of SMEs and universities (TCD, UCD, TUD and UCC). Details of successful COVID applications with Irish involvement can be found in Appendix 1.

In September 2020, the EU launched EU-RESPONSE – a five-year multinational project (€15.7M) in order to design and run a new adaptive European platform trial on COVID-19 and emerging infectious diseases. The project seeks to facilitate the European expansion of the DisCoVeRy study, a phase III, open-label, adaptive, randomised, controlled, multi-centre clinical trial designed to evaluate the safety and efficacy of medicinal products in hospitalised adult patients diagnosed with COVID-19. It also seeks to build a new multinational European Adaptive Platform Trial, based on existing initiatives, experiences and competencies – the EU- SolidAct. The project also includes a coordination module with the EU-funded RECOVER ensuring complementarity and cooperation across all large European COVID-19 Adaptive Platform trials, and their capacity to answer the needs of society through dialogue with the EMA, national competent authorities, HTAs and industry partners. Ireland is involved in application discussions, with UCC as a listed partner. The HRB is engaging with partners in Ireland, including CRCI, CRFs and investigators to monitor developments.

Clinical Trials

The Health Products Regulator Authority (HPRA) authorised eight clinical trials related to COVID-19 in 2020 (Appendix 1). The sponsor for all but one of the trials authorised to date is academic. Most trials were hospital-based, but one trial of a single IV dose of a monoclonal antibody (CT-P59) is authorised for conduct in outpatients. Not all authorised trials have commenced in Ireland and there is ongoing review of a trial of direct-acting oral antiviral in healthy, non-hospitalised patients, and a trial investigating therapeutic anticoagulation in hospitalised patients. The HPRA offers free scientific and/or regulatory advice to sponsors interested in conducting COVID-19 CTIMP research in Ireland. Approximately 20 expressions of interest have been received and teleconferences were conducted with potential sponsors. The HPRA experience is that pre-submission advice is particularly helpful for academic sponsors.

REMAP-CAP is an existing EU randomised, embedded, multifactorial, adaptive platform trial for community-acquired pneumonia. REMAP-COVID is a sub-platform of REMAP-CAP that evaluates treatments specific to COVID-19. In Ireland, the trial is being funded by the HRB and led by Alistair Nichol, a Consultant in Intensive Care Medicine at St Vincent’s University Hospital (SVUH) and lead of the HRB’s Clinical Trial Network in Critical Care. The trial is taking place in ICUs in SVUH, University Hospital Galway (UHG), Beaumont Hospital, Cork University Hospital (CUH), St James’s Hospital (SJH), Waterford and University Hospital Limerick (as well as the Royal Victoria Hospital in Belfast). A total of 77 REMAP-CAP patients have been randomised to date in Ireland. As part of the trial, the Irish arm published results showing that the common steroid hydrocortisone, administered to critically unwell patients in ICUs, reduced the mortality rate from 40% to 32% (similar to the results with dexamethasone in the UK RECOVERY Trial). A Ventilation Domain is almost ready to be implemented in selected sites in Ireland. The statins domain is under review with the HPRA and will be implemented in SVUH initially, once approved. Amendments to the research ethics committee and the HPRA are being drafted to include moderate illness state protocols for ICU and wards and also an antiplatelet domain.

WHO Solidarity is an international clinical trial to help find an effective treatment for COVID- 19, launched by the WHO and partners. The Solidarity Trial involves 30 countries and 500 participating hospitals (and an additional 15 countries are now joining the trial). The trial is recruiting about 2,000 patients per month and more than 13,000 patients have been recruited in this trial to date. The Department of Health is acting as trial sponsor on behalf of the Irish Government and has provided funding (up to €2.5 million) to enable Ireland to participate in the Solidarity Trial. The Trial is active in seven sites in Ireland and has recruited 111 patients to date. In recent months, Remdesivir supplies from Gilead have been a constraint on recruitment into the Irish study and this has essentially stalled randomisation in Ireland. The Solidarity Trial may expand to investigate newer antiviral drugs, immunomodulators, and anti-SARS-CoV-2 monoclonal antibodies. The Irish Oversight Committee agreed at their recent meeting to conduct a detailed review of current large-scale clinical trials underway or planned internationally to see how and where Ireland might best optimise the trials infrastructure and capacity the HRB and the Department of Health have put in place in hospitals around the country. Due diligence is being conducted by the HRB and others into trials in the UK and in Europe to assess study protocols and the requirements (sponsorship, indemnity etc) and resources needed to participate.

National/International Observational Studies

National Short PeRiod IncideNce sTudy of Severe Acute Respiratory Infection (SPRINT-SARI) is an international, multi-centre, prospective, short period incidence observational study of patients in participating hospitals and intensive care units (ICUs) with severe acute respiratory infection (SARI). The study is supported by ISARIC (International Severe Acute Respiratory and emerging Infection Consortium) and Ireland’s participation in the study is led by Professor Alistair Nichol, supported by the HRB through the Clinical Trial Network in Critical Care and the REMAP-CAP trial. Extensive global participation has resulted in a large standardised collection of comprehensive clinical data from hundreds of sites across dozens of countries. Data have been entered for 122,361 individuals from 578 sites across 42 countries and the data are analysed regularly and reported publicly to inform patient care and public health response. Seventeen Irish sites have contributed data on 987 patients, 688 of which had confirmed COVID-19 infection, and data for 316 ICU patients, 201 of which had confirmed COVID-19 infection. Addition of new modules across Irish sites now facilitates investigation of neurology, coagulation, critical care, cardiac and kidney research. The Ireland team will continue to produce SPRINT-SARI Ireland reports regularly (fortnightly) and make these available on the website. They are also preparing a manuscript describing the first Irish COVID- 19 wave.

The Irish Longitudinal Study on Ageing (TILDA) is a large-scale longitudinal study that collects information on the health, economic and social situation of a nationally representative sample of 8,504 older community-dwelling adults in Ireland. Since March 2020, TILDA has engaged with officials in the Department of Health and produced numerous research publications to inform the public health response to the pandemic, covering a range of topics as they relate to COVID-19 such as healthcare utilisation, physical frailty, loneliness and isolation, vitamin D deficiency and preparing for vaccination. All of these publications can be found at https://tilda.tcd.ie/CovidData/reports/. TILDA has also documented the impact the pandemic has had on the lives of older adults via a survey of 4,000 of its existing participants between July and November 2020. The findings are described in the recently published ‘Altered lives in a time of crisis: The Impact of the COVID-19 Pandemic on the Lives of Older Adults in Ireland’ (January 2021) at the following links (https://tilda.tcd.ie/publications/reports/pdf/c19-key-findings-report/Key%20Findings.pdf).

A second TILDA COVID-19 project intends to gather biological samples to establish who has been infected by the virus and determine the risk factors and consequences for developing COVID-19 in older adults in Ireland.

IDS-TILDA is a longitudinal study researching ageing in Ireland among people with an intellectual disability aged 40 and over. A supplemental COVID-19 survey was developed and administered to 710 IDS-TILDA participants between May and September 2020 in response to the COVID-19 crisis in Ireland and the results were launched (https://idstilda.tcd.ie/wave4/launch.php) on December 3rd 2020. IDS-TILDA has been granted additional funding of €60,000 from the Department of Health to carry out a follow- up survey to explore the ongoing impact of COVID-19 on older people with intellectual disabilities. This next wave of interviews with IDS-TILDA participants is due to commence in Spring 2021.

In January 2021, the HSE published results of the PRECISE study on COVID-19 antibodies in healthcare workers at Irish hospitals. Staff in SJH in and UHG were offered testing for antibodies to COVID-19 over a 10-day period in October 2020. The overall seroprevalence of antibodies to SARS-CoV-2 was 15% in SJH and 4.1% in UHG, reflecting the difference in community transmission and diagnosed disease incidence in each geographical area and each hospital during the first wave of the pandemic. The HCW seroprevalence was six times the community seroprevalence in each geo-area. Specific risk factors for antibody positivity included being a healthcare assistant or nurse, daily contact with patients (especially those known or suspected to have COVID-19 infection), age 18-29, living with others, in particular living with other HCWs, being of Asian background, and being male. Follow-up studies are planned for Spring 2021.

In August 2020, the HSE published results of the Study to Investigate COVID-19 Infection in People Living in Ireland (SCOPI): A national seroprevalence study. A representative sample of 1,733 people aged 12 to 69 years in Sligo and Dublin participated in the study carried out by the HPSC and the National Virus Reference Laboratory (NVRL) in June/July of 2020. The study, the first of its type in Ireland, measured antibodies to SARS-CoV-2 virus which are an indication of past infection with COVID-19. The study reported a prevalence of infection of 0.6% in Sligo and 3.1% in Dublin. Based on these results, the HPSC estimates a national prevalence rate of 1.7%. Using the prevalence data from this study, the HPSC estimates that that 59,500 people in Ireland in the age group 12 to 69 years had been infected with SARS- CoV-2 up to mid-July; three times more than that detected via surveillance of notified cases. No statistical differences were identified in the prevalence by age group, or between males and females.

EU Vaccine Trials Network

There are plans to develop an EU-wide network for COVID-19 vaccine trials to be supported with €10m Horizon 2020 funds. This EU-wide network will facilitate largescale trials in Europe for large industry, SME and academic developers, and help to ensure that public health questions are prioritised. The network will enable a collaborative approach for targeted studies of direct public health interest and is seen as an important investment to strengthen EU capacity for better preparedness for future epidemics or pandemics. The HRB is leading discussions in Ireland on assessing the opportunity, benefits and appropriate routes for Irish participation. The HRB has liaised with ECRIN and HRB-CRCI to ensure that Irish clinical trials sites are represented in the preparations and planning for this initiative in Europe.

COVID-19 Biobank

In October 2020, the Department of Health requested that the HRB convene relevant stakeholders to progress and review a detailed application to establish a National COVID-19 biobank. The biobank is intended as a national resource leveraging existing health research infrastructure and building on current COVID-19 biobanking activity to the greatest extent possible. Governance arrangements will ensure harmonised systems, processes and practices across the participating centres, and access policies will be clear, transparent and support access for high-quality research, including a focus on questions relevant for clinical care and public health policy. The HRB developed principles for a call for a single coordinated application from a suitable national team. An application was submitted to the HRB at the start of February and the proposal underwent review by a panel of international experts in biobanking on February 9th. Recommendations and summary report from the panel will be submitted to the Department of Health in the coming weeks.

COVID-19 National Research Ethics Committee

In accordance with a recommendation in the WHO Roadmap for R&D, in April 2020 the Minister for Health established a temporary National Research Ethics Committee (NREC) for COVID-19 to deliver an expedited process for robust ethical review for all COVID-19-related research. During its tenure (April to August 2020), the NREC COVID-19 met on average on a weekly basis and returned decisions on a total of 93 research applications from 61 institutions in 20 counties. The vast majority of applicants received an opinion within one to two weeks of submission of their proposal. The COVID-19 NREC, chaired by Professor Mary Horgan, reviewed COVID-19-related studies across the full spectrum of human health research, including basic research, translational research, clinical research, diagnostics and treatment, population health research, social research, health services research and applied health policy research.

Of note, the NREC process involved structured coordination with others involved in health research regulation, namely the HPRA and the Health Research Consent Declaration Committee (HRCDC) in order to facilitate all necessary decisions in a common, expedited timeline. Nine ethics applications submitted for expedited review also required a formal consideration for a consent declaration involving close coordination with colleagues in the HRCDC Secretariat. The Committee was hosted and ably supported by the newly established National Research Ethics Office. The COVID-19 NREC is proof of principle of the value of a ‘single national ethics opinion’ for Irish health research, a longstanding ambition of the Department of Health, provided for in the National Research Ethics Committee Bill 2019, and a requirement under the Clinical Trials Regulation. A standing subcommittee of the NREC COVID-19 now sits periodically to meet the ethics oversight responsibilities, including review of amendments, associated with the research approved by the Committee.

Optimal and Secure Use of COVID-19 Data for Research Purposes

As part of the COVID-19 response, an agreement was reached under Section 11 of the Statistics Act, 1993 between the Central Statistics Office (CSO), the Department of Health, the HSE and others to support the collection, collation and statistical analysis of COVID-19-related data, whilst protecting the privacy rights of individuals. In order to facilitate access to this data for the broader research community in a safe, responsible and compliant manner, the HRB, the Department of Health and the CSO are at an advanced stage of putting a mechanism in place for Approved Researchers in support of the public health and national policy response to COVID-19. A key safeguard, a Research Data Governance Board (RDGB), has been jointly appointed by the HRB and the CSO in close cooperation with the Department of Health. This independent body will provide a central governance focus integrating all the necessary protocols and safeguards to ensure compliance and best practice under the Statistics Act, 1993 and the Data Protection Act 2018/Health Research Regulations 2018 within a single, collaborative, transparent process. This initiative is expected to be announced by the Minister in the early part of February 2021.

The HRB, the Department of Health and the HSE have partnered in a new EU Coordination Support Action, Population Health Information Research Infrastructure (PHIRI) for COVID. PHIRI aims to facilitate and support open, interconnected and data-driven research through the sharing of cross-country COVID-19 population health information and the exchange of best practices related to data collection, curation, processing, use and reuse following FAIR principles. It will develop a Health information portal on real world population health and healthcare data in collaboration with national institutions, cross-border organisations and research networks. This EU initiative, led by Sciensano Belgium and involving 30 countries, was granted €5million in emergency funding under the ‘mobilisation of infrastructures for the COVID-19 Public Health Emergency’ call. Irish participation will ensure access to the strong knowledge translation and dissemination component, and to dedicated training in areas such as foresight modelling. This three-year initiative started in November 2020.

Building Capacity in Public Health Modelling

The HRB and the Department of Health are in discussions to assess how best to “strengthen epidemiological modelling” and to build “further capacity for public health research”, two specific aims contained in the Recovery and Resilience Plan 2020-2021. This also fits with a wider goal to strengthen the research infrastructure for health protection modelling more generally.
