7 June 2021 Details of Compulsory Testing Notice (Persons who have completed a COVID-19 vaccination course1 would be taken to have complied with the requirements set out in the compulsory testing notices)

1. Any person who had been present at Nina Mall 1, 8 Yeung Uk Road, Tsuen Wan, , for more than one hour at any time during the period from 12:00 noon to 6:00 p.m. on May 29, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

2. Any person who had been present at Nina Mall 2, 98 Tai Ho Road, Tsuen Wan, New Territories, Hong Kong for more than one hour at any time during the period from 12:00 noon to 6:00 p.m. on May 29, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

3. Any person who had been present at Volcano KitchenOne, G/F, 75 Tai Pa Street, Tsuen Wan, New Territories, Hong Kong at any time during the period from 1:30 p.m. to 3:30 p.m. on May 29, 2021 (including but not limited to full- time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

4. Any person who had been present at The Mills, 45 Pak Tin Par Street, Tsuen Wan, New Territories, Hong Kong for more than one hour at any time during

For both the Sinovac COVID-19 vaccine (CoronaVac vaccine) and Fosun Pharma/BioNTech COVID-19 vaccine (Comirnaty vaccine), completion of the vaccination course in general means having received two doses of COVID-19 vaccine 14 days ago, i.e. having received the second dose of COVID-19 vaccine on or before May 26, 2021. For persons previously infected with COVID-19, they may be deemed to have completed the COVID-19 vaccination course after receiving one dose of Comirnaty vaccine or one dose of CoronaVac vaccine 14 days ago, i.e. having received one dose of COVID-19 vaccine on or before May 26, 2021. For persons who received COVID-19 vaccines in places outside of Hong Kong and have received the recommended dose(s) as stipulated in relevant guidelines 14 days ago, i.e. having received all of the recommended dose(s) of COVID-19 vaccine on or before May 26, 2021, they will also be regarded to have completed the vaccination course of the relevant COVID-19 vaccine, subject to the vaccine used being included on the list of vaccines recognised for this purpose as published on the Government’s COVID-19 thematic website (www.coronavirus.gov.hk/pdf/list_of_recognised_covid19_vaccines.pdf). the period from 12:00 noon to 6:00 p.m. on May 29, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

5. Any person who had been present at Discovery Park (only the shopping mall is included), 398 Castle Peak Road Tsuen Wan, Tsuen Wan, New Territories, Hong Kong for more than one hour at any time during the period from 3:00 p.m. to 9:00 p.m. on May 29, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

6. Any person who had been present at Namco, Shops UG26-28 & UG37-38, UG/F, Citywalk 2, 18 Yeung Uk Road, Tsuen Wan, New Territories, Hong Kong at any time during the period from 12:00 noon to 6:00 p.m. on May 29, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

Any person who had been present at Chán•Taiwan Cuisine, Shop 122, 1/F, Nina Mall 2, 98 Tai Ho Road, Tsuen Wan, New Territories, Hong Kong at any time during the period from 8:00 p.m. to 10:00 p.m. on May 29, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

8. Any person who had been present at Cats Tea Room, Room 3, 3/F, Hai Phong Mansion, 52 Lock Road/53-55 Haiphong Road/99-101 , , , Hong Kong at any time during the period from 1:30 p.m. to 3:30 p.m. on May 23, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

9. Any person who had been present at the eslite spectrum Tsim Sha Tsui Store, 2/F & 3/F, Star House, 3 Salisbury Road, Tsim Sha Tsui, Kowloon, Hong Kong at any time during the period from 3:00 p.m. to 4:00 p.m. on May 23, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

10. Any person who had been present at K11 Musea, , 18 Salisbury Road, Tsim Sha Tsui, Kowloon, Hong Kong for more than one hour at any time during the period from 12:00 noon to 6:00 p.m. on May 23, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

11. Any person who had been present at , 37K Yen Chow Street, , Kowloon, Hong Kong for more than one hour at any time during the period from 3:00 p.m. to 9:00 p.m. on May 23, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.

12. Any person who had been present at Kakurega Ramen Factory, Room 7083, 7/F, Dragon Centre, 37K Yen Chow Street, Sham Shui Po, Kowloon, Hong Kong at any time during the period from 5:00 p.m. to 8:00 p.m. on May 23, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by June 9, 2021. If persons subject to compulsory testing have previously undergone testing between June 4 and June 6, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice.
