Introduction of Scaffolding Rental and Supply for Year 2020/2021 at Mukah Power Generation Sdn Bhd (MPG)

The Services to be provided by the Service Provider include the following:

1. This tender specification is intended to describe and quantify, as far as possible, the works which are required to be done by the contractor who has intention to do Scaffolding Rental and Supply for Year 2020/2021 at Mukah Power Generation Sdn Bhd.

2. Each Tenderer will be required to submit its Tender Offer in the Tender price and to provide with all information which is either requested or is otherwise required to be provided in accordance with the Tender Documents. Tenderers shall note that MPG may, in its sole discretion, elect to:

A. appoint more than one tenderer to perform the works; B. not appoint any tenderer to perform the works, or C. cancel this invitation to tender process at any time.

3. MPG consists of Unit 1 & Unit 2, with each Unit can deliver 135MW. Scaffolding rental and supply will cover all area within Mukah Power Generation Sdn Bhd as per listed work scopes.

SCOPE OF SERVICES / SPECIFICATIONS

The work scope shall include, but not limited, to the following:

1. Scaffolding Requirements

All scaffolding shall be constructed in accordance with BS 5973 Code of Practice for Access and Working Scaffolds and Special Scaffold structures in steel or its equivalent.

2. Scaffolding Materials & Component

a) Scaffold tubes: i. All tubes must be galvanized steel 48.3mm OD and wall thickness 4mm comply to BS 1139 Part 1, 1982 and free from crack, splits or excessive corrosion and straight.

b) Coupling and Fittings i. They must be of sound construction, comply with BS 1139 Part 2. Do not mix tubular and fittings of different standards. ii. 3 basic most comment couplers types: a. Right angle couplers b. Putlog couplers c. Swivel couples iii. Right angle and swivel couplers can be used to fix tube in a load bearing connection iv. To joint tubes end to end joint pins (also called spigots) or sleeve coupler are used.

c) Board and Decking i. All timber scaffold boards shall meet the recommended BS 2842:1981 or regulation 87 of factory and machinery act 1967, Regulation 1986. ii. Ended are protected by metal plates called hoop irons. iii. Comes in 3 thickness 38mm (usual), 50mm & 63mm are standard width (225mm)

d) Ladders i. Shall meets BS 1129:1982

e) Frame or tubular scaffolds

i. Only propriety steel frame or tubular scaffold of sound design and construction are allowed as per manufacturer specification

f) Inspection i. A qualified scaffold supervisor should inspect all material and inspect the relevant erected scaffolding with the report signed and forwarded to the safety officer for record keeping. ii. Any scaffolding that has passed inspection shall be tagged to indicate that it is safe for usage. iii. A qualified scaffold supervisor shall be at site full time to supervise any scaffold works. iv. A qualified scaffold supervisor shall apply permit to remove any scaffolding material and shall tagged relevant scaffolding that removal work in progress. v. Any scaffolding above 40 meters height must be designed and endorsed by professional structural engineer and forwarded to DOSH.

g) Erection i. Scaffold must only be erected, altered or dismantle by competent scaffolder under supervision of charge hand scaffolder. All inspection of any scaffolding shall be done by a scaffold inspector. ii. The maximum duration for scaffolding volume installs at one area is one month (30 days). iii. For hanging scaffolding, the volume calculated based on how much volume of scaffold erected.

h) Scaffolder: i. The contractor must provide 1 Certified License Scaffolding installer to each team that consists of 4 members. Minimum four (4) teams must be provided by the contractor to Mukah Power Genaration Sdn Bhd. ii. All the resume and certificate for the team leader that have Certified License for Scaffolding Installer from DOSH must be attached inside the tender. Mukah Power Generation Sdn Bhd will conduct an interview to each of them. iii. List of staff as a scaffolding erector must be attached also in the tender.

i) Scaffolding Inspector: i. Contractor must provide one (1) scaffolding supervisor to do inspection for the scaffolding that erect by the scaffolding erector. ii. Scaffolding supervisor must prepare monthly checklist and finding report to Mukah Power Generation Engineer in charge. iii. Resume and valid certificate must be attached inside the tender.

j) Material: i. Contractor must provide material lists that will be used for scaffolding work in Mukah Power Generation Sdn Bhd. The material list must consist of material data sheet and quantity of every part or material that will be used for this contract.

k) Company: i. Company must be registered with CIDB (Construction Industry Development Board) registered as Scaffolding Installer. ii. Please provide the certified true copy of the company certificate. iii. Provide the Company experience in Scaffolding sector together with this tender. iv. UPKJ – At least Class C or above, Register under Services & Supplies with Head I – Civil Engineering & Building Materials [Sub-Head 2(a)] v. Local, Restricted (Bumiputra)

l) On-call: i. Contractor must be available to be call 24 hours and 7 days for any work related to scaffolding. ii. Upon calling, the scaffold must reach THE WORK SITE within 1 hour interval. Fail to do so may cause the contract to be terminated. iii. Upon request or work completion, scaffold must be dismantled within 1 day.

m) PPE (Personal Protective Equipment) and UA/UC (Unsafe Act/ Unsafe condition) i. All the scaffolder must be equipped with full PPE and MPG Sdn Bhd will not liable to any incident happen due to lack of PPE. ii. If any of the scaffolder received UA/UC, the contractor will be penalized, and it will be taking care by MPG Sdn Bhd safety section.

n) Daily Audit: i. Contractor must conduct daily audit on the scaffolding erected and the PPE use by the scaffolder. ii. The audit must be submitted to SCAFFOLDING person in charge.

o) AESP (Authorized Entrance and Standby Person) i. Contractor need to make sure any of the scaffolder that executes the erection inside confine space has the AESP card from NIOSH. p) WAH (Working at Height) i. Contractor need to make sure any of the scaffolder that executes the scaffolding erection involve working at height as per FMA 1967 regulation has valid WAH card from NIOSH.

q) Probation Period i. Successful bidder shall be bound by Probation Period of 6 months. Performance evaluation will be done by MPG and decision will be made either to continue or to terminate the contract.

Scaffolding material and erection should follow the Regulation 72 on FACTORIES AND MACHINERY (BUILDING OPERATIONS AND WORKS OF ENGINEERING CONSTRUCTION) (SAFETY) REGULATION.

3. Work commencement and duration

a. The contract period is for one (1) year from the date of award and shall be extended on yearly basis for maximum of two (2) years subject to satisfactory performance.

4. Services Warranty

a. The Contractor shall ensure that the work performed is according to the specification and acceptable standards.

5. Penalty Clause

a. Delay of scaffolding erection and didn’t meet the scaffolding quality criteria: Contractor Performance will be reviewed and affected.

7. Contractor's Obligation:

a. Compulsory to attend Tender Briefing and Site Visit (Virtual if necessary due to Covid-19 pandemic).

b. To prepare full set of Method Statement which inclusive of safe work procedure (SWP) / job safety analysis (JSA), work procedure, methodology and risk assessment etc. for the Scaffolding Rental & Supply at Mukah Power Generation Sdn Bhd and to obtain "Permit to Work" from MPG before any work allow to commence. All Method Statement must be submitted to MPG Engineer for review two (2) week before work started. The SWP shall be reviewed by MPG safety section before proceeding to work.

c. The tenderers are obliged to have site visit assessment of the work in MPG before submitting the tender and to fill in the ‘Site Visit Acknowledgement Form’ section along with MPG Person in Charge Signature. (Virtual if necessary due to Covid-19 pandemic)

d. The contractor shall attend MPG safety induction course, life-saving rules (LSR), confine space and working at height briefing by MPG safety section. If contractor found not follow the SEB safety regulation and policy, the demerit point will be imposed with refer demerit point system

e. Contractor with valid Sarawak Energy Safety Passport (SESP) will have an advantage.

f. The resume of the supervisor or engineer, scaffolder, inspector shall be submitted during the tender submission. The tenderer will be rule out if not strictly follow the tender specification.

g. The contractor is obliged to perform day to day housekeeping at the work area and necessary upon request by Employer. All schedule waste shall be disposed at the designated area and it must be proper seal and label

h. To provide the organisation chart.

i. All personnel shall acquire Sarawak Work Permit for Non-Sarawakian before entering MPG for business and work purpose.

j. Contractor need to provide themselves the cover/shelf for the material or vehicles at designated place inside MPG to ensure the safety of the materials and parts (with proper signage).

k. Accommodation, transportation and site office by own contractor.

l. Consumables and tools and machineries by contractor including Personnel Protective Equipment (PPE).

m. Any change to the MPG structure or equipment need to be subject by MPG management agreement

n. Machineries parking area subject to contractor proposal upon site visit and MPG management agreement.

o. MPG have the right to stop the work if the construction work affects the plant performance and not safe to equipment/people.

8. HSSE Requirements

a. The Contractor shall comply with the regulations, procedures and guidelines with respect to work according to ‘Safety Guidelines for Contractors and Contractors’ Workers Working in Power Station Premises’ at all time.

b. The Safety officer or his representative has the right to inspect and/or stop the progress of work if deemed necessary due to violation of safety procedures and operation.

c. The Contractor shall be responsible for the safety of his workforce and the safe keeping of the materials, tools and equipment used for this entire work.

d. The Contractor shall be responsible in providing safety and personal protective equipment to his workforce necessary for the entire work. Such equipment includes safety harness, safety helmet, safety shoe, eye protector, etc.

e. The Contractor shall be responsible in ensuring that all his workforce utilise proper safety equipment and adhering to Station Safety Regulations throughout the entire work.

f. The Station disclaims any liabilities involving injury and / or death to any Contractor’s workforce while carrying out work at the Station.

g. All entry into the Station area must be vetted by Station Security Staff.

h. Vehicles entering and leaving work site are subjected to Security checks.

i. All tools and equipment transported into the Station area must be declared by submitting tools list to Security Officer. Items not listed may be detained when leaving the work site.

j. All Contractor’s vehicles must be parked at designated area.

k. Contractor’s workers are not allowed to enter areas beyond their work site they are involved in.

l. For Station materials that required to be taken out of the Station premises, proper main gate pass and authorization from Maintenance Engineer or Maintenance manager are compulsory.

9. Work reporting:

a. Contractor shall submit Monthly Report, which shall include the following information:

i. Scaffolding volume for each section (Boiler, Turbine, Electrical, C&I, Facility and Operation)

10. Supervision

a. The Contractor and its work teams will be supervised by experienced MPG staff / a group of specialists who are employed by MPG under separate contract to give advice, instructions regarding the work procedures, work method and passing of judgment during the crucial stage of the operations. The contractor’s personnel, work team leader etc, shall co-operate fully with the specialists to ensure smooth implementation of the major inspection.

Nevertheless, the contractor is free to contact MPG to propose any suggestion for improvement and / or to exchange views on issues related to work.

11. Group Leader

a. MPG will provide an experience staffs to lead the contractor’s working group on some of the major and critical equipment and given step to step instruction regarding the work procedures. However, MPG ‘s involvement shall not relieve the contractor from his responsibility.

12. Inspection

a. MPG authorized engineer and technician’s personnel shall be involved in all the inspection works. MPG's involvement shall not relieve the Contractor from his responsibility

13. Contractor's Workforce

a. The Contractor shall provide its own work teams. The total number of workers needed to complete the work shall be determined by the Contractor pending on the capability of the workforce to complete the request within the stipulated time frame

14. Working Hours

a. Once the maintenance work commences, the contractor is required to work every day including Saturday, Sunday and public holidays. Special rest day should be approved by MPG base on the work progress and availability of additional time.

b. The working time for the Contractor shall be as follows:

Night shift: 8pm until 8am

Maintenance:

Friday Monday - Thursday From 8:00 am to 11:45am From 8:00 am to 1:00 noon and and

From 2:00 pm to 17:00 pm From 2:15 pm to 17:00 pm

c. If the jobs required 24 hours, contractor is requested to inform the company for supervision arrangement.

15. Accommodation, Meals, and Transport

a. Mukah Power Generation Sdn. Bhd. is located some 30 km away from Mukah town. The Contractor shall provide its own transport to ferry its own workers to the work site. The Contractor shall also make its arrangements and bear the costs of providing accommodation to its workers.

b. Similarly, the provision and costs of meals shall also be the responsibility of the Contractor. The Corporation's canteen will not be able to accommodate and to serve meals to Contractor's workers due to its limited facilities. As such, the Contractor shall make separate arrangements to deliver meals to its workers when necessary

16. Worker's Attire and Safety Compliant

a. The Contractor's workers shall be properly dressed in uniforms during the performance of their duties. Each worker must wear identification name tag to enable easy identification. The Contractor shall issue to his workers personal safety equipment such as safety helmet, safety shoes PPE and other equipment stated as requirements in MPG's safety regulations.

17. Equipment, material plus special personnel to be provided by the contractor

a. The contractor is required to provide own tools, equipment and material which are needed to perform the work in the power station. MPG however shall not to be held responsible for the safeguarding of the contractor equipment, tools and material brought to the power station. Housekeeping at working are shall be carried out day to day after completion of work. To provide tools list and tools inspection / audit by MPG personnel shall be carried out one week prior to overhaul work. Only tools that has been verified & tagged with sticker are allowed to be used.

18. Language

a. The specialist contractor from China if any will as far as possibly assign personnel who can read and understand English language to reduce language problem. Nevertheless, MPG will try to provide English-Chinese interpretation to smoother the communications at work site

19. Performance Evaluation

a. Based on work quality and duration of cleaning works as per MPG requirements.

Sarawak Energy shall provide to the Service Provider the following, for the purposes of providing the Services:

i. Electricity supply at 415V and 240V from existing terminals at site.

ii. Compressed air supply from existing terminal outlets at site. iii. Town (domestic) water from existing outlets at site.
