Kentucky Provider Directory Kentucky Provider Directory December 2013 How To Use Your Provider Directory This is a list of health care providers you may go to. A Primary Care Provider (PCP) is your main doctor who takes care of you. Call the Humana - CareSource™ Member Services Department at 1-855-852-7005 (TTY users: 1-800-648-6056 or 711) if you need help reading or understanding this list. It can be read to you in English or in your first language. Written translations may be possible. It may also be printed in some other way if you have trouble seeing. The data in this list is up-to-date as of 12/5/2013. The data can change. We check and update it often. You should always check with the provider to make sure. Call the Humana – CareSource Member Services Department at 1-855-852-7005 (TTY users: 1-800-648-6056 or 711) if you have any questions. This list is split into sections by county. You can go to any provider no matter what county they are in. If you don’t find the provider you’re looking for listed in your county, please check neighboring counties in this list. Or look on our website at Caresource.com/KY. Select “Find a Doctor/Provider” under the Kentucky Member tab. Your PCP is a doctor, or a doctor group. Your PCP could also be an Advanced Practice Registered Nurse (APRN) or APRN group. Or your PCP could be a Physician Assistant (PA). Network Pharmacies CareSource provides coverage for prescription drugs and some prescription medical supplies. CareSource contracts with pharmacies in order to provide members with a full range of prescription benefits. Members may choose and receive prescriptions from any pharmacy that is contracted with CareSource. These are often referred to as network pharmacies. It is important that members receive prescriptions from network pharmacies because prescriptions received from non-network pharmacies are generally not reimbursable or covered by CareSource, except as otherwise required by applicable federal and state law and your Evidence of Coverage. Accordingly, members may be responsible for the entire amount charged by a non-network pharmacy. Network pharmacies can include local pharmacies, mail-order pharmacies or specialty pharmacies.

Q: I already have access to the Provider Portal for Marketplace and Medicare Advantage, does this mean I will also have access to HIP and HHW? A: Yes. Once you have access, it’s for all lines of business. You do not need to do anything else. Q: How do I access the CareSource Provider Portal? A: Providers can access the portal on the CareSource website at caresource.com. To create an account, you will need your tax ID number, CareSource provider number, and zip code. Q: Can multiple people at one provider location have access to the CareSource Provider Portal? A: Multiple accounts can be set up from one Tax ID number.
