To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC). You will have to choose one of our network providers listed in this directory to be your Primary Care Provider (PCP). The term “PCP” will be used throughout this directory. Generally, you must get your health care services from your PCP. Your PCP is the healthcare provider that you see to receive the care that you need to keep you healthy. He or she is usually the physician that you see most frequently and will diagnose and treat most common medical problems. Your PCP will help you coordinate all of your medical care and will communicate with specialist physicians and other healthcare providers to address your medical needs. As an in-network provider, your PCP will request prior authorization when required. Your PCP may be any of the following provider types: Family Medicine Internal Medicine Geriatrician The network providers listed in this directory have agreed to provide you with your health care and vision services. You may go to any of our network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider. You must use network providers except in emergency or urgent care situations. If you obtain routine care from out-of-network providers, neither Medicare nor SelectHealth Advantage will be responsible for the costs. SelectHealth Advantage does not require members to obtain a referral to receive services that are covered by our plan. If you visit a network provider, the provider will contact us to get prior authorization. Billing Information Providers generally bill SelectHealth Advantage directly for your care. We then pay the provider for you, minus the copayment or coinsurance. If you receive a bill directly from a provider, please mail the bill to the following address: SelectHealth Advantage P.O. Box 30196 Salt Lake City, 84130-0196 iv Emergency Coverage It’s important to know you are covered and where to go when you need medical care. Our plan covers all urgent care, emergency care, and emergency hospital admissions worldwide, no matter where you go. You also get benefits for renal dialysis and access to participating retail pharmacies and lab facilities nationwide. Care provided by out-of-network providers that is not an urgent or emergency situation is not covered. Also, our plan does not cover any care that is non-urgent or non-emergency care received outside of the United States. What is the service area for SelectHealth Advantage? It seemed that John v'itherspoon T s career had been thoroughly described. His place in history is fixed by the great service he rendered in America during the years that include the birth of the United States. He ^Tas president of a leading college; he was a member of the Continental Congress; he was one of the signers of the Declaration of Independence; and he was moderator of the first General Assembly of the Presbyterian Church in the newly founded American republic. On closer examination,however,the subject showed great promise. The career of John '.Yitherspoon in America has overshadowed his earlier life in his native land. He was born in Scotland, and for twenty-four years he was a minister of the Scottish Church. This period should be brought to light, because it is important in itself and because it contains the elements of his later achievements. V.L.Collins,in his two volume work, President Witherspoon, leaves nothing to be desired in his exposition of ""itherspoon ! s American career; but he has not fully treated his attainments in his native land. No one has written fully on this period. It is the object of this thesis to discover and elucidate the facts of the career of John V'/it her spoon as a Scottish churchman.
