    Updated November 2019 KIT Mutation The KIT gene is associated with autosomal dominant piebaldism, gastrointestinal stromal tumors (GISTs), and familial mastocytosis.1-3 The type of mutation identified in the KIT gene determines the symptoms/cancer risks that the individual may have. Pathogenic loss-of-function (LOF) mutations in the KIT gene are generally only associated with Piebaldism. However, pathogenic gain of function mutations in the KIT gene are associated with systemic mastocytosis and gastrointestinal stromal tumors.4,5 Piebaldism: This is a rare condition characterized by the patchy absence of melanocytes in certain areas of the skin and hair. Melanocytes produce the pigment melanin, which contributes to hair, eye, and skin color. The absence of melanocytes leads to patches of skin and hair that are lighter than normal. These unpigmented areas are typically present at birth and do not increase in size or number.6-8 Gastrointestinal stromal tumors (GIST): GISTs are tumors that occur in the gastrointestinal tract, most commonly in the stomach or small intestine. Small tumors may cause no signs or symptoms. However, some people with GISTs may experience pain or swelling in the abdomen, nausea, vomiting, loss of appetite, or weight loss. These tumors can be cancerous (malignant) or noncancerous (benign). Mastocytosis: This is a blood disorder that occurs when white blood cells called mast cells accumulate in one or more tissues (most commonly in the bone marrow). Mast cells normally trigger inflammation during an allergic reaction. When an environmental trigger activates mast cells, they release proteins that signal an immune response. In systemic mastocytosis, excess mast cells mean more proteins are being released in the tissues where the cells accumulate, leading to an increased immune response.

Summary CD137 (ILA/4-1BB) is a member of the tumor necrosis factor receptor family (6), expressed on activated T lymphocytes (1, 11). The expression of CD137 is strictly activation dependent in primary cells (4). Co- stimulation through CD137 enhances T lymphocyte activation and may enable rejection of tumors in vivo (3, 7, 2). The gene for human CD137 resides on chromosome 1q36, in a cluster of related genes, and this chromosomal region is associated with mutations in several malignancies (9). Bidirectional signalling has been demonstrated for the CD137 receptor/ ligand system. While cross-linking of CD137 activates T lymphocytes, cross-linking of the CD137 ligand has the opposite effect. This reverse signalling through the CD137 ligand inhibits proliferation of T lymphocytes and induces programmed cell death (10).

Initially, UPR signaling aims at cytoprotection and restoration of ER homeostasis; that failing, it drives apoptotic cell death. ER stress initiates apoptosis through intracellular activation of death receptor 5 (DR5) independent of its canonical extracellular ligand Apo2L/TRAIL; however, the mechanism underlying DR5 activation is unknown. In cultured human cells, we find that misfolded proteins can directly engage with DR5 in the ER-Golgi intermediate compartment, where DR5 assembles pro-apoptotic caspase 8-activating complexes. Moreover, peptides used as a proxy for exposed misfolded protein chains selectively bind to the purified DR5 ectodomain and induce its oligomerization. These findings indicate that misfolded proteins can act as ligands to activate DR5 intracellularly and promote apoptosis. Soluble Fas receptor (sFas), produced by skipping of exon 6, inhibits apoptosis by sequestering Fas ligand. Serum sFas is associated with poor prognosis of non-Hodgkin’s lymphomas. We found that the alternative splicing of Fas in lymphomas is tightly regulated by a long-noncoding RNA corresponding to an antisense transcript of Fas (FAS-AS1). Levels of FAS-AS1 correlate inversely with production of sFas, and FAS-AS1 binding to the RBM5 inhibits RBM5-mediated exon 6 skipping. EZH2, often mutated or overexpressed in lymphomas, hyper-methylates the FAS-AS1 promoter and represses the FAS-AS1 expression. EZH2-mediated repression of FAS-AS1 promoter can be released by DZNeP (3-Deazaneplanocin A) or overcome by ectopic expression of FAS-AS1, both of which increase levels of FAS-AS1 and correspondingly decrease expression of sFas. Treatment with Bruton’s tyrosine kinase inhibitor or EZH2 knockdown decreases the levels of EZH2, RBM5 and sFas, thereby enhancing Fas-mediated apoptosis. This is the first report showing functional regulation of Fas repression by its antisense RNA. Our results reveal new therapeutic targets in lymphomas and provide a rationale for the use of EZH2 inhibitors or ibrutinib in combination with chemotherapeutic agents that recruit Fas for effective cell killing.
