Strategic Plan 2013 - 2016 Health & Wellness for All Prairie Mountain Health (initially named Western RHA) was established in June 2012 through the amalgamation of Assiniboine, Brandon, and Parkland Regional Health Authorities. This Strategic Plan is a bridge document covering the three year period from 2013-2016 and was developed by the Board based on Manitoba Health’s Vision, Mission and Strategic Priorities as well as the Strategic Plans from the three former RHAs. Prairie Mountain Health spans an area from the 53rd parallel in the north to the United States border in the south and reaches from the Saskatchewan border across to the lakes and central Manitoba. We employ over 8500 people and through a network of hospitals, health centres, personal care homes and community based services we deliver medical and surgical care, long term and transitional care, rehabilitation, mental health care, home care, public health and primary care, and emergency medical services. Encompassing mountainous parkland, wide prairies and an urban trading centre, the Prairie Mountain Health region is growing in population and diversity. Many people immigrating to Canada are choosing to settle here. New Hutterian communities are being created and the Aboriginal population is growing. Delivery of health care is becoming more complex, with increasing acuity in many settings. It was also noted that there are differences in health status across Prairie Mountain Health. With this in mind, a regional profile was created over the summer of 2012 to acquaint the Executive Team and Board with the nuances of the new region. The Prairie Mountain Health Board held a strategic planning session March 16th, 2013. -

2013 Community Health Needs Assessment – Cleveland Clinic Main Campus Founded in 1921, Cleveland Clinic is an academic medical center offering patient care services supported by research and education in a nonprofit group practice setting. More than 3,000 Cleveland Clinic staff physicians and scientists in 120 medical specialties care for more than 5 million patients across the system. Patients come to Cleveland Clinic locally, from all 50 states and from more than 132 nations around the world. Cleveland Clinic’s main campus, located in midtown Cleveland, includes a hospital with 1,288 staffed beds; an outpatient clinic; a medical school; a research institute; 26 specialty institutes; and supporting labs and facilities in 46 buildings on 167 acres. Cleveland Clinic patients represent the highest CMS case-mix index in the nation. Comprehensive services include heart care, digestive disease, nephrology and urology, cancer, neurology, diabetes and endocrinology, otolaryngology (ENT), rheumatology, gynecology, orthopaedics and pulmonology. Cleveland Clinic’s health system in Northeast Ohio consists of an academic medical center, two children’s hospitals and eight community hospitals. Each hospital is dedicated to the communi- ties it serves. We verify the health needs of our communities by performing periodic community health needs assessments (CHNAs). These formal assessments are analyzed using widely accepted criteria to determine and measure the health needs of a specific community. In accordance with Internal Revenue Code Section 501(r)(3), each hospital has conducted its own community health needs assessment. Upon review of all of the community health needs assessments for all of our Northeast Ohio facilities, Cleveland Clinic has identified five community health needs that are present in the ma- jority of hospital communities we serve. -

Please be aware that crying may start unexpectedly, and the student should be prepared. Failure to install Baby in a car seat in a motor vehicle could result in Baby or supplies becoming projectiles in the event of a sudden stop or accident. The student must bring his or her vehicle to a complete stop in a safe location before caring for Baby, or to retrieve a piece of Baby’s equipment that falls. Do not place Baby in or near water. SPRING 2021 Milo donates gifts and toys to Bluegrass Care Navigators each December to help children who are grieving the loss of their loved one.
