GRADE 8 Summer Waves Celebration!

On May 13, 2021, the 8th grade class will be going to Summer Waves in Jekyll Island, Georgia for a special end of year celebration. We have spoken at length with the physician on our covid task force to plan this trip as safely as possible for our students. Students will be traveling via bus and must be masked when they are: • On the bus • Waiting in line for food, drinks, etc. • Going to the bathroom • Hanging out anywhere on site They do not need to be masked when they are: • Eating • Waiting in line for and sliding down a water slide • Please bring an extra mask in case it gets wet or lost.

The park will be open to our 8th grade students, our faculty and the park’s employees only for this special event. Lunch and drinks will be provided. Buses will leave from Bartram promptly at 8:30 a.m. (Please leave home early to account for traffic/issues.) We expect to return at approximately 3 p.m. for regular school dismissal. To secure a reservation, payment of $75 and the signed forms (all three of them) must be turned in to your student’s advisor by Tuesday, April 27, 2021.

I give ______permission to travel to Summer Waves in Jekyll Island, Georgia on May 13, 2021. I hereby hold the Bolles School harmless from any and all liability. This statement shall serve as a release and assumption of risk for my child.

I authorize the School to obtain emergency care for my child in my absence. I agree to assume responsibility for any medical services rendered for my child and not to hold the School responsible for those services. I have included information below regarding my child’s special medical needs (i.e. asthma inhaler, food or insect sting reactions).

I also grant The Bolles School permission to use my child’s photograph in any promotional literature or on the school web site.

Thursday, May 13, 2021 students leaving from Bartram at 8:30 a.m. and returning by 3 p.m.

No student will be permitted to leave the park early.

Students will eat a catered lunch in the park. Food will not be allowed on the busses.

Students may bring electronics for the bus ride. However, the School is not responsible for loss or damage to any of these items.

IMPORTANT – We will be enforcing a code for this event and will refuse admission to anyone we deem to be inappropriately dressed. Therefore, any student not complying with this will not be permitted to board the bus. All students must wear a mask – pack an extra one.

The dress code is as follows:

GIRLS may wear Board , gym shorts, or running shorts (not spandex) with their Bolles t- One piece swim with their Bolles t-shirt and shorts An appropriate two piece with their Bolles t-shirt and shorts Comfortable , including tennis shoes and flip-flops

BOYS may wear of an appropriate length with their Bolles t-shirt Comfortable shoes, including tennis shoes and flip-flops Hats

Students may NOT wear Cut-offs, khakis, or street shorts. Swim must not have metal rivets, buckles or anything that might scratch the surface of the slides. Gym shorts, basketball shorts, , , or as a swimsuit with holes, tearing, or shredding Visible Tank tops or

FIELD TRIP AGREEMENT AND RELEASE

I, ______, parent or legal guardian of ______, who is a student at The Bolles School, do waive and release all claims against the school and authorized agents for any injury, loss, damage, accident, delay, or expense from the use of any vehicle, strikes, war, weather, sickness, quarantine, government, service, hotel, restaurant, or other firm or institution. I also release the school and its agents and agree to indemnify them with regard to any financial obligations or liabilities that my child may personally incur or any damage or injury to the person or property of others that they may cause while participating in the school’s program noted above.

I hereby grant the school and its agents full authority to take whatever actions they may consider to be warranted under the circumstances regarding my child’s health and safety. I fully release them from any liability for such decisions or actions as may be taken in connection therewith. I authorize the school and its agents, at their discretion, to place my child at my expense, and without further consent, in a hospital within or outside the United States for medical services and treatment or, if no hospital is readily available, to place my child in the hands of a local medical doctor for treatment, when the school or its agents deem it necessary.

I understand that this is a supervised program and that group standards must be observed. My child will at all times remain under the supervision of the school and its agents, and comply with the tour’s rules, standards and the chaperones’ instructions. I agree that they may terminate my child’s participation in the tour for failure to maintain these standards or for any actions or conduct which the tour chaperones consider to be incompatible with the best interest, harmony, comfort, and welfare of other students. I hereby waive and release all claims against the school and its agents arising out of my child’s failure to remain under such standards or to comply with such rules and instructions. I consent to my child being sent home at my expense with no refund or fees should, in the chaperones’ judgment, the situation warrants such action.

I certify that I am the parent or legal guardian of the above applicant, and that I have read the foregoing agreement and release (including such parts as may subject me to personal financial obligation and responsibility). I hereby relinquish any claim that I might have against the school or its agents, both in my own behalf and in my capacity as legal representative of the applicant, including without limitation, any claim arising as a result of the applicant leaving the supervision of the school’s representatives.

The Bolles School Student Behavior Contract Middle and Upper School

As stated in the Student-Parent Handbook, the Bolles student, whether on- or off-campus, represents the School at all times. As a result, it should be understood that conduct which brings discredit to the student, his/her family, teacher or the reputation of the school and its community may result in disciplinary action by the School. Therefore, each student is expected to demonstrate the self-discipline and positive values characteristic of a member of the Bolles community.

In particular, there should be a special emphasis placed on the following rules while participating in this off-campus activity:

 The Bolles Honor Code does apply.  Physical, emotional or sexual harassment will not be permitted.  The use of alcohol, tobacco, and other controlled substances will not be permitted.  All participants will adhere to the appropriate dress code as defined by the faculty sponsor.  Disrespect towards classmates, students or teachers from other schools will not be permitted.  Other: ______
