Finally, my gratitude to my wife Justina Dittoh. iv ABSTRACT This thesis titled “Poverty and disease: Effect of Guinea Worm disease on school attendance in the Tolon/Kumbungu District”, is an attempt at measuring the socio-economic impact of Guinea Worm disease on endemic communities (measured in terms of school attendance). This is with the understanding that Guinea Worm disease is a result of poverty and can consequently trap its victims in a vicious cycle of poverty and ill-health. Background studies across the world reveal that the incidence of Guinea Worm disease had decreased by 93% in the year 2000 from 3.6 million cases in 1986. The reduction could be largely attributed to the interest developed by Jimmy Carter (former president of United States of America) and The Melinda and Bill Gates Foundation towards eradicating Guinea Worm disease within the nearest possible time. Methodologies used in this study varied from utilization of formal questionnaires, focus group discussion and the employment of scoring and ranking tools. Heads of twenty (20) basic schools were interviewed comprising 10 primary schools and 10 Junior high schools, also focus group discussions were carried out with an average of ten (10) students in five (5) Junior high schools. At the community level, 10 endemic communities were selected to participate in focus group discussions whiles 38 patients infected with Guinea Worm disease between January and April 2008 (as presented by the district health directorate) were interviewed using formal questionnaires. Results from data collected have shown that Guinea Worm disease is still a problem in the district and has had a negative effect on the productivity of residents. It also reveals that majority of those infected are within school going age (5 to 18 years) and that students/pupils are absent v from school due to direct infection or infection of a relative. More so, 50% of schools surveyed revealed that a number of pupils/students never returned after they had been infected by the disease. Recommendations based on the study include (amongst others) the need for properly equipped Guinea Worm disease containment centres and the inclusion of education on Guinea Worm disease into the curricula of schools in endemic areas. There is also the need to work at alleviating poverty as the only way of permanently eradicating Guinea Worm disease and a clarion call for intense vigilance at the critical end of the eradication The effect of introduced infectious disease on wildlife populations is particularly pronounced on islands where parasite populations are characterised by increased intensity, infra-community richness and prevalence (the “Island Syndrome”). This thesis studied parasite and bacterial pathogens of conservation and zoonotic importance in feral cats from two islands (Christmas Island, Dirk Hartog Island) and one mainland location (southwest Western Australia), and in black rats from Christmas Island. The general hypothesis tested was that Island Syndrome increases the risk of transmission of parasitic and bacterial diseases introduced/harboured by cats and rats to wildlife and human communities.

The greatest interface between the parasite and its host is the cuticle surface, the outer layer of which in many species is covered by a carbohydrate-rich glycocalyx or cuticle surface coat. In addition many nematodes excrete or secrete antigenic glycoconjugates (ES antigens) which can either help to form the glycocalyx or dissipate more extensively into the nematode's environment. The glycocalyx and ES antigens represent the main immunogenic challenge to the host and could therefore be crucial in determining if successful parasitism is established.     Otranto and Eberhard Parasites & Vectors 2011, 4:41 http://www.parasitesandvectors.com/content/4/1/41 REVIEW Open Access Zoonotic helminths affecting the human eye Domenico Otranto1* and Mark L Eberhard2 Abstract Nowaday, zoonoses are an important cause of human parasitic diseases worldwide and a major threat to the socio-economic development, mainly in developing countries. Importantly, zoonotic helminths that affect human eyes (HIE) may cause blindness with severe socio-economic consequences to human communities. These infections include nematodes, cestodes and trematodes, which may be transmitted by vectors (dirofilariasis, onchocerciasis, thelaziasis), food consumption (sparganosis, trichinellosis) and those acquired indirectly from the environment (ascariasis, echinococcosis, fascioliasis). Adult and/or larval stages of HIE may localize into human ocular tissues externally (i.e., lachrymal glands, eyelids, conjunctival sacs) or into the ocular globe (i.e., intravitreous retina, anterior and or posterior chamber) causing symptoms due to the parasitic localization in the eyes or to the immune reaction they elicit in the host. Unfortunately, data on HIE are scant and mostly limited to case reports from different countries. The biology and epidemiology of the most frequently reported HIE are discussed as well as clinical description of the diseases, diagnostic considerations and video clips on their presentation and surgical treatment. However, little is known about the occurrence of Wolbachia in taxa closely related to the superfamily Filarioidea. Our study addressed the status of Wolbachia presence in members of the superfamily Dracunculoidea by screening the human parasite Dracunculus medinensis and related species from wildlife for Wolbachia. Findings: D. medinensis, D. lutrae and D. insignis specimens were all negative for Wolbachia colonization by PCR screening for the Wolbachia ftsZ, 16S rRNA and Wolbachia surface protein (wsp) sequences. The quality and purity of the DNA preparations was confirmed by amplification of nematode 18S rRNA and cytochrome c oxidase subunit I sequences. Furthermore, Wolbachia endobacteria were not detected by whole mount fluorescence staining, or by immunohistochemistry using a Wolbachia-specific antiserum. In contrast, positive control Brugia malayi worms were shown to harbour Wolbachia by PCR, fluorescence staining and immunohistochemistry. Conclusions: Three examined species of Dracunculus showed

(e-mail: [email protected]) ABSTRACT: A survey of the nature and phylogenetic distribution of nematode vulval appendages revealed 3 major classes based on composition, position, and orientation that included membranes, flaps, and epiptygmata. Minor classes included cuticular inflations, protruding vulvar appendages of extruded gonadal tissues, vulval ridges, and peri-vulval pits. Vulval membranes were found in Mermithida, Triplonchida, Chromadorida, Rhabditidae, Panagrolaimidae, Tylenchida, and Trichostrongylidae. Vulval flaps were found in Desmodoroidea, Mermithida, Oxyuroidea, Tylenchida, Rhabditida, and Trichostrongyloidea. Epiptygmata were present within Aphelenchida, Tylenchida, Rhabditida, including the diverged Steinernematidae, and Enoplida. Within the Rhabditida, vulval ridges occurred in Cervidellus, peri-vulval pits in Strongyloides, cuticular inflations in Trichostrongylidae, and vulval cuticular sacs in Myolaimus and Deleyia. Vulval membranes have been confused with persistent copulatory sacs deposited by males, and some putative appendages may be artifactual. Vulval appendages occurred almost exclusively in commensal or parasitic nematode taxa. Appendages were discussed based on their relative taxonomic reliability, ecological associations, and distribution in the context of recent 18S ribosomal DNA molecular phylogenetic trees for the nematodes.
