Non-vitamin K antagonist oral anticoagulants (NOACs) include thrombin inhibitor dabigatran and coagulation factor Xa inhibitors rivaroxaban, apixaban, edoxaban, and betrixaban. NOACs have several benefits over warfarin, including faster time to the achieve effect, rapid onset of action, fewer documented food and drug interactions, lack of need for routine INR monitoring, and improved patient satisfaction. Local hemostatic measures, supportive care, and withholding the next NOAC dose are usually sufficient to achieve hemostasis among patients presenting with minor bleeding. +e administration of reversal agents should be considered in patients on NOAC’s with major bleeding manifestations (life-threatening bleeding, or major uncontrolled bleeding), or those who require rapid anticoagulant reversal for an emergent surgical procedure. +e Food and Drug Ad- ministration (FDA) has approved two reversal agents for NOACs: idarucizumab for dabigatran and andexanet alfa for apixaban and rivaroxaban. +e American College of Cardiology (ACC), American Heart Association (AHA), and Heart Rhythm Society (HRS) have released an updated guideline for the management of patients with atrial fibrillation that provides indications for the use of these reversal agents. In addition, the final results of the ANNEXA-4 study that evaluated the efficacy and safety of andexanet alfa were recently published. Several agents are in different phases of clinical trials, and among them, ciraparantag has shown promising results. However, their higher cost and limited availability remains a concern. Here, we provide a brief review of the available reversal agents for NOACs (nonspecific and specific), recent updates on reversal strategies, lab parameters (including point-of-care tests), NOAC resumption, and agents in development. 1.     Update overview 2019 of reports on direct oral anticoagulants (DOACs) and the antidote idarucizumab Introduction Lareb previously published yearly overviews of reports (most recently in 2018) in consultation with the Medicines Evaluation Board CBG-MEB, concerning the direct oral anticoagulants (DOACs) dabigatran Pradaxa®, registered in the Netherlands in 2008 [1], rivaroxaban Xarelto®, registered in 2008 [2], apixaban Eliquis®, registered in 2011 [3] and edoxaban (Lixiana®), registered in 2015 [4-9]. The current overview provides a new yearly update of the reports received by Lareb for these DOACs. Furthermore, reports received by Lareb for the antidote idarucizumab are described in this overview. Idarucizumab is a specific antidote for dabigatran and was registered in the Netherlands in 2015 [10]. For this overview, data from the national ADR database were used. These data include reports with serious outcome from the Lareb Intensive Monitoring System (LIM). The DOACs have been monitored with the LIM methodology since September 2012. Prescription data The number of patients using DOACs in the Netherlands according to the GIP database is shown in table 1 [11]. These data are based on extramurally provided medication included in the Dutch health insurance package. Because the antidote idarucizumab is administered in the hospital and not reimbursed directly via the healthcare insurance, these data are not available for this drug. The number of reports received by the Netherlands Pharmacovigilance Centre Lareb per year since 2013 for each DOAC, is also shown in table 1. Furthermore, table 1 shows the calculated number of these reports per 1.000 users according to the GIP database. As noted before, the data from the GIP database represent reimbursed medicines and these may differ from actually prescribed medicines.

DOACs directly, selectively, and reversibly inhibit factors IIa or Xa. The coagulation effect follows the plasma concentration–time profile of the respective anticoagulant. The short half-life of a DOAC constrains the daily oral intake. Because DOACs have predictable pharmacokinetic and pharmacodynamic responses at a fixed dose, they do not require monitoring. However in specific clinical situations and for particular patient populations, testing may be helpful for patient management. The effect of DOACs on the screening coagulation assays such as prothrombin time (PT), activated partial thromboplastin time (APTT), and thrombin time (TT) is directly linked to reagent composition, and clotting time can be different from reagent to reagent, depending on the DOAC’s reagent sensitivity. Liquid chromatography–mass spectrometry (LC-MS/MS) is considered the gold standard method for DOAC measurement, but it is time consuming and requires expensive equipment. The general consensus for the assessment of a DOAC is clotting or chromogenic assays using specific standard calibrators and controls. The aim of this study was to test the efficacy of reversing the DOAC rivaroxaban using four-factor PCC (prothrombin complex concentrate), a non-specific reversing agent. Methods: Patients with life-threatening bleeding events during rivaroxaban treatment were included and administered 25 U kg−1 of PCC. Blood samples were collected immediately prior to as well as after PCC treatment at predefined time intervals. The primary endpoint was defined as the difference in thrombin generation (TG) parameters ETP (endogenous thrombin potential) and Cmax (peak thrombin generation) prior to and ten minutes subsequent to PCC treatment. Results: Thirteen patients, of whom the majority suffered from intra-cranial haemorrhage (ICH) or subdural haemorrhage (SDH), were included and administered PCC. The results show that the ETP (TG) significantly (p = 0.001) improved by 68% and Cmax (TG) by 54% (p = 0.001) during PCC treatment. In addition, the Quick value (prothrombin time: QuickPT) significantly improved by 28% and the activated partial thromboplastin time (aPTT) was PT decreased by 7% ten minutes after PCC administration. Cmax was reduced at baseline, but not ETP, aPTT or Quick . Lag time until initiation (TG, tlag), thromboelastometry clotting time (CTEXTEM) and time to peak (TG, tmax) correlated best with measured rivaroxaban levels and were out of normal ranges at baseline, but did not improve after PCC administration.
