Eric Holcomb, Governor State of Indiana Madison State Division of Mental Health and Addiction 711 GREEN ROAD MADISON, IN 47250-2199 812-265-2611 FAX: 812-265-7377

UPDATED: Tuesday, June 8, 2021 To our ’ families and friends,

Again, we would like to thank you for your patience and understanding with the suspensions and restrictions of visitations at Madison State Hospital during the Coronavirus pandemic. This action has been the norm for residential facilities since March of 2020 and has helped us to prevent and control infection spread in our congregate setting.

As the peak of the pandemic seems to be behind us, Madison State Hospital will be expanding visitation opportunities for our patients to every other weekend. Visitations will be available in a scheduled rotation of each building. Attached are the “Visitation guidelines during coronavirus precautions” - which is unchanged, and the revised “Visitation schedule during coronavirus precautions.” These will help inform you of the new visitation procedures and expectations, as well as outline when your loved one’s unit is available for visits.

There will continue to be pre-visitation screening questions that will be asked of you prior to and upon your arrival. We want to make you aware of these questions in advance in case your answers impact the ability of the visitation to take place. COVID-19 screening questions include:

1. In the last 72 hours, have you had a fever and/or taken medications for a fever? 2. In the last seven days, have you had symptoms of a lower respiratory illness? (cough, difficulty breathing) 3. In the past 14 days, have you been in close contact with a person known or suspected to have coronavirus or COVID-19? 4. Have you been diagnosed with COVID-19 in the last 14 days?

Please understand if one visitor in a pair (not more than two visitors per ) presents with a temperature (over 100.1) or positively answers any of the questions above, then both visitors will be denied entry for visitation. It is important to note that MSH may suspend visits with short notice in cases of localized outbreak, a rise in state and/or county metrics, or at the administrative staff’s discretion based on recommended health guidelines. You will continue to be able to review MSH’s COVID-19 status, as well as updates to the visitation limitations and expectations, through our webpage at https://www.in.gov/fssa/dmha/state-psychiatric-hospitals/madison-state-hospital/ or by calling the MSH COVID hotline at (812) 265-7475.

We appreciate your efforts over the past 15 months to be a part of your loved one’s life. The pandemic has proven how resilient we are when we exercise best practices and protect our vulnerable citizens.

Sincerely, Alicia Isaacs, MSW, LCSW Superintendent Madison State Hospital Indiana State Psychiatric Division of Mental Health and Addictions

Division of Mental Health and Addictions Madison State Hospital Visitation guidelines during coronavirus precautions

 Visitation requests will be scheduled as usual by contacting the social services specialist on your loved one’s unit.

 As usual, MSH prefers weekend visits so it does not conflict with active treatment scheduling for patients. Visits will be offered on Saturdays and Sundays of the weekend as assigned in the attached document.

 No food or drinks will be permitted during visits or brought for later consumption due to the removal of masks when eating and for infection control practices. Any food items brought for the visitation must be returned to the car before you enter the Administration Building.

 If your loved one needs additional items, it is strongly recommended that you mail packages at least one week in advance of your visit to allow for screening, cleaning and logging of the items. Please enclose a note indicating whether you want the items to be given during the visit or if they are to be distributed to your loved one once through processing. If not sent ahead of the scheduled visit, items will not be given at the time of the visit due to infection control processes. Please contact the unit social service specialist prior to sending any items ahead of your visitation or bringing any items during your scheduled visitation for your loved one to assure they can receive these items.

 Morning session (9-11 a.m.) and afternoon session (1-3 p.m.) last up to two hours each.

 Two visitors per patient per time slot.

 Visitors that have not been approved by the treatment team or do not have a valid release of information signed by the legal representative or patient prior to the visit will not be permitted to visit if they come with the scheduled family members/friends.

 Visitors are to arrive 30 minutes before the start of their scheduled visit time to allow for visitor processing.

 Visitor processing includes waiting at appropriately marked stations for check-in; temperature screening and symptom checklist; proper hand sanitizing; donning of face masks; escorting to the visitation area.

 Visitors must adhere to the marked lines in the Administration Building when undergoing visitor processing. This is to ensure at least 6-foot social distancing is safely occurring between visitors.

 MSH will supply masks and hand sanitizer for each visitor and patient for use during the visit.

 Visitors and patients must wear masks during the entire visit as well as maintain constant 6-foot social distancing between visitors and patients. This will be enforced by staff providing supervision during visits.

 At the conclusion of the visit, visitors will be escorted back to the Administration Building, properly doff their face mask, dispose of it in marked containers and use hand sanitizer before exiting the Administration Building.

 Administrative staff, security staff and those providing visit supervision must wear masks at all times.

 Visitation stations will be cleaned (using sani-wipes) by the staff providing supervision after each visit prior to the start of another visit.
