It lists pharmacies in your state and national pharmacy chains that are in our network. Pharmacies are listed in alphabetical order. All network pharmacies may not be listed in this directory. It is current as of the print date on the last page. Pharmacies may have been added or removed from the directory after it was created. For complete, up-to-date information about the plan’s network pharmacies in your area, or to check for a pharmacy not listed in this directory, please contact Customer Service. Network pharmacies We call the pharmacies in this directory our network pharmacies because we have made arrangements with them to provide prescription drugs to our plan members. A network pharmacy is a pharmacy where you get the prescription drug benefits provided by your plan. In most cases, your prescriptions are covered under the plan only if they are filled at a network pharmacy or through the plan’s mail order pharmacy service. You do not have to go to the same network pharmacy to fill each of your prescriptions. You may go to any of our network pharmacies. We may cover prescriptions filled at non-network pharmacies in certain situations as described in your Evidence of Coverage. For a complete description of your prescription drug coverage, including how to fill your prescriptions, please see your Evidence of Coverage and Formulary (Drug List). Pharmacy Saver™ The plan has negotiated with some of our network pharmacies to get even lower costs on hundreds of prescription drugs for our members.1 These include drugs to treat high cholesterol, high blood pressure, diabetes and much more. Prices may be as low as $2.2 All you have to do is show your member ID card at participating pharmacies. To learn more about Pharmacy Saver or to see the full list of participating pharmacies, visit www.UnitedPharmacySaver.com. Pharmacies included in the Pharmacy Saver program are indicated by a symbol in the pharmacy listings on the following pages. Mail service pharmacies You can use one of the plan’s network mail service pharmacies to fill prescriptions to receive a 90-day supply of your prescription drugs. Generally, it takes mail service pharmacies seven days to process your order and ship it to you. Please be aware of this turnaround time, and request refills early enough to leave time for your refills to be processed and sent to you. You should fill your prescription at a local network pharmacy if you are out of your medication and cannot wait for your mail service prescription to arrive. Please refer to the pharmacy listing on the following pages for our other network pharmacies in your area. The cost-shares for mail order drugs are listed in the Evidence of Coverage. 90-day supplies from retail pharmacies You do not have to use mail service to get a 90-day supply of your medication. Some of the retail pharmacies in the plan’s network also offer 90-day supplies of medications. If you fill a 90-day supply at a retail pharmacy, you may pay more.3 If you qualify for extra help, you may have lower or no copays or coinsurance when filling a 90-day supply. Retail pharmacies in the network that fill 90-day supplies of medications are indicated by a symbol in the pharmacy listings on the following pages. E-prescribing Some of our network pharmacies use electronic prescribing, or e-prescribing. The pharmacy receives your prescriptions electronically, directly from your doctor. Your prescription may be sent before you even leave your doctor’s office. E-prescribing can be a safe and efficient way for network pharmacies to get your prescriptions. It can help the pharmacy avoid reading mistakes and may alert your doctor to drugs that should not be taken with the prescribed drugs. Network pharmacies that use e-prescribing are indicated by an symbol in the pharmacy listings on the following pages. 1Other pharmacies are available in our network. Members may use any pharmacy in the network, but may not receive Pharmacy Saver pricing. Pharmacies participating in the Pharmacy Saver program may not be available in all areas. 2Drugs and prices may vary between pharmacies and are subject to change during the plan year. Quantities may be limited by retailer based on their dispensing policy or by the plan based on Quantity Limit requirements; if prescription is in excess of a limit, copay amounts may be higher.
