Damaged Schools /Classrooms in City, de Oro City and • Agusan del Sur () - 4 (GOMBURZA ES (2 Classrooms), roofing, windows and walls; Sta. Cruz ES (2 classrooms,); Sta. Irene ES (2 classroom), uneven settlement with crack wall. D. Osmeña ES (2 classroons, ripped apart); Lapinigan CES (4 classrooms; and Bunawan Brookside (4 classrooms) • City (Region X) – 4 schools namely ES, Angeles Sisters National High School; Balulang West1 ES and Bonbon North 2 ES were totally submerged with more or less 5 ft floodwater level – inspection and validation on the extent of damages incurred • del Sur (ARMM) – Sambolawan ES, Dilabayan ES and Riric ES in Taraka, Batangan CES, Rogero ES, Alambas PS in Bubong II • Iligan City Division: 12 schools and 136 classrooms were damaged

• Five (5) bridges and one (1) road section were damaged in Regions VII, X and XI

III. EMERGENCY PREPAREDNESS AND RESPONSE

A. EMERGENCY RESPONSE • Prior to the entry of TD “Sendong” in the Philippine Area of Responsibility, NDRRMC OpCen was already activated as early as 12 December 2011 under “Blue Alert” status to provide necessary alert and warnings to the RDRRMCs concerned for further dissemination to the provincial, municipal and city DRRMCs likely to be affected by TD Sendong. On 16 December 2011, OPCEN was further elevated to “Red Alert” status wherein representatives of respective member agencies rendered duty at NDRRMC OpCen to fast track coordination with their local counterparts for the necessary preparations and possible emergency response • Member agencies represented at OPCEN such as DSWD, DOH, AFP, PNP, PCG have been closely coordinating with their local counterparts

2 to address the needs and requirements particularly in the heavily devastated areas specifically Cagayan de Oro City and Iligan City • NDRRMC convened on 17 December 2011, presided over by the SND and Chairperson, NDRRMC, attended by all member agencies wherein His Excellency President Benigno Simeon C. Aquino III provided guidance to the NDRRMC Member Agencies. – TAB G • Chairperson, NDRRMC recommended to the Office of the President the declaration of the cities of Cagayan de Oro and Iligan under a State of Calamity the areas that suffered most in terms of casualties and population affected. • Likewise, accepted the offer of assistance from the international humanitarian community thru the United Nations Resident Coordinator and Humanitarian Coordination in the to the Philippine government for the victims of Tropical Storm Sendong

Search, Rescue and Retrieval Operations

• SAR and retrieval operations have been effected in heavily devastated areas through the concerted efforts of AFP, PNP, LGUs concerned and other emergency responders from NGOs

PCG Cagayan de Oro City (Rescued 1,722 survivors and 34 cadavers retrieved)

• Coast Guard District Northern immediately deployed two (2) teams from Special Operations Unit (SOU)-NM together with CGDNM and CGS Cagayan de Oro (CDO) personnel with rubber boat to conduct SAR operations at Brgys Macasandig, Mabini, Burgos and Isla de Oro. ‹ Team A (8 personnel with rubber boat rescued 200 survivors) ‹ Team B (8 personnel with rubber boat rescued more or less 50 survivors) • Other team (3 personnel on board M/V Maria Stella 1 rescued) • Vessel Traffic Monitoring System of Macalajar Bay manned and controlled by the DCC personnel directed all shipping lines and vessels transiting along Macalajar Bay to conduct surface SAR operation, • MV Solid Gold, MV Snoppy, MV Super Shuttle Ferry 3, MV Maria Stella 1 and MT Tagoluan successfully rescued 121 survivors • All survivors were provided with assistance CGSNM and were brought to City Central School evacuation center and the cadavers were brought to Bulusog Funeral Parlor, Buluo CDO • Four (4) SOU-NM with rubber boat onboard PG371 of PN conducted joint SAR mission in Macalajar Bay, CDO and successfully re recovered 2 survivors • BRP (SARV 3504 departed Port of CDO enroute to Island and retrieved 8 cadavers and brought to funeral parlor for proper disposition and identification • A total of 255 survivors were rescued and 14 cadavers retrieved • BRP Davao del Norte (SARV-3504) and MCS 3004 conducted SAR operations in , CDO and 12 cadavers retrieved (4 cadavers by SARV-3504; 8 cadavers by MCS 3004) and in Iligan City (49 survivor rescued and 5 cadavers retrieved)

3 • CGDBCL stations/detachments conducted monitoring and security to stranded passengers in ports in their respective AOR • Quick Reaction Team composed of PCG personnel, auxiliary and PNRC volunteers on board MT Seaborne, QRT team and PG-390 rescued 23 survivors who were turned over to LGUs of Iligan and 4 cadavers brought to Cavin funeral homes • CG Detachment coordinated with HOLCIM Philippines to direct anchored vessel to conduct maritime Search and rescue at vicinity of , Lugait and areas. MV Alai Capricorn 3, MV Lorenzo and PG390 of conducted SAR operations covering 20 Sq miles of surface area, Rescued 10 survivors • MCS 3004 with 2 rubber boats on board departed port of Iligan and conducted SAR and retrieval operations and Jibe medical evacuation for possible victims reported to be isolated at Brgy Santiago, Hinaplanon and Bayug island - Rescued 16 survivors and retrieved 1 cadaver

PNP • PNP Crime Laboratory dispatched 10-man team led by PSupt Francisco A Supe Jr, MD on 19 December 2011 to conduct dead victim identification (DVI) in Cagayan de Oro City • TG Tabang Iligan proceeded to its designated area of operations to conduct SAR operations • RD PROX proceeded to Iligan City to oversee the ongoing SAR operations being conducted by combined elements of ICPO, one platoon RPSB 10, one platoon PPSC and RTS 10 led by City Director PSSUPT Celso Regencia • PRO X ladies visited MSU Iligan Institute of Technology (the biggest evacuation center) and distributed food, water and other relief goods worth Php120,000 to more than 300 greatly affected families • RD PRO X and party including the PRO 10 ladies proceeded to Cagayan de Oro City and provided relief goods worth PhP100,000 to 350 families at Brgy Balulang, Cagayan de Oro City • A total of 150 personnel from PRO CARAGA and 96 personnel from PRO 11 arrived PRO 10 and provided support in the SAR efforts in Iligan City Cagayan de Oro City • Students of PNP Regional Training School were mobilized to conduct SAR operations in Brgys of Macasandig, Balulang and Consolacion in Cagayan de Oro City • RPSB personnel and RTS Police recruits augmented police personnel of COCPO

AFP • CENTCOM-AFP directed two (2) UH-1H helicopters with personnel to complement the 5052 nd SRS in Mactan Airbase to augment SAR operations in , Valencia, , and City • The (PAF) aircrafts (C-130 and Fokker planes) airlifted PhP21, 532, 299.42 worth of relief supplies (food, medicines and non-food items from DSWD, UNWFP, Maynilad, PRC, DOH as augmentation support to the ongoing relief operations in Region X • 4ID dispatched Rapid Assessment Teams (RRT) and the Technical Operations Group 10 (TOG10) to the affected areas of Region X • Philippine Navy (PN) BRP City (LC551) actual time of departure 10:27PM 20 December 2011 bound for Iligan/CDO loaded with 179.82

NGCP • The NGCP has restored all its 69-KV transmission lines affected by Tropical Storm Sendong in Panay area and Mindanao areas (Lanao, Northcentral and northeastern) and assured the public that it is ready to implement similar preparations and precautions to minimize the impact of succeeding typhoons on NGCP operation and facilities

DOH • As of Dec 19, 2011, DOH had so far provided logistical support amounting to PhP5,100,692.08 • Coordinated with NBI for the implementation of protocol for management of the dead and missing; with DOH Central warehouse for mobilization of logistics, with cluster partners (WASH, Health and MHPSS); and with all RHEMS Coordinators concerned in the affected regions

• As of 20 Dec, one (1) medical team and one (1) psychosocial team from CHD XI were deployed to CHD X; one (1) medical team from Mayor Hilarion Ramiro Regional Training and Teaching Hospitals augmented assorted medicines/ supply to CHO Iligan City; and Medical Center mobilized ambulance to Coast Guard to transfer patients retrieved by PCG • CHD CARAGA augmented 9 cadaver bags and 72 boxes of hyphosol to CHD X; pre-positioned medicines and supplies; issued alert messages to local HEMS in respective provinces and cities for appropriate response and coordination; • CHD VII mobilized team to Sibulan, Valencia, Dumaguete City, San Jose, Pamplona and and augmented assorted medicines, sodium hypochloride and IVF fluids in Oriental worth PhP30,000.00 • Public health concerns: WASH lack of potable water supply in CHD X, ARMM and Lanao del Norte

Dep Ed • DepEd dispatched an inspectorate team composed of technical staff from Dep Ed Central, Regional and Division Office to conduct damage and needs assessment to all affected schools in Cagayan de Oro City and iligan City, respectively • Dispatched technical staffs (Division Project Engineers/Architects and PFCs) to conduct ocular inspection and evaluation to quantify the amount of damages of all affected schools and Northern Mindanao flashflood. • COMVAL CES (14 classrooms) are being utilized as evacuation centers as temporary shelter of affected families in the province

DSWD • The Disaster Response Operations Monitoring and Information Center (DROMIC) has been closely coordinating with DSWD-FOs concerned for any significant reports caused by said weather disturbance;

5 • SWAD Teams are closely coordinating with their local counterparts in terms of validation of data as basis for augmentation anytime • DSWD NROC airlifted thru AFP Aircraft relief assistance worth PhP21,532,229.42 to DSWD FO X broken down as follows:

Region I • DPWH Region I expedited clearing operations in the areas affected by landslide utilizing heavy equipment and private contractors

Region V • PCG-DCEV issued hold orders to vessel plying areas under public storm warning signals specifically , Negros, and routes • PDRRMO conducted on site inspection of road bed slippage • SAR Service conducted monitoring activities in all areas of concern especially in the 4KM Permanent Danger Zone of the Bulusan Volcano

Region VII • PCG-DCEV issued hold orders to vessel plying areas under public storm warning signals specifically Bohol, Negros, and Siquijor routes • PDRRMCs , Bohol, and Siquior were convened on 14 December 2011 to review pro-active and operational measures of council members; assessment and coordination with the OCD Regional OpCen is being done on a regular basis; and monitoring of hazard-prone areas • DRRMC sent teams to affected areas for road clearing operations • MDRRMC and -based Rescue Teams conducted road-clearing operations and pre-emptive evacuations in identified hazard areas • BFP Dumaguete, 79 th IB and 302 nd Bde, PA based in Negros Oriental mobilized to assist the affected populace in areas affected by flooding

7 • CDRRMC Dumaguete City and PDRRMC Negros Oriental simultaneously held Emergency Meetings on 17 December 2011 to assess extent of damage and needs requirement of affected families • 302 nd Bde, PA provided search and rescue support to LGUs affected • Provincial Public Safety Company (PPSC) Negros Oriental deployed one (1) team to the affected areas of Sibulan for SAR operations • DSWD VII in close coordination with the OCD and the AFP are now facilitating the shipment of relief goods and the possible withdrawal of 100 bags of rice from NFA Dumaguete City; and has a standby fund worth PHP445,000.00 and PHP 4,000,000.00 worth of relief goods • DOH VII dispatched a medical team from the HEMS to augment the emergency medical services in the affected areas • RDRRMC VII convened an emergency meeting yesterday, 18 December 2011 at 5:15PM, with DILG RD, DPWH VII, CENTCOM-AFP, PRO VII,PCGDCEV, DOH VII, and OCD VII to assess the extent of damages in Negros Oriental ; conducted DANA and ocular inspection in the affected areas of Negros Oriental; and met with the President to discuss measures undertaken for response and rehabilitation efforts of the affected areas • CDRRMs and Naga extended financial assistance to the areas affected in Negros Oriental • RDRRMC VII indicate that the immediate needs of the affected populace are water, blankets, clothing, food, milk for children, and basic kitchen utensils • Joint SAR Team of CGS Dumaguete City together with Negros Oriental SAR team, BFP Dumaguete, PNP Dumaguete, PNP Marig, 211 th & 204 PCGA Squadron conducted SAR operations for M/V Ever Transport • Rescued crew of M/V Ever Transport was temporarily turned-over to the LGU of Brgy. Calindagan, Dumaguete City, where they were fed and given medical assistance and were billeted at the

Region IX • PDRRMC ZDN and CDRRMC City conducted emergency meeting to assess the condition, damages and needs of reported families/persons • CDRRMC Dipolog City is in constant coordination with MDCC Polanco Action Officer for any development of Polanco River; provided rubber boat to augment rescue equipment; and conducted assessment and profiling of affected families in collaboration with CSWD; and provided 1 sack of rice, noodles, and sardines to support relief operations in BDCC Turno • BDRRMC Turno headed by PB Meynardo Baes with the support of rescue teams evacuated the affected families and brought them to Turno Elementary School for temporary shelter; established community kitchen; and provided initial food assistance to the evacuees • 101 st Bde Rescue Team headed by Capt. Art Villanueva together with Special Forces assigned in the area responded in Municipality of Polanco and Dipolog City and assisted in transporting passengers trapped by floods along the National Highway at Brgy Guinles, Polanco • Polanco MPS led by PI Carmelo Cruzada Velasco, one (1) team from PPSC, PPO together with troops of 101st IB PA, LGU Polanco, and PRC ZDN chapter conducted SAR operations at severely affected areas • City Police Station led by PSupt Glenn Macaio Dulawan together with one (1) team from PPSC, AFP Condor 3, and CDRRMC Dapitan conducted SAR operations in affected areas; provided assistance to the vehicles passing the National Highway near Brgys Sulangon, Oyan, Burgos, and Ilaya; assisted in the evacuation of all the families near the Dapitan river to a safer place

8 • ORACIS Radio Communications Group headed by Dr. Asis provided radio communication link, rescue ropes, transport vehicles and additional rescuers in the Municipality of Polanco and Dipolog City • Dakak Beach Rescue Group headed by Roel Hamoy assisted in the conduct of rescue operations using two (2) jet skis • Coast Guard Station in Dipolog City augmented the rescue operations utilizing 1 speedboat and 2 rubber boats • Local Government of Polongco distributed 30 bags of noodles, sardines, mineral water, rice and canned goods to the affected families • LGU Polanco distributed 30 bags of noodles, sardines, mineral water, rice, and canned goods amounting to PHP 7,500.00 • CSWD Dipolog provided Food and other services for the affected families in the Dipolog City flashfloods

Region X • At the height of flooding, 10 evacuation centers were being utilized in Cagayan de Oro City, • PCG Iligan deployed one (1) Search and Rescue (SAR) to Brgy Hinaplanon, Iligan City and rescued three (3) unidentified persons with bruises • CHD X raised to Code White Alert and conducted rapid health assessment in the affected areas; prepositioned medicine worth PHP 50,000.00 per province (all 6 provinces in region X) and coordinated with the PDRRMC, Philippine National Red Cross, Rescue 118, and other responders • CDRRMC Iligan established four (4) evacuation centers at the height of flooding, namely: City Central School; Sta. Filomena Gym; Santiago Covered Court; and the Upper Hinaplanon Barangay Hall & covered court for the affected families in Iligan City • TOG-PAF mobilized three (3) choppers: 2 from Davao and 1 from • RDRRMC/OCD X Operations Center conducted ocular inspection in affected areas; assessment of immediate needs; leading the mobilization of resources, assets, and responders; and continuing survey and monitoring of affected areas • Members of the 4ID-PA, TOG-PAF, PNP, and OROALERT were deployed to different areas to rescue stranded commuters and persons in areas reachable by the rescue teams • CDO CDRRMC established evacuation centers in the City Central School, West Central School, and Macasandig Covered Court. Survivors were given basic needs • The 10 th Tactical Operations Group conducted search and rescue operations and aerial survey of the affected areas • PNP Maritime Office X dispatched three teams with three rubber boats to various areas in Cagayan de Oro City • Regional Director PCSupt Juel Corpus Adriatico of PRO X together with the Regional Chief Directorial Staff, members of Quad Staff, and PRO 10 Ladies headed by Mrs Joji Adriatico went to Iligan City to oversee the continuing SAR operations being conducted by the combined elements of ICPO, RPSB10, Lanao del Norte PPO, and RTS10 led by City Director PSSupt Celso Regencia; distributed food, water, and other relief goods worth PHP120,000 to the 300 affected families staying at the MSU-Iligan Institute of Technology; and distributed relief goods worth PHP 100,000.00 to 350 affected families in Brgy Balulang, Cagayan de Oro City • PRO X deployed 10 SAR teams to conduct SAR operations

9 • 1002 nd Maritime Police Station (MARPSTA) Iligan City led by PINSP Renato G. Rama together with elements from the PCG-Iligan conducted joint SAR operations at the Orchids Village, Santiago, Iligan City • DSWD X provided initial augmentation to the LGU of CDO: 40 bags of rice, 40 cases of sardines, 20 big kettles, 10 big kawa, 10 pieces of sandok, an 241 family packs, all worth PHP 170,400.00 • NDRRMC provided financial assistance to the beneficiaries of 4 dead victims • CDRRMC Cagayan de Oro will conduct mass burial for the unidentified bodies at Bolonsori, Camaman-an, Cagayan de Oro City after the forensic processing of the NBI • CDRRMC Iligan City started burying unidentified bodies at Brgy. Pugaan Cemetery and will do the same in Brgy. Paitan • RDRRMC X indicate that they need supplies of drinking water, water purifiers, portable tents, food, and NFIs

Region XI • PDRRMC Comval and MDRRMCs Compostela, Monkayo, and Montevista extended relief assistance to the affected families and assisted in the evacuation operations • PDRRMC Davao del Norte conducted damage and needs assessment in the area and closely monitored the level of flood waters in Asuncion • MDRRMC Monkayo conducted search and rescue operations

Region XII • OCD XII and PIA XII coordinated activities intended to generate support for the flood affected areas • Province of deployed a team of responders and volunteer nurses to Region X and established collection point at provincial gymnasium for processing and dispatch of relief goods • City is sending relief assistance and deploying a response team to CDO

CARAGA • PDRRMC convened at 8:30 AM while CDRRMC of convened at 9:00 AM on 16 December 2011 • PDRRMCs and MDRRMCs conducted damage assessment and needs analysis in the affected areas • PCG issued a notice to mariners to denied all vessel departure including motor bancas, fishing boats, small boats; information was received from PCG Nasipit, Station that there are 200 passengers stranded at Nasipit Port bound to , Bohol; Port Authority coordinated with MDRRMC of Nasipit to support the stranded passengers • 36 th IB of PA stationed at Tago, , alerted their troops and sent out teams to monitor Tago River and other flood prone areas in their AOR • Received information from CAAP Butuan, Bancasi Airport, flight from to Butuan was cancelled, but PAL flight from Manila to Butuan had landed and departed Butuan back to Manila as scheduled • DPWH Regional Office activated their Regional and District Disaster and Risk Management Teams and deployed along flood and landslide prone areas • PNP Hinatuan provided assistance in the evacuation of the victims and are providing security in the evacuation centers • Surigao del Norte Matugas cancelled the scheduled Barangay Development Forum in Surigao City

ARMM • Acting ARMM Governor Adiong provided relief goods to the affected families in • Elements of the 64 th Infantry Batallion are helping the affected LGUs of Lanao del Sur in their SAR operations

NCR • OCD NCR facilitated the deployment of assistance pledged by the following government agencies, volunteer groups and private organizations to Cagayan de Oro City and has recommended that said equipment and manpower be accommodated to the C-130 flight on December 21, 2011 MMDA – 1 unit water purifying equipment with truck ( 2 tons ); 1 rescue team ( 19- man ); 1 multi-cab; medical kit; basic tolls and rescue kit deployed at 4:00 PM on Dec 20, 2011 at Sulpicio Lines BFP-SRU – Pelican light Infrared Camera; 2 gen sets, 1 unit rubber boat with outboard motor; 1 unit water purifier with truck and 1 rescue team (10 man) Association of Philippine Volunteer Fire Brigade (APVFB) – assorted relief goods ( clothes and slippers) CDAG – 10 aquaboxes with standby 15-man team; 1 unit rescue van with rubber boat (no outboard motor) and 1 unit ambulance for deployment CDAG – relief assistance TAO/C3 Snaptech – assorted medicines, communication equipment, medical team (1 squad (7-man) ready for deployment C3DSI –DESAR – Composite teams for deployment Flood Control Barangay Zone Alliance - mobilizing relief assistance Local Water Utilities Administration – 2 portable water treatment 10 gal/min; 2 mobile treatment plant; commercial boat; Electrical and Mechanical Engineers to help CDO Water district; LWUA Administrator Salvador Ner and Chairman Rene Villa conducted needs assessment to CDO CDO

A. Floodings

Region VI • - Sigma, (Brgys Agbo, Amaga, Cogon, Dayhagon, Norte, Poblacion Sur, Guintas, Pagbunitan, Bangon-bangon, Mangoso, Capuyhan, Tawog, and Matangcong and Panitan (Brgys Cabanghan and Cabugao)

Region VII • Negros Oriental – Sibulan, Valencia, and Dumaguete City

Region IX • Zamboanga del Norte - Polanco- were temporarily evacuated to higher grounds for safety • Barangays Turno (Sitio Wakwak) and Dicayas in Dipolog City on 15 December 2011 at around 10:00 AM. • Sixty (60) families/300 persons from Sitio Wakwak, Brgy Turno evacuated to Turno Elementary School. Six (6) houses and one (1) Day Care Center were totally destroyed by raging waters. Five (5) other sitios were also affected but no report of displacement. Residents in Brgy. Dicayas, opted to stay with their respective houses in spite of the reported rise of flood level.

Region X • Massive flooding occurred in the following areas due to the overflowing of Mandulog and Agus Rivers: Lanao del Norte - Iligan City (24 brgys); (Valencia City); Misamis Oriental - El Salvador City (Brgy. Mougan) and Cagayan de Oro City (29 Brgys; (Ozamis City (6 Brgys) and Clarin (1 Brgy)

Region XI • Compostela Valley Province (Municipalities of Compostela (2 Brgys), Monkayo (3 Brgys), (2 brgys , New (1 Brgy), and Montevista (1 Brgy) • A total of 106 families from Compostela evacuated to the Municipal Gymnasium and Central Elementary Schools and 18 families from Montevista at the Municipal Gymnasium • Davao del Norte (17 December 2011) with 514 families affected in Brgy. Napungas, Asuncion

B. Landslides

Region XI • Compostela Valley – Moncayo (Purok 4, Purok 22 and Mt. Diwata, Diwalwal)Lanao del Sur in Kapai, (Brgys: Togonuan; Parao; Malimono; and Dimagalig) and Bubong (36 Barangays) and due to the overflowing of the and Siguan River

C. Sea Mishaps

Region VII

• One Cargo Ship “Ever Transport Cargo” anchored at Brgy. Calindagan, Dumaguete City sank due to strong winds and big waves. Said cargo ship is owned by Kherl Lines loaded with Asia Brewery products

• LCT Tampi Twin submerged halfway while moored at the Maayo Private Wharf in Sibulan, Negros Oriental due to big waves. The sea water entered through the open portion of its stern and thereby flooded the engine room

• A pump boat named “Bantay Dagat” owned by LGU Lazi, Siquijor sank while docked at the Lazi Port after being battered by strong winds and big

• F/BCA ALLEN JOHN with eight (9) fishermen on board, was reported missing since 16 Dec at Vicinity 60 NM East of Cateel Bay , City was recovered and eventually sunk when battered by strong waves. The fishermen arrived safely in good condition
