Academy Player Registration and Participation – Terms and Conditions.

Perth Glory Academy Teams U13s to U20s compete in the PS4 National competition which is overseen by Football Federation (FFA) in partnership with our, state-based Member Federation, .

1. PARTICIPATION Player development is our highest priority while your son is registered to play for Glory FC, and we require 100% commitment from every player and parent in return. All Parents and Players are to attend the induction night held at the Club headquarters to ensure all parties involved are aware of the expectations of all players registered with the Perth Glory Academy. If you foresee this to be a problem moving forward, please contact our administration email [email protected]. We would prefer you are certain that your child will be attending, as fees cannot be refunded in full as outlined in our Cancellation and Refund Policy.

1.1. Training Sessions and Games We are one of four, 2 Star Academies in Australia. Our program is both extensive and expansive across all areas from technical, tactical, physical and social. With this in mind, we will be increasing our on and off field contact sessions to 4/5 training contacts plus 1-2 games per week. These will range in content, from further Athletic Development, Analysis reviews, Skill Development and the increase of our Elite games played, and given this WE REQUIRE THE FULL COMMITMENT FROM ALL PLAYERS AND DUE TO THE INCREASE IN TRAINING LOADS, WE WILL NOT SUPPORT ANY EXTERNAL FOOTBALL TRAINING OUTSIDE OF PGFC ACADEMY.

1.2. Provision of Training Kit Players will be sized for Kit at the beginning of preseason and will receive all kit prior to or on the first training session. The cleaning of and maintenance of the Kit will remain the responsibility of the Player and their Parent/Guardian. a) Players will be sized for Kit after selection and registration completed. All kit will be distributed accordingly. b) Players must wear only PGFC training kit while at Academy training. All kit must be clean and presentable at all times. c) Players must Wear PGFC Tracksuit & Polo to all games and as directed by the Team coach or Technical Director.

2. REGISTRATION Perth Glory FC is unable to verify if a player has secured their place into the Academy until such time the player has completed the following;

a) Provide important Player information and pay Registration Fees (within the time frames set-out in point 4.) via the Perth Glory FC online portal. https://premier.sportsubs.com.au/aleague/perthglory/myaccount/login A Perth Glory Member Account will have already been created either as a previous parent/guardian for an Academy Player or if you are new to The Club when you signed up for the 2019/20 Academy Trials. If you have forgotten your log-in details, please click on Forgot details and follow the prompts.

b) By registering with the Perth Glory Football Club using the Club’s on-line Portal (SportsSubs) you (the Member) or, if the Member is under 18 years of age, the Member's parent or legal guardian:

i) Declare to have read and agreed to be bound by the conditions of registration and that the details provided via the online registration form are, to the best of my knowledge true and correct. ii) By submitting any registration forms online, you give your consent that you have read in its entirety, understand and accept all terms and conditions.

3. MEDICAL The Club’s Medical team are responsible for training and match day assessment of injuries. If a player needs assessment or medical attention, the Club has preferred medical providers which will be discussed between the parent/guardian and our Club’s Physiotherapist. Once diagnosis and prognosis are established the player will be progressed according to the stages of rehab detailed in the Academy Induction.

3.1. Payment of Medical Bills Perth Glory FC are not responsible for the payment or reimbursement of Medical and Hospital accounts for injuries sustained whilst training or playing.

3.2. Medical Cover and Health Insurance Parents are responsible for players being insured through family Private Health Insurance policies or through comprehensive state insurance plans. All costs associated with injury must be covered by the player/parent outside of the FFA National Insurance Programme. It is important for the player to have private health cover as some elective / non-essential surgery can be delayed in the Public Health system for some time.

4. PAYMENT OF REGISTRATION FEES 4.1. Non refundable Deposit A non-refundable Deposit of $500 (40%) of full fee payable will be required within one week of notification of successful position within the Academy. You will be required to acknowledge you have read and understood the payment terms and conditions when processing your payment on the Perth Glory FC website.

4.2. Payment options a) Payment in Full is preferred b) Instalments will be offered and will incur processing Fees. Failed instalments will incur further admin fees and may result in the player either not being selected for game participation or his place being taken by another player. c) PGFC reserves the right to elect or reject payment plan options in its absolute discretion. If you are having difficulty, for whatever reason in paying fees of any kind to the academy, it is strongly advised to email [email protected]. The academy will make every effort to view the situation of individual students sympathetically, but it is bound ultimately to collect any income owed.

5. REGISTRATION CANCELLATION AND FEE REFUND POLICY The Club’s position on eligibility for refund of registration fees either in part or full will be at the discretion of the Peth Glory Football Club. By submitting the online Registration form and paying the fees (in part or full), you as the purchaser, agree with the Terms and Conditions of the Perth Glory Academy Registration.

6. RELEASE OF REGISTERED PLAYER UNTIL ALL REGISTRATION FEES HAVE BEEN PAID OR UNTIL PLAYER’S HAVE ADHERED TO THE REGISTRATION TERMS AND CONDITIONS - NO PLAYER WILL BE RELEASED ON PLAY FOOTBALL BY PERTH GLORY FOOTBALL CLUB.

7. RULES, REGULATIONS AND POLICIES By agreeing to these terms of participation, the Player: a) Has completed their registration via the Football West, Play Football (formally known as MyFootballClub) website. Whether a first-time player or a returning player, you must register to the Perth Glory Football Club on Play Football website each year prior to the Football West Registration Window closes.

b) agrees to comply with the Rules, Regulations and Policies of the Club, FFA Statutes, including the National Registration Regulations, National Code of Conduct, National Disciplinary Regulations and Grievance Resolution Regulations, copies of which are available on www.footballaustralia.com.au c) acknowledges that a failure to adhere to the Rules, Regulations and Policies of either the club, the FFA, and the Member Federation may result in the Player being de-registered; d) Acknowledges that the Player may inspect copies of the Rules, Regulations, Policies and Laws of Football upon request to any of the relevant governing bodies. e) agree to comply with the Laws of the Game and the Competition Administrator's Competition Rules; f) Acknowledge and agree that registration is with Perth Glory FC until the later of the end of the Season or the time as specified in the Competition Rules; and g) Acknowledge and agree that Perth Glory may de-register you at any time by notice in person or in writing to the address or email address provided as part of your registration, including in the event that you breach these Terms or do not comply with its incorporated documents, such as the FFA Statutes and the Competition Administrator's Competition Rules. h) Acknowledge and agree with the Terms and Conditions of payment via the Perth Glory online portal. i) PGFC reserves the right to not accept any application for Registration in its absolute discretion.

8. USE OF IMAGES Grant Perth Glory FC, FFA, the Member Federation, the Competition Administrator a non-exclusive, royalty free, world-wide, irrevocable license to use your name and/or image in any form or media format for the purpose of marketing and promotional activities. Photos/images will not be sold to third parties;

If you do not wish your image to be used, you must indicate at the time of your registration on the portal with the tick in the box that you “DO NOT” authorise event staff to take pictures/videos.

9. PRIVACY STATEMENT a) Acknowledge that the personal information that Perth Glory collects from you is used and disclosed for the purposes set out in our Privacy Policy, including processing your registration, the organisation, conduct and promotion of Competitions and Matches, the provision of football-related information and offers, and information about our commercial partners, tickets and events as part of the FFA Football Family. You may direct any questions or concerns about how the Club may collect, use or store your personal information to [email protected]. A copy of the Privacy Policy is available on www.perthglory.com.au. b) You consent to the collection, use and disclosure of your personal information as described above. If you do not want to receive certain information from the Club or FFA and their commercial partners, you will be given an opportunity to opt out of such communications;

10. DISCLAIMER AND EXCLUSION OF LIABILITY a) Acknowledge and agree that there are inherent risks associated with participating in football that may result in personal injury and you agree to fully bear those risks; b) Agree not to bring any claim or proceeding against Perth Glory FC (including each of their directors, officers, employees, agents, contractors and assigns) at any time for any damage, loss, injury or liability that you may suffer or incur in connection with or arising out of your participation in football. Except for liability that by law cannot be excluded, Perth Glory FC exclude all liability to you in tort (including negligence), contract or otherwise for acts or omissions of the Perth Glory Football Club (including each of their directors, officers, employees, agents, contractors and assigns); c) Agree to indemnify Football Administrators (including each of their directors, officers, employees, agents, contractors and assigns) from and against all actions, proceedings, suits, claims, liabilities, damage, costs and expenses that may be incurred or sustained by each of them arising out of any person commencing any proceedings against you for any assault, negligence and/or breach of the FFA Statutes, Laws of the Game and/or Competition Administrator's Competition Rules; d) Acknowledge that the Member Federation of the Club will provide insurance to cover injuries sustained while participating in football and football related activities for that Club or Member Federation, subject to the scope and coverage as prescribed by that Member Federation. A copy of the insurance policy will be provided by your Club's Member Federation upon request, including details of the applicable benefits (such

as death and capital benefits, loss of income and non-Medicare medical expenses), exclusions, maximum benefits and deductibles; e) Acknowledge that if you suffer any injury (including death) whilst participating in football activities and a cause of injury was the negligence, assault or breach of other obligations imposed by laws, then the liability of Perth Glory FC (including each of their directors, officers, employees, agents, contractors and assigns) is limited to the extent of any applicable insurance policy held by Perth Glory FC and the amount which such insurer pays under the policy or applicable statutory insurance scheme;

11. LOST PROPERTY

Participants will be responsible for their own belongings. Perth Glory FC will not be held responsible for the loss of any property including electronic devices. If you need to contact your child during the program, please call the staff on the mobile number provided in your communications.

4
