Directory of Participating Dentists California Group Dental Managed Dental Plans* Directory of Participating Dentists California DIR_DHMO Directory of Participating Dentists This directory contains the names and locations of the dentists who provide care to Dental HMO/Managed Care plan members. Each one has been pre-screened and each office carefully evaluated to ensure the dentist meets credentialling standards prior to their being accepted into our program. Additionally, these offices are re-credentialed every three years. You can be assured that any office you choose will provide care and service that meets your needs. How to find a dentist in your area Participating dentists are listed alphabetically by the city where they are located. Once you have found one near your home or office, include the Facility Number in the indicated space on your enrollment form. This is the dentist who will provide your basic dental care. Each enrolled person in your family may choose a different dentist. (Please be sure to select two participating dentists. If your first choice is unable to accept new members at this time, you will have an alternate to help ensure your access to care is not delayed.) Specialty care dentists (periodontists, endodontists, oral surgeons, orthodontists and pedodontists) are available through a referral process outlined in your Schedule of Benefits. If your current dentist does not appear in this directory, we would be happy to contact the practice with information about joining our network. You can nominate a dentist conveniently online through the MyBenefits Web site or by calling Customer Service at 1-800-880-1800. To submit a nomination online, just log on to www.metlife.com/ mybenefits and click the “Find a Dentist” link. Once submitted, we will contact that dentist and provide them with an application to join the dental network. Some dental offices are unable to accept new members at this time; they are noted in the Directory with an asterisk (*). If you are already a patient at a facility that is listed as closed, you may not have to change dentists. However, it is important that you contact Customer Service to ensure that you can continue using your current facility under the plan. For the most current information… This directory is updated frequently — you will find the most current listing on the MyBenefits Web site at www.metlife.com/mybenefits. Just click on the “Find a Dentist” link, select “DHMO/Managed Care” for the Plan Type and complete all of the required information (please refer to the Schedule of Benefits for the plan name). Once you have enrolled, you may change dentists for you and your enrolled dependents by: 1. Visiting the MyBenefits Web site at www.metlife.com/mybenefits and clicking on the “My Dentists” page. 2. Contacting Customer Service at 1-800-880-1800. Your transfer will be effective the first of the following month. Please note: any requests made after the 25th of the month will occur effective the first of the following month (e.g., a facility request changed on March 28th will go into effect on May 1st). Please refer to the Evidence of Coverage for your plan for more information. Any languages spoken in offices that are indicated in this directory are self-reported by the participating dental office. MetLife or its affiliate has not independently verified the language skills indicated by the offices. If you need access to interpretation services, please call 1-800-880-1800.
