La COVID-19 apunta literalmente a los residentes de los hogares de convalecencia: las personas débiles, de la tercera edad y aquellas con varias afecciones médicas que viven juntas bajo un mismo techo. Desafortunadamente, nadie (los hogares de convalecencia, otros proveedores de atención médica ni el Gobierno) pudo predecir la magnitud de esta pandemia. Apoyamos a nuestro personal y nuestras comunidades que están de luto y les damos nuestras condolencias más sentidas a las familias que han perdido a sus seres queridos. Nunca olvidaremos a aquellos que esta pandemia nos arrebató ni al heroico personal que se mantuvo con valentía en la primera línea para ayudar a proteger a los más vulnerables de nuestra sociedad contra un enemigo tan feroz y letal. También agradecemos el liderazgo del gobernador Cuomo en este momento crítico. En Isabella Center for Rehabilitation and Nursing Care, una organización sin fines de lucro que cuenta con 705 camas y atiende adultos mayores en el Alto Manhattan, estamos comprometidos a brindar atención compasiva y de calidad frente a este desafío, tal como lo hemos hecho los últimos 145 años. Desde el primer brote en marzo, el país ha aprendido más sobre este virus: cómo puede estar presente en aquellas personas asintomáticas, con cuánta facilidad se puede propagar y cuán frecuente era mucho antes de que alguien lo supiera. Esto ha sido fundamental para entender su impacto en los residentes de hogares de convalecencia y para limitar su propagación a futuro. De acuerdo con lo que hemos aprendido, creemos que hay cuatro recomendaciones importantes que deberían implementarse: 1. Los hogares de convalecencia deben tener un lugar en la discusión. Además de la evidente vulnerabilidad de nuestros residentes, somos los únicos que podemos hablar con conocimiento de primera mano sobre el impacto de la pandemia en los hogares de convalecencia, sus residentes y su personal. Los hogares de convalecencia son una parte fundamental del ciclo de la atención médica. Todos los hogares de convalecencia, especialmente aquellos sin fines de lucro, deberían ser actores clave en la planificación futura para esta crisis de atención médica y las que vendrán, así como de un sistema resiliente de atención médica. 2. Los hogares de convalecencia necesitan urgentemente más fondos para brindar una atención adecuada para la población más vulnerable de la sociedad. La pandemia ha devastado la situación financiera de los hogares de convalecencia, en particular la de aquellos sin fines de lucro, que ya tenían grandes dificultades financieras antes de la pandemia. Los hogares de convalecencia, que han sido excluidos de la recepción de fondos federales de cualquier tamaño para enfrentar la pandemia, necesitan desesperadamente y en el corto plazo fondos considerables del Gobierno nacional para ayudar a proteger a nuestros residentes y nuestro personal durante esta pandemia. Asimismo, necesitan mejores tarifas de Medicaid y Medicare a largo plazo para sostener la calidad de la atención y mantener las medidas intensificadas de control de la infección. Los fondos para los centros de atención a largo plazo son clave para limitar la propagación del virus y para proteger a los residentes y el personal vulnerables. Para lograrlo, necesitamos el apoyo del presidente Trump y de la delegación del Congreso del Estado de New York. 3. Todos los hogares de convalecencia deberían tener acceso a pruebas y reservas de equipos de protección personal (PPE). Como todos los demás hogares de convalecencia en la ciudad de New York y en cualquier lugar del estado, Isabella tuvo acceso muy limitado a las pruebas en el establecimiento, lo que impidió que pudiéramos diagnosticar de forma rápida a nuestros residentes y personal. En reconocimiento a la necesidad crucial, el estado está priorizando el acceso ininterrumpido a las pruebas para los hogares de convalecencia de New York. Asimismo, a pesar de que tenemos PPE para nuestro personal, los hogares de convalecencia, junto con los hospitales, necesitan estar protegidos bajo la autoridad del gobernador para contar con reservas de PPE para 90 días y así estar preparados para brotes futuros. 4. Todos los centros de atención a largo plazo de New York deberían informar datos precisos sobre la COVID-19 al Departamento de Salud. Desde el comienzo de esta pandemia, Isabella ha informado con precisión y en su totalidad los datos al Departamento de Salud y ha compartido a diario la cantidad de casos confirmados y presuntos casos positivos en el hogar de convalecencia, incluidas las muertes. Todos los hogares de convalecencia del estado de New York deberían proporcionar datos relacionados con la COVID-19 con precisión y en su totalidad al Departamento de Salud de manera oportuna. Es de vital importancia que los epidemiólogos cuenten con datos exhaustivos para que puedan aconsejar de forma eficaz cómo combatir a este virus en caso de que regrese, y cómo actuar en futuras pandemias. A largo plazo, basar la planificación a futuro y la toma de decisiones en estos datos será la mejor manera de proteger a los residentes y al personal de los centros de atención a largo plazo. Isabella y los hogares de convalecencia de todo el país continuarán desempeñando un papel fundamental en nuestro sistema de atención médica. Para hacerle frente a este desafío, resulta claro que esta nación deberá transformar la manera en que concibe y financia los hogares de convalecencia como proveedores dedicados a la atención esencial de los más vulnerables entre nosotros.
