The development area for this project is in an industrial park located approximately 3 km from the Hirakata-higashi and Hirakata Gakken interchanges on the Second Keihan Highway, and approximately 1.5 km from Nagao Station on the JR Katamachi Line. The location is suitable for deliveries to the Osaka and Kyoto areas, being located approximately 3 km from National Route 1, a major highway connecting Kyoto and Osaka. Moreover, from the business continuity planning perspective, the Kansai inland area is highly sought-after and considered scarce land resources suitable for logistics facility development. The project includes a whole-building lease to OTT Logistics Co., Ltd. The five-story building (four stories in the warehouse section) will have a gross area of 20,398.12 m2 on a site of 10,629.36 m2. Construction will commence in September 2014, and is scheduled for completion in July 2015. Health Economics Review 2013, 3:31 http://www.healtheconomicsreview.com/content/3/1/31 RESEARCH Open Access Impact of specialization in gynecology and obstetrics departments on pregnant women’s choice of maternity institutions Yoshimi Adachi1*, Hiroyasu Iso1, Junyi Shen2, Kanami Ban3, On Fukui4, Hiroyuki Hashimoto5, Takako Nakashima6, Kenichiro Morishige7 and Tatuyoshi Saijo8 Abstract In April 2008, specialization in gynecology and obstetrics departments was introduced in the Sennan area of Osaka prefecture in Japan that aimed at solving the problems of regional provisions of obstetrics services (e.g., shortage of obstetricians, overworking of obstetricians, and provision of specialist maternity services for high-risk pregnancies). Under this specialization, the gynecology and obstetrics departments in two city hospitals were combined and reconstructed into two centers, i.e., the gynecological care center in Kaizuka City Hospital and the prenatal care center in Izumisano City Hospital. This paper investigates to what extent and how this specialization affected pregnant women’s choices of the prenatal care center and other maternity institutions. We used birth certificate data of 15,927 newborns from the Sennan area between April 1, 2007 and March 30, 2010, for Before and After Analysis to examine changes in pregnant women’s choices of maternity institutions before and after the specialization was instituted. Our results indicated that this specialization scheme was, to some extent, successful on the basis of providing maternity services for high-risk pregnancies at the prenatal care center (i.e., Izumisano City Hospital) and having created a positive effect by pregnant women to other facilities in the nearby area.

Wakayama In Osaka Prefecture, Neonatal Mutual Coop- erative System (NMCS) was established in 1977, and then Obstetric & Gynecologic Cooperative System (OGCS) in 1978. Under OGCS, six hospitals (namely Osaka Medical Center and Research Institute for Maternal and Child Health, Osaka City General Hospital, Takatsuki General Hospital, Aizenbashi Hospital, Kansai Medical University Hirakata Hospital, and Yodogawa Christian Hospital) are designated as Base Hospitals, plus, nine other facilities are designated as Sub-base Hospitals. These hospitals play a large role in the Osaka’s regional perinatal healthcare system. In 1999, Osaka Council on Perinatal Health- Tokyo care Strategies was established, and in December of that year Osaka Medical Center and Research 500 km Institute for Maternal and Child Health was des- ignated as the general perinatal medical center for Osaka Prefecture. We examined the heterogeneity in the cardiac performance of patients with HFpEF by clarifying the relationship between the indices of left atrial (LA) volume (LAV) overload and pressure overload with echocardiography. We enrolled patients with HFpEF (N = 105) who underwent transthoracic echocardiography during stable sinus rhythm. Relative LAV overload was evaluated using the LAV index or stroke volume (SV)/LAV ratio. Relative LA pressure overload was estimated using E/e’ or the afterload-integrated index of left ventricular (LV) diastolic function: diastolic elastance (Ed)/arterial elastance (Ea) ratio = (E/e’)/(0.9 × systolic blood pressure). The logarithmic value of the N-terminal pro-brain natriuretic peptide was associated with SV/LAV (r = −0.214, p = 0.033). The pulmonary capillary wedge pressure was positively correlated to Ed/Ea (r = 0.403, p = 0.005). SV/LAV was negatively correlated to Ed/Ea (r = −0.292, p = 0.002), with no observed between-sex diferences. The correlations between the LAV index and E/e’ and Ed/Ea and between SV/LAV and E/e’ were less prominent than the abovementioned relationships.

The daytime temperatures observed on 10 HT events in this area were the highest in the southern area of Kyoto [area with no Automated Meteorological Data Acquisition System (AMeDAS) observation sites]. To quantitatively evaluate the formation mecha- nisms of HT events, a heat budget analysis on an atmospheric column was conducted using the Weather Research and Forecasting (WRF) model. The results showed that over the HT area the daytime column temperature increased as a result of sensible-heat diffusion generated from the urban surface at the contri- bution rate of 54% and as a result of the sensible-heat advection and diffusion supplied from the sides and at the top of the column at the rate of 46% of all sensible heat supplied. To clarify previously unreported effects of synoptic-scale winds under typical summer pressure patterns on the HT events, a sensitivity experiment with no surface heat fluxes, backward trajectory analysis, and Euler forward tracer analysis was performed.
