Blood Bank Lab Quick Reference Ordering Guide Test Name: Blood Type-Group & RH (Nemonic: GROUP RH) When To Order: This test is Generally ordered in the ER on Pregnant female patients who are experiencing vaginal bleeding to determine if she is eligible for RH Immune Globulin (RhoGAM). When Not To Order: This test Should Not be ordered if the patient may need blood products. If the administering of blood products may be required, order a Type and Hold (TYHD) instead. Additional Notes: Only order this test if RhoGAM May Be Needed. The RhoGAM Can Only be obtained from the Blood Bank Lab. Test Name: Type & Screen/Hold ( Nemonic: TYHD) When To Order: This test should be ordered if blood is Not Needed But Might Be Needed Later (i.e. surgical patient not expected to need a blood transfusion during surgery). This test will only ensure that the patient's blood type is de- termined and a basic blood antibody screen is completed. No Blood Will Be Setup For Transfusion. Blood Bank Armband: The blood bank armband for a TYHD Will Expire At Mid- night On The 3rd Day after the patient's blood has been drawn. If the blood was drawn after 9pm, the computer will add 1 More Day to the expiration date. The date of draw is day 1. Additional Notes: If units are needed after a TYHD has been ordered, Do Not Order A Type And Crossmatch. Please Call The Blood Bank At 4-2610 To Obtain Blood Units. Test Name: Type & Crossmatch ( Nemonic: XM) When to Order: This test should be ordered if blood is Needed Initially once the sample is drawn or if there is a Strong Likelihood that blood will be needed during a procedure (i.e. surgery). This test will ensure that the patient's blood type is determined, a basic blood antibody screen is completed and donor blood units are setup ready for use. Additional Notes: After the initial donor units are administered and additional units are need, Do Not Order Another XM. Please Call The Blood Bank At 4-2610 To Obtain Additional Units. Blood Bank Armband: The blood bank armband for a XM Will Expire At Mid- night On The 3rd Day after the patient's blood has been drawn. If the blood was drawn after 9pm, the computer will add 1 More Day to the expiration date. The date of draw is day 1. Test Name: Blood Bank Platelets (Nemonic: PLT) When To Order: Platelets (PLT) component products can Only Have 1 Computer Order Per Day. Please call the Blood Bank each time PLT are needed throughout the day after the initial computer order has been completed. Additional Notes: One pheresis pack of platelets is equivalent to 6 to 10 random units of platelets. Only Pheresis Packs Of Platelets Will Be Given. Example: If you require 4 platelet units, you will receive 1 pheresis pack. If you require 12 units of platelets, you will receive 2 pheresis packs. Please Call The Blood Bank At 4-2610 To Obtain Additional Units. Blood Bank Armband: The blood bank armband Will Not Expire for platelets pheresis packs provided that the armband remains Legible And On The Patient. Blood Bank Lab Quick Reference Ordering Guide Ordering Guide For Blood Bank Fresh Frozen Plasma (FFP) When To Order: Plasma (FFP) component products can Only Have 1 Computer Order Per Day. Please call the Blood Bank each time FFP are needed through- out the day after the initial computer order has been completed. Additional Notes: Please Call The Blood Bank At 4-2610 At Least 30-40 Minutes before coming to pick up the units so that the FFP can be thawed appropriately. Once thawed, the FFP Will Expire Within 24 Hours. Blood Bank Armband: The blood bank armband Will Not Expire for FFP provided that the armband remains Legible And On The Patient. Ordering Guide For Blood Bank Cryoprecipitate (CRYO) When To Order: Cryoprecipitate (CRYO) component products can Only Have 1 Computer Order Per Day. Please call the Blood Bank each time CRYO are needed throughout the day after the initial computer order has been completed. Additional Notes: Please Call The Blood Bank At 4-2610 as to the number of CRYO to be pooled. Example: If you require 1 CRYO, you will receive 10cc. If you require 5 CRYO, you will receive 50cc. Please Confirm With The Physician As To How Many CC Are Needed. Once pooled, the CRYO Will Expire Within 4 Hours. Please Call The Blood Bank At 4-2610 To Obtain Additional Units. Blood Bank Armband: The blood bank armband Will Not Expire for CRYO provided that the armband remains Legible and On The Patient. Blood Bank (BB) Lab Draws Part 1 Important Notes: The Pink tube Must Always be labeled at bedside at the time of draw. Complete the blood bank armband using the patient information in its entirety as it appears on the patient hospital armband (i.e. Doe, John Caring). Collect sample in the Tall 6 mL Pink Top Tube. Only A Minimum draw of 2mL of blood is required. However, it is Highly Recommended that the Full 6 mL of blood is collected to maximize testing potential. Place the BB specimen label from the BB Armband over the Pre-existing Label on the Tall 6 mL Pink Top Tube. Place the BB Armband Number Tail Label on the Opposite Side of the BB Armband Specimen Label on the tube. Do Not Place Any Other Labels/Stickers On The Pink Top Tube. See Reference Pictures Below. Blood Bank (BB) Lab Draws Part 2 Important Notes: Record your employee #, time & date (MM/DD/YYYY) of collection on the Lab Label. Do Not Place Any Piece Of The Lab Label On The Sample Or It Will Be Rejected. Place the Lab Label in the outside pouch of the biohazard bag. Hemolyzed Samples Mask Test Reactions/Results And Therefore Will Always Be Rejected. Hemolyzed Samples are Often associated with IV start or traumatic draws (i.e. pulling hard on a syringe, slow blood flow into the tube).
