Health Warranty and Purchase Agreement

This contract is between Suncoast Goldendoodles representative (Hereafter ​ ​ ​ “Seller”) and (Hereafter “Buyer”) and is entered on this date: / /2020 Jane & John Doe ​XX XX Pertaining to: Goldendoodle Puppy ID Collar Color: Collar ​ ​ ​ XXX Born on: / /2020 Gender: ​XX XX ​ ​ XXXXX Sire Name: CKC# ​Stud Dame Name: CKC# ​Female Dog Sold as Pet Companion Only Total Sale Price: $ , 00.00 ​ ​ ​ ​ X X Seller's Responsibility: 1. Suncoast Goldendoodles guarantees the puppy is in good health, has had its first set of immunizations (DHPP), has been dewormed, and has been tested negative for worms. The buyer has three days (72 hours) to take the puppy to a veterinarian ​ ​ for an examination. If the puppy is found to be in poor health, the buyer has the option of returning the puppy to the seller, at buyer’s cost, for a full refund. The buyer will provide a written statement from a licensed veterinarian clearly stating defect or illness. If the puppy is not taken to a licensed veterinarian within three business days, the health warranty is void.

2. Suncoast Goldendoodles provides this warranty for 24 months from birth against any debilitating congenital defects as ​ ​ long as the puppy/dog maintains the following for the first 8-12 months: 1. The owner will avoid any physical repetitive activity such as stairs, hiking, jogging and excessive exercise with the puppy for the first 8 months of life. 2. Annual Vaccines and check ups are done through a licensed DVM. 3. The puppy is not spayed or neutered before 12 months of age. 4. Diet recommended by the Breeder to the Owner and is fed TLC Pet Food (Puppy or All Stages Dog food). for the first 12 months. The following Genetic diseases are guarantee & warranty for life of the puppy; (Degenerative Myelopathy, Dystrophic Epidermolysis Bullosa, GM2 Gangliosidosis ( Type), Muscular ​ Dystrophy (Golden Type), Neonatal Encephalopathy with Seizures, Osteochondrodysplasia, Osteogenesis Imperfecta (Golden Retriever Type), Progressive Retinal Atrophy (Golden Retriever 1), Progressive Retinal Atrophy (Golden Retriever 2), Progressive Retinal Atrophy (Progressive Rod-Cone Degeneration), Sensory Ataxic Neuropathy, Von Willebrand Disease I) defined as veterinarian recommends euthanasia). In the event the puppy exhibits symptoms of a ​ suspected debilitating congenital defect, the buyer must supply any requested veterinary records to the seller within seven days of informing the seller of the defect. Suncoast Goldendoodles has the right to require a second opinion from a veterinarian of the seller’s choice at the seller’s expense. If the puppy is euthanized or in any way disposed of, without a second opinion from the seller’s veterinarian, the buyer waives all rights stated in contract.

3. This warranty does not include viral illnesses, infections, improper bites, hernias, hypoglycemia, Guardia, or Coccidiosis. It also does not include any illness due to the ingestion of foreign objects, food, chemicals or physical injury.

4. If it is determined that the puppy does have a genetic disorder from the list we genetic test against that is identified from the above list (the buyer has the option of a replacement puppy from the next available litter OR a refund of the original purchase price, minus deposit. Your puppy need not be returned to us provided there is documented support of medical condition through a Genetic Testing Company such as Paw Print Genetics, Embark, Animal Genetics etc.. Shipping fees will not be refunded, and if it becomes necessary to ship a replacement puppy, those fees are to be paid by the buyer.

5. If the puppy should die from a suspected congenital defect within the puppies first 24 months of life, an autopsy must be performed at the buyer’s expense by a licensed veterinarian acceptable to the seller’s own veterinarian. If the autopsy shows a congenital defect as the reason for the death, the seller will provide another puppy of equal value to the buyer, or a refund of the original purchase price, minus deposit. A written statement from the Veterinarian must be provided. This warranty is void in the event of an undetermined or inconclusive diagnosis. *All puppies are tested for a heart murmur at least 2 times by the Seller’s veterinarian before going to new homes. 6. Suncoast Goldendoodles does not cover any form of hypoglycemia (low blood sugar). To avoid hypoglycemia, make sure to offer your puppy food regularly and make sure your puppy doesn't get too cold. Puppies should have unlimited access to water.

7. No warranties or guarantees, expressed or implied, are made under this contract except as stated. For example, coat color or type, non shedding or hypoallergenic, personality, size, weight or ability to perform any discipline.

8. This warranty is non transferable.

Buyer's Responsibility: The buyer agrees to pay Suncoast Goldendoodles a total sum of $ , 00.00 (Deposit of $ 00.00 received will be deducted 1. ​ X X ​ X from the final payment due at pickup in cash or via Good Dog Payment System 3 days prior to pick up - NO EXCEPTIONS) This fee does not include shipping, if shipping is required.

2. The buyer bears all costs relating to the sale of the above animal, including transportation. The risk of loss or injury to the above animal is on the buyer at all times after the dog leaves the seller.

3. The buyer agrees that their Goldendoodle will be housed indoors.

4.. The buyer agrees to make and keep annual veterinary appointments for their Goldendoodle, keep all immunizations current according to the guidelines of the

American Veterinary Medical Association and properly administer heartworm preventative medication.

5. The seller highly recommends providing basic obedience training with their Goldendoodle puppy within the first year of owning their dog. We highly recommend Baxter & Bella online training. The buyer agrees to postpone strenuous, repetitive exercise until after their Goldendoodle is over 12 months of age.

7. The buyer agrees to contact Suncoast Goldendoodles immediately if the buyer determines they are unable to keep their Goldendoodle for any reason. The buyer and Suncoast Goldendoodles will determine how the dog will be rehomed.

8. The buyer agrees to never place the Goldendoodle in a shelter, pound, or similar facility or research laboratory.

9. All Goldendoodle puppies from Suncoast Goldendoodles are sold on a spay/neuter contract unless stated above and breeding rights are paid for. Buyer must send seller proof of spay/neuter by14 months of age of puppy or buyer will be fined 2x ​ the original purchase price of $ 00.00 for breeding rights and be in breach of contract. Buyer will be responsible for all fees ​ XX ​ incurred, including but not limited to lawyer fees, court fees, and travel costs, to enforce this contract.

GENERAL; This written Agreement contains all of the terms & the above conditions of the Puppy Purchase Agreement. By signing below, Buyer states that they have read the Puppy Purchase Agreement & is in complete agreement. Buyer agrees that they understand this contract fully, that this contract is under the jurisdiction of the State of Florida, in Manatee County 12th Circuit Court. It is agreed that the place of venue shall be in Bradenton, FL. The parties acknowledge and agree that this agreement may be executed by electronic signature,which shall be considered as an original signature for all purposes and shall have the sameforce and effect as an original signature.
