HealthPartners AtlasSM PROVIDER DIRECTORY This directory provides a list of HealthPartners® AtlasSM network providers. This directory is for western Wisconsin residents living in Burnett, Douglas, Pierce, Polk or St. Croix counties. This directory is current as of Jan. 1, 2019. Some network providers may have been added or removed from our network after this directory was published. We do not guarantee that each network provider is still accepting new members. Quality HealthPartners uses multiple measures to assess clinic quality including measuring breast cancer screening rates as defined by Minnesota Community Measurement. 4 Atlas network hospital selection standards Experience HealthPartners uses multiple measures to assess member experience with their hospital including how patients rated the hospital overall. Cost HealthPartners’ hospital and surgery center cost assessment compares the cost of a facil- ity including the inpatient and outpatient services provided. The overall cost index is case mix adjusted and place of service case mix adjusted (inpatient vs. outpatient care). The cost index for each facility is indexed to the aggregate 13 county metro Total Cost Index. Anytime you or a member in your family needs care, you may choose to see any provider in this network. If you seek care from a provider who is not listed in this directory, your care is considered out-of-network and may not be covered. Consult your Membership Contract for benefits available to you.
