Proximal Radioulnar Joint Type of joint Pivot type synovial joint Articulating surfaces The head of radius articulates with the radial notch of the ulna Articular capsule The fibrous part blends into the elbow joint The synovial part is continuous with the elbow joint There is also a SACCIFORM RECESS of the joint, a distal extension of it down the radius which allows the radius to rotate without tearing the synovium Ligaments The ANULAR ligament encircles the head of the radius Stability factors The bones articulate well The ANULAR ligament is the main stability factor, preventing dislocation of the radial head. The INTEROSSEOUS MEMBRANE also prevents distraction of the radius The joint is surrounded by muscles eg. brachioradialis and brachialis, which contribute to its stability in a minor way Movements Pronation and supination Supination is the palm turning up, as if to receive alms The axis of rotation passes through the head of radius and through the site of attachment of of radius and ulna distally THE RADIUS IS THE ONE THAT ROTATES the ulna stays stationary Blood supply Supplied by the radial portion of the periarticular arterial anastomosis of the elbow, which is the anastomosis of radial and middle collateral arteries with the radial and recurrent inteosseous arteries Nerve supply Supplied by the musculocutaneous, median and radial nerves, where pronation is work of the median supination is the work of the radial and musculocutaneous nerves .

The patient is asked to extend the elbow and supinate the fore- arm. A slight bending of the patient’s body toward the examined side makes full supination and as- sessment of the anterior compartment easier. Full elbow extension can be obtained by placing a pillow under the joint. Transverse US images are first obtained by sweeping the probe from approximately 5cm above to 5cm below the trochlea-ulna joint, a Pr perpendicular to the humeral shaft. Cranial US images of the supracondylar region reveal the superficial biceps and the deep brachialis mu- Br scles. Alongside and medial to these muscles, follow the brachial artery and the median nerve: * the nerve lies medially to the artery. * Legend: a, brachial artery; arrow, median nerve; arrowheads, distal biceps tendon; asterisks, articular cartilage of the Humerus humeral trochlea; Br, brachialis muscle; Pr, pronator muscle 2 distal biceps tendon: technique The distal biceps tendon is examined while keeping the patient’s forearm in maximal supination to bring the tendon insertion on the radial tuberosity into view.

    Physical and Sports Therapy Hand, Elbow, Wrist Pain The hand is a wondrously complex structure of tiny bones, muscles, ligaments, and tendons which work together to perform tasks. The wrist and elbow are stabilizing joints that support the steady use of the hand and provide attachment points for the muscles that control the hand and wrist. All three of these areas are prone to injury from overuse or trauma. Their complexity requires the skills of an expert for proper rehabilitation from injury. Some Hand, Wrist, and Elbow Issues Include: Tennis/Golfer’s Elbow: Tendonitis, or inflammation of the tendons, at the muscular attachments near the elbow. Symptoms typically include tenderness on the sides of the elbow, which increase with use of the wrist and hand, such as shaking hands or picking up a gallon of milk. Tendonitis responds well to therapy, using eccentric exercise, stretching, and various manual therapy techniques. Carpal Tunnel Syndrome: Compression of the Median Nerve at the hand/base of your wrist. Symptoms include pain, numbness, and tingling of the first three fingers. The condition is well-known for waking people at night. Research supports the use of therapy, particularly in the early phase, for alleviation of the compression through stretching and activity modification. Research indicates that the longer symptoms are present before initiating treatment, the worse the outcome for therapy and surgical intervention due to underlying physiological changes of the nerve. The purpose of this is to keep your muscles primed and ready to generate movement. Several neurological causes may change a person’s muscle tone to increase or decrease resulting in a lack of movement. Over time, a lack of movement can cause stiffness, pain, and spasticity. In severe cases this may also lead to contractures. Spasticity Spasticity can be defined as a tightening or stiffness of the muscle due to increased muscle tone. It can interfere with normal functioning. It can also greatly increase fatigue. However, exercise, properly done, is vital in managing spasticity. The following tips may prove helpful: • Avoid positions that make the spasticity worse • Daily stretching of muscles to their full length will help to manage the tightness of spasticity, and allow for optimal movement • Moving a tight muscle to a new position may result in an increase in spasticity. If this happens, allow a few minutes for the muscles to relax • When exercising, try to keep head straight • Sudden changes in spasticity may

What is the Cubital Tunnel? The cubital tunnel is made up of the bones in your elbow and the forearm muscles which run across the elbow joint. Your ulnar nerve passes through the tunnel to supply sensation to your fingers, and information to the muscles to help move your hand. What causes Cubital Tunnel Syndrome? Symptoms occur when the nerve becomes restricted by pressure within the tunnel. The reason is usually unknown, but possible causes can include: swelling of the lining of the tendons, joint dislocation, fractures or arthritis. Fluid retention during pregnancy can also sometimes cause swelling in the tunnel. Symptoms are made worse by keeping the elbow bent for long periods of time. What are the symptoms? Symptoms include numbness, tingling and/or pain in the arm, hand and/or fingers of the affected side. The symptoms are often felt during the night, but may be noticed during the day when the elbow is bent for long periods of time. You may have noticed a weaker grip, or clumsiness when using your hand.     During the summer months, many people stay active by playing golf or tennis. These sports, however, carry a risk of injury to the tendons – bands of tissue that connect muscles to bones – in the elbow. This month’s AT Corner will explain how these injuries happen, how to treat them if they occur and, most importantly, how to prevent them. Common Elbow Injuries Tendonitis: Inflammation, pain and difficulty using the joint caused by repetitive activities and/or sudden trauma. Tendonosis: A degeneration (breakdown) or tear of tendons which occurs as a result of aging. Symptoms of tendonosis usually last more than a few weeks. Note: Your risk of tendonitis and tendonosis increases with age. They also occur more frequently in those who routinely perform activities that require repetitive movement, as this places greater amounts of stress on the tendons. Tennis elbow: Also referred to as lateral epicondylitis, this condition occurs when there is an injury to the outer elbow tendon. Golfers’ elbow: Also referred to as medial epicondylitis, this condition occurs when there is an injury to the inner elbow tendon. Note: Injuries to these tendons can occur in other sports and activities that use the wrist and forearm muscles. Most times, the dominant arm is the one affected. Symptoms • Pain that spreads from the elbow into the upper arm or down the forearm • Forearm weakness • Pain that can begin suddenly or gradually worsen over time • Difficulty with activities that require arm strength Treatment Over-the-counter medications: NSAIDs, such as ibuprofen (Advil®, Motrin®) and naproxen (Aleve®), or acetaminophen (Tylenol®) can provide pain relief.

    Shoulder, Elbow, Wrist, & Hand Releases This series of exercises for the shoulder, elbow, wrist, and hand release tension, relieve pain, and prevent and alleviate carpal tunnel syndrome, tennis elbow, golfer's elbow, frozen shoulder, and thoracic outlet syndrome. Note: Each of these exercises can be practiced by itself, or as part of another series that you put together. When you do teach this series, it is beneficial to teach the Side Curl and Diagonal Curl first so the student can release their entire pattern of tension on that side of their torso. EXERCISE DESCRIPTION Starting Position: Lie down on the floor on your carpet or exercise mat. Bend your knees and put your feet on the floor, a comfortable hip width distance apart. Rest your hands on the floor by your sides. Bent Arm Rotation: Bend your right arm at a 90 degree angle, and keeping your elbow next to your body, let your right hand rest out to the side on the floor with your palm facing up. Then, keeping your right elbow on the floor, slowly rotate your shoulder inward. Your shoulder will press off the floor and your hand will lift up. You should feel the muscles on the front of your shoulder contracting. Then, slowly release to the count of 16. As you lower down, you should feel the muscles on the front of your shoulder slowly and gently releasing. Repeat this movement if you wish. Now, slide your arm upward on the floor, so that your elbow is pointing straight out from your shoulder. You should still have a 90 degree angle at your elbow.
