IJAAS is a peer-reviewed publication, enlisting the support of an international panel of consulting editors. Each consulting editor was chosen for his or her expertise in one or more areas of interest in aviation. Using the blind-review pro- cess, three or more consulting editors are selected to appraise each article, judging whether or not it meets the requirements of this publication. In addition to an overall appraisal, a Likert scale is used to measure attitudes regarding indi- vidual segments of each article. Articles that are accepted are those that were approved by a majority of judges. Articles that do not meet IJAAS requirements for publication are released back to their author or authors.     Collegiate Aviation Review International Volume 39 | Issue 2 Peer-Reviewed Article #1 6-2-2021 The Impact of Human Factors and Maintenance Documentation on Aviation Safety: An Analysis of 15 Years of Accident Data Through the PEAR Framework Natalie Zimmermann Purdue University Flavio Antonio Coimbra Mendonca Embry-Riddle Aeronautical University Regardless of the type of maintenance performed on aircraft, instructions are to be used to provide the aviation technicians completing the maintenance activities with guidance on, and an outline of, the maintenance items to be performed and completed. However, the use of instructions does not guarantee the correct and proper completion of the maintenance activities as the instructions may be erroneous and/or maintenance personnel can misunderstand, misinterpret, or improperly follow the procedures outlined. Resulting maintenance errors can potentially result in aircraft accidents, as illustrated by Air Midwest Flight 5481. With the purpose of understanding how human factors associated with written maintenance instructions have contributed to aircraft accidents, the researchers qualitatively analyzed, using the people (P), environment (E), actions (A), resources (R) – PEAR – framework, 12 aircraft accidents that occurred from January 1, 2003, through December 31, 2017, under Part 121 or Part 135 operations in the United States that had maintenance instruction-related errors as contributing or causal factors. The detailed accident information, including causal factors, were retrieved from the aircraft accident reports provided by the National Transportation Safety Board (NTSB). The findings indicated that maintenance activities, specifically in terms of the adequacy and proper use of maintenance instructions, are largely impacted by human factor elements, such as the overall organizational environment and the resources available.

    IEEE TRANSACTIONS ON PROFESSIONAL COMMUNICATION, VOL. 47, NO. 4, DECEMBER 2004 233 Teaching the Air Midwest Case: A Stakeholder Approach to Deliberative Technical Rhetoric —MEREDITH W. ZOETEWEY AND JULIE STAGGERS Abstract—What constitutes a cause is a particularly important question for those who teach or study technical writing. This article describes a case that helps students look beyond the technical “causes” of a commuter airplane crash in order to address the complex web of policies, practices, actions, and events that contributed to the crash. Using an approach grounded in stakeholder theory and ethical theory, students use real documents ranging from news accounts to Federal Aviation Administration policies to National Transit Safety Board hearing exhibits to identify systemic problems that contributed to the disaster. Working from particular stakeholder perspectives, they work collaboratively to develop and argue for policy changes that will help prevent future tragedies. The abundance of real documents that drive this case make it an especially useful tool for engaging students in difficult-to-teach subject matter, including the role of writing in the failure of technical systems, deliberative and judicial rhetoric, stakeholder theory, visual rhetoric, and ethics. ALPA’s Economic A and Financial Analysis (E&FA) Department dissected, by the numbers, the last 10 years of the airline industry, putting together a compelling story of inflation, consolidation, and even growth. Putting It in Perspective During the last decade, the average cost of a dozen large Grade A eggs jumped from 91 cents to $1.66, an increase of 82.4 percent. Yet the Air Transport Association (ATA) reports that the average domestic round-trip ticket cost just $1.81 more in 2010 than at the turn of the decade—$316.27 as compared to $314.46 in 2001 (excluding taxes). That’s an increase of just 0.6 percent more.

    1997-2017 Update to FAA Historical Chronology: Civil Aviation and the Federal Government, 1926-1996 (Washington, DC: Federal Aviation Administration, 1998) 1997 January 2, 1997: The Federal Aviation Administration (FAA) issued an airworthiness directive requiring operators to adopt procedures enabling the flight crew to reestablish control of a Boeing 737 experiencing an uncommanded yaw or roll – the phenomenon believed to have brought down USAir Flight 427 at Pittsburgh, Pennsylvania, in 1994. Pilots were told to lower the nose of their aircraft, maximize power, and not attempt to maintain assigned altitudes. (See August 22, 1996; January 15, 1997.) January 6, 1997: Illinois Governor Jim Edgar and Chicago Mayor Richard Daley announced a compromise under which the city would reopen Meigs Field and operate the airport for five years. After that, Chicago would be free to close the airport. (See September 30, 1996.) January 6, 1997: FAA announced the appointment of William Albee as aircraft noise ombudsman, a new position mandated by the Federal Aviation Reauthorization Act of 1996 (Public Law 104-264). (See September 30, 1996; October 28, 1998.) January 7, 1997: Dredging resumed in the search for clues in the TWA Flight 800 crash. The operation had been suspended in mid-December 1996. (See July 17, 1996; May 4, 1997.) January 9, 1997: A Comair Embraer 120 stalled in snowy weather and crashed 18 miles short of Detroit [Michigan] Metropolitan Airport, killing all 29 aboard. Scovel III Inspector General U.S. Department of Transportation Chairman Dorgan, Ranking Member DeMint, and Members of the Subcommittee: We appreciate the opportunity to testify today regarding the Federal Aviation Administration’s (FAA) role in the oversight of air carriers. Ensuring that airlines safely meet the demand for air travel is of paramount importance to the flying public and the national economy; this remains one of the top priorities for the Department of Transportation. Safety is a shared responsibility among FAA, aircraft manufacturers, airlines, and airports. Together, all four form a series of overlapping controls to keep the system safe. The past several years have been one of the safest periods in history for the aviation industry. This is largely due to the dedicated efforts of the professionals within FAA and throughout the industry as well as significant advances in aviation technology. In January, we witnessed a dramatic example of aviation safety at its best when U.S. Airways flight 1549 made an emergency landing in the Hudson River, and, miraculously, all 155 passengers and crew survived due to the skillful efforts of the pilot and crew. However, the tragic accident in February of Colgan flight 3407, which resulted in 50 fatalities, underscores the need for constant vigilance over aviation safety on the part of all stakeholders.

Sevillian [email protected] (386) 846-9293 ABSTRACT In May 2008 the FAA completed an assessment regarding inspections at major airlines. The assessment concluded with findings that the FAA failed to perform more than 100 recommended safety reviews at major air carriers in recent years. According to an article published in the Wall Street Journal, the importance of this examination became clear earlier in 2008 because of revelations that FAA managers allowed Dallas, Texas based Airline Southwest Airlines to fly airplanes that hadn’t undergone mandatory structural safety inspections. The article also stated that The FAA hadn’t reviewed Southwest’s system for complying with agency safety directives since 1999. The researcher will provide a qualitative approach and collect opinion data to support the previous stated questions. The researcher also intends to research possible examples of more efficient ways for the FAA and the Airline industry to manage and communicate issues that could possibly lead to deviations in federal regulations, and that could subsequently lead to an incident or accident. Since the Air Transportation Oversight System (ATOS) program is designed to support the airline regarding possible issues within their perspective departments, the interface with the FAA ATOS program needs to be reviewed to determine if the ATOS program should be voluntary or mandatory.

    1 1997-2012 Update to FAA Historical Chronology: Civil Aviation and the Federal Government, 1926-1996 (Washington, DC: Federal Aviation Administration, 1998) 1997 January 2, 1997: The Federal Aviation Administration (FAA) issued an airworthiness directive requiring operators to adopt procedures enabling the flight crew to reestablish control of a Boeing 737 experiencing an uncommanded yaw or roll – the phenomenon believed to have brought down USAir Flight 427 at Pittsburgh, Pennsylvania, in 1994. Pilots were told to lower the nose of their aircraft, maximize power, and not attempt to maintain assigned altitudes. (See August 22, 1996; January 15, 1997.) January 6, 1997: Illinois Governor Jim Edgar and Chicago Mayor Richard Daley announced a compromise under which the city would reopen Meigs Field and operate the airport for five years. After that, Chicago would be free to close the airport. January 6, 1997: FAA announced the appointment of William Albee as aircraft noise ombudsman, a new position mandated by the Federal Aviation Reauthorization Act of 1996 (Public Law 104-264). (See September 30, 1996.) January 7, 1997: Dredging resumed in the search for clues in the TWA Flight 800 crash. The operation had been suspended in mid-December 1996. (See July 17, 1996; May 4, 1997.) January 9, 1997: A Comair Embraer 120 stalled in snowy weather and crashed 18 miles short of Detroit [Michigan] Metropolitan Airport, killing all 29 aboard. In the wake of the Pan Am Flight 103 bombing and the USAir Flight 427, ValuJet Flight 592, and TWA Flight 800 accidents, the House of Representatives overwhelmingly passed the Aviation Disaster Family Assistance Act of 1996 (ADFAA).1 With only minor revisions, this bill was enacted as Title VII of the Federal Aviation Reauthorization Act of 1996.2 The ADFAA, codified at 49 U.S.C. §§ 1136 and 41113, requires the National Transportation Safety Board (NTSB) and individual air carriers to take actions to address the needs of families of passengers involved in aircraft accidents in which there is a major loss of life.
