There is clear mention of SPA support , revalidation and education . Person specification is rel;vant to this post and it is also clear that RSA comments supervision of juniors is expected. My main concern is that it lists 6 current consultants within the department with no clear understanding of WTE or WTE across sites at hospital and Hosptal. This mean that the incombent in the post may or may not have sufficent syuppotr and the expanding serivce may not be covered adequalty . I would expect an indication of WTE and what amount of ciumminty cover is expected or provided before I would fully support this. Once this is completed I would approve this post. As this is inteded to cover retirement I expect these details are already available Good JD with a clear layout and personal specification etc. It was however, the indcative time table did not include start/finish time or RA comments PA allocation for each duty. I wouldalso advice to clarify the duty in more detail i.e., out patient clinic or in patients ward at hospital. And also to explain what particular duty at Hospice the time was allocated for. I noted a day in Lieu for weekend work. Recent strategic developments across the STP have seen the creation of a single integrated medical team for Oncology working across the three acute trusts. The vision for this larger single team will facilitate cross cover, support increased sub specialisation and facilitate individual job plans in line with Royal College Guidance on tumour site cover.

The Oncology team provides the Oncology Service for South Essex from the Oncology Centre based at Southend University Hospital. Outpatient clinics are held at Southend University , Broomfield Hospital (Mid Essex Hospitals NHS Trust) and Basildon & Thurrock University Hospitals. The Department is referred in excess of 2,500 new patients per annum.

The Oncology service within Southend Hospital is provided in the following locations:-

 In Patients - Elizabeth Loury & Estuary Oncology Wards  Out Patients – Centre for Clinical Oncology  Chemotherapy Treatment Unit  Radiotherapy Department

Elizabeth Loury Ward comprises 22 beds, with 20 beds on Estuary Ward. Elizabeth Loury Ward includes a purpose built suite of seven en-suite side rooms and a radio-active iodine suite.

The Chemotherapy Treatment Unit (CTU) moved to a new purpose built facility in May 2010. The CTU has twenty-two chairs and two side rooms for treatment and two consulting rooms. There is an Oncology Nurse Consultant, who is also the Lead Cancer Nurse for the Trust, and undertakes oral chemotherapy clinics and provides support and expert advice to the chemotherapy nurses. The CTU is managed by a chemotherapy Clinical Nurse Specialist supported by 15 WTE chemotherapy nurses of varying grades, 3 HCAs and clerical staff. Within the building is a MHRA licensed cytotoxic reconstruction unit staffed specialist oncology pharmacists and technicians. All solid tumour chemotherapy for South Essex is delivered at Southend Hospital.

The Radiotherapy Department is compromised of the following equipment:-

 3 Elekta Linear Accelerators (plus one service continuity machine) with 6 & 10MV photon energies, electron treatment capabilities, electronic portal imaging and multileaf collimation. All 3 deliver external beam Radiotherapy using IGRT and IMRT.  The department implemented VMAT in 2013.  Nucletron microselectron HDR Brachytherapy Unit – a dedicated theatre facility opened in 2016.  Nucletron Simulix Simulator with CT scanning facility  Toshiba wide-bore dedicated CT-Sim.  The Oncology Management System is IMPAC, MOSAIQ  Mould room suite  XIO CMS and Monaco planning system used for external beam radiotherapy.  Prosoma Virtual Simulation

The Oncology Centre is situated in a two storey building which includes:

Consultants are responsible for the care of patients and for the clinical activities of the Department; undertake the administrative duties associated with the care of their patients and the running of their clinic. All consultants are expected to assume responsibility for the management of junior medical staff. This includes the professional development of their trainees.

Oncology Junior Medical Staff at SUHFT complement is:-

Departmental Clinical Links

There are historically very strong links between the Oncology Department and all other departments at Broomfield (MEHT), Basildon and Southend University Hospitals.

There are CT & MRI facilities available on site and a static PET scanner operated by Alliance medical.

There are a number of weekly and monthly joint clinics in both Trusts in sub-specialities such as head and neck cancer, gynaecological cancer, urological cancer, thyroid cancer & clinical haematology. There are also weekly multidisciplinary meetings on both sites in all tumour types.

The newly appointed consultant would be expected to take part in the relevant joint clinics and sub-groups.

Governance Arrangements

There are regular audit, mortality and morbidity, and clinical governance meetings for oncology, haematology and palliative care services. The post holder will be expected to attend these meetings and participate in the presentation of audits and any actions arising from the meetings.

Fairhavens Hospice

Fair Havens has been serving the local community since 1983. It has grown significantly and now Havens Hospices is a charity, receiving some NHS grants, with a very active board of trustees. We employ a CEO and board of directors, with good working relationships between them and the Clinical Lead. There is a well-developed clinical governance structure including quality improvement and audit plans, medicines management and pharmacy support. The post holder will be joining at an exciting time of further expansion. The clinical lead will have a unique opportunity to have a key role in shaping these services and deliver quality end of life care.

In 2020, we will be opening our new adult hospice in Prittlewell. With its welcoming and reassuring environment, Fair Havens will cover the population of Southend, Castle Point and Rochford, bringing together a range of specialist services to support people in making choices about their care from the day they receive their diagnosis, as they live with their illness, and as the end of their life approaches.

This larger, modern hospice can care for twice as many patients, offer more treatments and support more families in a welcoming and therapeutic environment which contrasts with the stark, clinical atmosphere many people want to avoid. It will incorporate up to 16 in-patient bedrooms, clinic rooms, increased Day Hospice services, additional family support and beautifully landscaped gardens and grounds.

Southend University Hospital is looking to appoint two dynamic and enthusiastic Consultants to join our Specialist Palliative Care Team.

The successful candidates will support the provision of a seamless specialist palliative care and end of life care service across South Essex.

The services provide Specialist Palliative Care to the adult population of South Essex. The team is currently comprised of seven Consultants in Palliative Medicine, each with lead responsibility for one clinical area but cross covering and providing on call cover to all areas.

One of the posts also includes working with the local hospice, Fairhavens. The successful candidate will also be expected to represent Fairhavens Hospice in regional palliative care networks. This is an exciting opportunity to be involved with the hospice as it begins its journey into a brand new, £16.5 million purpose built unit at the heart of the community.

 Provide specialist advice in the field of Palliative Medicine to clinical staff within the community, hospital or hospice.  Provide rapid assessment of patients referred by GP’s, Consultants or Community Palliative Care services through outpatient clinic, domiciliary visiting and review in day care or admission, liaising closely with the GP and primary care team in their management.  Accept ongoing responsibility for patients admitted under the Palliative Care services within the hospitals or hospices.  Share in the night and weekend on call rota and provide cover for holiday and study leave of colleagues by mutual agreement.  Participate in the delivery of specialist education to local medical and other clinical staff.  Lead on embedding evidence based clinical practice to Fairhavens Hospice.

Organisational

 To promote interdisciplinary working between relevant palliative care providers.  Consultants undertake the administrative duties associated with the running of the department and are responsible both single, and corporately, for the management of junior medical staff, their training and professional development.  The successful candidate is expected to concern themselves with the professional development, both clinical and personal, of their trainees.  The successful candidate will be required to undertake an active part in both post-and under-graduate teaching within the Department, hospital and hospice as a whole. Active research is encouraged.  The successful candidate will be expected to partake in hospice and hospital wide audit. Formal audit half-day meetings occur monthly.  Work with the Director of Care at the hospice to explore the development of new innovative services.

Job Plan

The Clinical Lead for Palliative care and General Manager for Oncology at Southend Hospital are responsible for the review of the job plan in conjunction with the post-holders.

A formal job plan will be agreed with the appointees, the Clinical Lead and General Manager on behalf of the Medical Director, three months after the commencement date of the appointee. The job plan for the first three months will be based on the provisional timetables shown below. The job plan will then be reviewed at least annually, following an appraisal meeting.

The job plan will be a prospective agreement that sets out the consultant’s duties, responsibilities and objectives for the coming year. It should cover all aspects of the consultant’s professional practice, including clinical work, teaching, research, education and managerial responsibilities. It should provide a clear schedule of commitments, both internal and external. In addition it should include personal objectives, including details of their link to wider service objectives and details of the support required by the consultant to fulfil the job plan and the objectives. If the appointee takes on additional teaching or educational supervisory roles, these will attract an additional supporting professional activity payment.

The successful candidate has a continued responsibility for patients in his or her care. Proper clinical records must be kept and good communications maintained with patients and their General Practitioners in the form of timely discharge summaries and clinic letters.

Management responsibilities will be by arrangement and agreement between the Clinical Lead, the Departmental Chairman, the Clinical Tutor and the appointee.

Clinical support

There are presently two full time SpRs based in South Essex from the Eastern Deanery rotation in Palliative Medicine. The SpR posts are one year posts in each of the acute Trusts.

The other shared medical staff at Southend University Hospital comprise four FY1 posts, two GP VTS speciality training post and three permanent Staff Grade shared between the Palliative Medicine, Oncology and Haematology teams (the latter also have SpRs / ST3s). The beds at

Basildon University Hospital are supported by a FY1 post and one GP VTS speciality training post.

Fairhavens and St Lukes hospice both have one and a half GP VTS speciality training doctor.

The consultant would take responsibility for supervising the trainees when they are working in an area where they have lead clinical responsibility. Both the independent hospice trusts are committed to providing specialist palliative care and hence have consultant level clinical leadership. The area is committed to implementing all end of life guidance. All new and complex cases are reviewed at a weekly MDT in each locality (Southend and Basildon).

On call

The on call rota will be 1 in 7 for the Palliative Care services. Both hospitals have junior staff cover. The hospices require first on call cover during the week and second on at the weekend, but this is under review. There will be consideration of time off in lieu if the on call led to long periods in any unit.

Annual leave and study leave will require agreement and cover from within the senior medical team. Generally no more than two are away at the same time. Clinical Governance

The post holder will provide clinical services in line with the Trust’s clinical governance arrangements which are designed to ensure that agreed quality standards are achieved. These requirements includes:-

 Audit – The Palliative care department has a full and varied clinical audit program, including both departmental and multidisciplinary audit. The appointee will be expected to contribute to this and will be encouraged to play an active role in the audit programme.

 Continuing medical education – Clinical governance and professional revalidation, requires the appointee to practice to a high standard and to maintain that standard. To achieve this, the appointee will be expected to participate in the CPD scheme of the Royal College of Radiologists (RCR) and will be responsible for maintaining a portfolio to include evidence of structured learning and a personal CPD action plan, which can be used as a basis for annual professional review/appraisal. This Trust supports the concepts of CPD, clinical audit and External QA and actively encourages all consultants to participate in this. It currently provides £1000 per annum and thirty days per three years to support each consultant's CPD activities.

 Risk management – To work within the Trust’s Clinical Risk Management Policies and in particular the individual is expected to follow the Trusts procedures with regards to clinical incident reporting. Concerns and issues can also be raised to the Medical Director via the Clinical Lead in writing or by email. A bi-monthly communications board is also set up in the department.

 Appraisal – All consultants in the Trust must participate in the Trust’s appraisal process. In lines with the Trust’s guidance consultants will be able to select their appraiser from a pool of trained appraisers within the Trust.

Teaching & Training Activities

There is an expectation that part of this job should involve on-going teaching and training of primary care colleagues through group teaching and practice-based sessions in order to disseminate specialist skills and provide a forum for the discussion of case studies and set clinical guidance.

Southend Hospital has University status. Consequently the training of medical students, junior hospital doctors, nurses, physiotherapists and other health care professionals has a high priority. The new colleague will be actively encouraged to pursue any interest in this area.

Southend Hospital has a dedicated clinical governance department with allocated clinical and administrative staff to facilitate this.

There is a well-stocked medical library with audio-visual teaching aids, computer equipment and a full time librarian in the individual sites. There is a thriving Postgraduate Medical Centre with regular clinical meetings and a general education programme.

All consultants are expected to take advantage of study leave as appropriate to maintain and develop their clinical skills to comply with CME requirements.

Other Matters

The list of duties given above is not an exhaustive list and the post holder may be asked to adjust duties to meet the needs of the service. The job description reflects the core activity of the post at a particular time, and that as the department and the individual develop there will inevitably be changes in the emphasis and duties of the post. All new consultants will be provided with a mentor from the consultant body in order to aid professional development and provide advice.

Residence

The successful candidates will be required to maintain his/her residence within easy reach and not more than 10 miles by road from their base hospital though alternative arrangements may be approved by the Clinical Director.

Office, Administrative Support and IT Support

The consultant would have office space at Fair Havens which would provide secretarial support. Each office has desk space, telephone and computer terminals. There would also be desk space available in the Trust.

Both hospital offices have a small library of specialist media.

Main Terms and Conditions of Service

The appointment will be made by the Board on the recommendation of an Advisory Appointments Committee, constituted in terms of the Guidance on Advisory Appointments. Any person suitably qualified and experienced who is unable for personal reasons to work full- time, will be eligible to be considered for the post.

The appointment is subject to Terms and Conditions, Consultants (England) 2003. It is subject to the National Health Service (Superannuation) Regulations. The conditions are exclusive of appeal rights to the Secretary of State (Section 190).

The starting salary for new Consultants is £75,249 per annum.

Registration

The person appointed will be required to be fully registered with the General Medical Council and hold a licence to practice.

Rehabilitation of Offenders Act

Because of the nature of the work of this post, it is exempt from the provision of section 4(2) of the Rehabilitation of Offenders Act 1974 (Exemption Order 1975). Annual and Study Leave

The annual leave is 30 working days plus two statutory day holidays.

All consultants are expected to take advantage of study leave to maintain and develop their clinical skills to comply with CME requirements.

Study leave is available as provided for under the Terms and Conditions of Service and Hospital Medical and Dental Staff. Study leave consists of 30 days over a three year period. A maximum of £1,000 per financial year (1st April – 31st March is payable for full time employees in accordance with the Trust’s study leave guidelines.

Approval of annual and study leave is subject to the procedures set out within the individual departments and the Medical Education Department.

So far as is practical, the Consultant appointed will be expected to share in the provision of cover for the annual and study leave of other Consultants in the specialty. Generally no more than two are away at the same time.

Termination of Contract

Termination of the appointment is subject to three months' notice on either side.

Private Practice

The successful applicant may undertake private practice in accordance with the Schedules 9 & 10 of Terms and Conditions of Service. However where such practice might conflict directly with the Trust’s interests, the appointee must bring this to the attention of the Medical Director. Page | 20

Medical Indemnity

The Trust is financially responsible for the negligent acts and omissions of consultant medical and dental staff in the course of their Trust employment. If, however, any private practice, within a NHS hospital or any other private hospital is undertaken, the appointee will be personally responsible for subscribing to a recognised defence organisation. The Trust will not be responsible for category 2 (e.g. reports for insurance) or ‘Good Samaritan’ Acts. Health Circular HC (89) 34 provides full details of ‘Claims of Medical Negligence against NHS Hospital and Community Doctors and Dentists’.

The Department of Health advises practitioners to maintain defence body membership in order to ensure they are covered for any work which does not fall within the scope of NHS Indemnity.

Prospects for Change

The proposals set out in the High quality care for all: NHS Next Stage Review final report (Darzi report) are likely to impact on current working arrangements. NHS South East Essex has recently published its 5 year strategic plan and the Trust is moving towards a new model of specialty ward based inpatient working. Together these may entail considerable change to the job plans of consultants including the appointee, and to the distribution of junior medical staff. The Trust will consult the members of staff concerned at the appropriate time, but meanwhile wishes to draw the attention of applicants to the possibility of change in the future.

Place of Work

The appointee will be based at Southend University Hospital with regular clinical commitments at Fairhaven’s Hospice. The Trust is working closely with neighbouring organisations within the Mid and South Essex Hospital Group to design the models for future service provision which may require clinical teams and individuals to work at others sites within Mid & South Essex in the future. The appointee will be fully consulted with regards to any potential changes in the future in line with the terms and conditions for Consultants.
