All 4 NMONAT patients had relapses and 2 had an increase of disability. AQP4-ab titers were higher in NMONAT (n 5 9) vs NMOIT (n 5 13; p 5 0.0059). IL-8, IL-1b, and IFN-g serum levels were significantly higher, and CXCL-10 was significantly lower in NMONAT vs NMOIT. Conclusions: Misdiagnosis of NMOSD with MS is rare. NAT was not able to control disease activity in NMONAT patients, who had higher serum levels of AQP4-IgG and proinflammatory cytokines than patients with NMOSD treated with other immunotherapies. Neuromyelitis optica (NMO) is a relapsing autoimmune CNS disease which mainly affects the optic nerves and spinal cord and often leads to severe disability.1 The detection of a serum antibody targeting the astrocytic water channel aquaporin-4 antibody (AQP4-ab)2 led to the definition of revised diagnostic criteria3 and to renaming of the entity as NMO spectrum disorder (NMOSD).4 Although distinction between MS and NMOSD was facilitated by AQP4-ab testing, there is a substantial overlap between clinical phenotypes of MS and NMOSD, which caused diagnostic uncertainty or misdiagnosis, in particular prior to the availability of AQP4-ab testing. Methods We included mAb ADRs that were reported in VigiBase, and identifed those related to depression and suicidal ideation and behavior. Reporting odds ratios (RORs) were estimated for each mAb (bevacizumab as the reference) and according to their infuence on the immune system (not directly targeting [reference], stimulating, or suppressing). Those suppressing the immune system were further divided into their intended indication (auto-immune diseases, cancer). Results Overall, 2,924,319 ADRs for 44 mAbs were included; 9455 ADRs were related to depression and 1770 were related to suicidal ideation and behavior. The association was strongest for natalizumab and belimumab, both for depression (ROR 5.7, 95% confdence interval [CI] 5.0–6.4; and ROR 5.1, 95% CI 4.2–6.2) and suicidal ideation and behavior (ROR 12.0, 95% CI 7.9–18.3; and ROR 20.2, 95% CI 12.4–33.0).

Research in this area is lacking in the previous literature. In a three-year retrospective and clinical–paraclinical study, we compared 173 DMF patients and 94 NTZ patients with a similar average age (40 years) and disease duration (DD) (10 years). Expanded Disability Status Scale (EDSS) scores were higher in the NTZ group than in the DMF group at 3.5 vs. 2.5, respectively (p = 0.001). However, in both groups, age values correlated with DD (r = 0.42; p < 0.001), EDSS (r = 0.52; p < 0.001) and age at onset (r = 0.18; p < 0.001). Furthermore, age-adjusted Kaplan–Meier curves showed that NTZ-treated subjects maintained a 1.0–3.0 EDSS status score (p = 0.003) more frequently and a 3.5–7.0 score (p = 0.022) significantly less frequently compared with DMF-treated subjects.

Ipilimumab, a monoclonal antibody that binds and inhibits cytotoxic T-lymphocyte–associated antigen 4, and nivolumab, a monoclonal antibody that blocks programmed cell-death protein 1, have been approved. As these treatments, respectively, block T-cell inhibition and potentiate the activation of T cells, they may trigger – a number of neurologic immune-related adverse events,1 9 as illustrated here. We describe a 71-year-old woman who was diagnosed in 2014 with stage IV small-cell lung cancer and treated with 6 cycles of cisplatin/etoposide, followed by mediastinal and pro- phylactic cranial irradiation. Because of tumor progression in the lung and liver, she received a first dose of nivolumab 1 mg/kg, followed by ipilimumab 3 mg/kg. On day 4 following administration, she developed short-term memory deficits that worsened over the next 5 days. On admission, she was found to be disoriented in time and space and unable to recall any word after 5 minutes. Autobiographic memory was preserved. She presented dysexecutive disorder and was unable to read or write.
